Computationally guided personalized targeted ablation of persistent atrial fibrillation.

Atrial fibrillation (AF)-the most common arrhythmia-significantly increases the risk of stroke and heart failure. Although catheter ablation can restore normal heart rhythms, patients with persistent AF who develop atrial fibrosis often undergo multiple failed ablations, and thus increased procedural risks. Here, we present personalized computational modelling for the reliable predetermination of ablation targets, which are then used to guide the ablation procedure in patients with persistent AF and atrial fibrosis. First, we show that a computational model of the atria of patients identifies

Model based non-invasive estimation of PV loop from echocardiography.

We introduce a model-based approach for the non-invasive estimation of patient specific, left ventricular PV loops. A lumped parameter circulation model is used, composed of the pulmonary venous circulation, left atrium, left ventricle and the systemic circulation. A fully automated parameter estimation framework is introduced for model personalization, composed of two sequential steps: first, a series of parameters are computed directly, and, next, a fully automatic optimization-based calibration method is employed to iteratively estimate the values of the remaining parameters. The proposed m

Computer modeling and intraoperative navigation in maxillofacial surgery.

PURPOSE: Recent advances in computer-modeling software allow reconstruction of facial symmetry in a virtual environment. This study evaluates the use of preoperative computer modeling and intraoperative navigation to guide reconstruction of the maxillofacial skeleton. METHODS: Three patients with traumatic maxillofacial deformities received preoperative, thin-cut axial CT scans. Three-dimensional reconstructions, virtual osteotomies, and bony reductions were performed using MIMICS planning software (Materialise, Ann Arbor, MI). The original and "repaired" virtual datasets were then imported i

Endoscopic robot-assisted extended thymectomy by subxiphoid approach with sternal lifting: feasibility in the pig.

BACKGROUND: In the search for novel approaches to thoracoscopic thymectomy we assessed the feasibility of a subxiphoid approach using computer-enhanced instruments and sternal lifting. METHODS: In 12 pigs, after lifting of the sternum, ports were placed subxiphoid (stereoscope) and in the left and right fourth intercostal space (instruments). Using computer-enhanced instruments, dissection of the thymus and anterior mediastinal fat pads was started at the diaphragm and continued cephalad. RESULTS: After setup of the robot system (23 +/- 6 min, mean +/- SD), the thymus, including both superio

Neurosyphilis in an eight-year-old child: usefulness of the SPECT study.

An 8-year-old boy with symptomatic late-onset congenital syphilis is reported. The child had been undertreated when he was 78 days of age, but his clinical and serologic follow-up did not occur until he was 3 years of age. At that time, he was asymptomatic, and cerebrospinal fluid disclosed not only hypercellularity but also a reactive Venereal Disease Research Laboratories test. Although he was then retreated at 4 years of age, he developed seizures. Cranial computed tomography and magnetic resonance imaging were normal, but two single photon emission computed tomography scans performed when

Localized hypertrichosis associated with periorbital neurofibroma: clinical findings and differential diagnosis.

BACKGROUND: Congenital localized hypertrichosis in the periorbital region is an uncommon finding. The authors report two patients with hypertrichosis and cutaneous hyperpigmentation overlying a periorbital neurofibroma. METHODS: In addition to a complete ophthalmic and systemic examination, the patients underwent computed tomography of the head and biopsy of the tumor. RESULTS: Case 1 previously had received a diagnosis of neurofibromatosis type I. On examination, hyperpigmentation, hypertrichosis, and swelling in the right supraorbital region were noted. A computed tomographic scan showed a

Attractor Stability in Finite Asynchronous Biological System Models.

We present mathematical techniques for exhaustive studies of long-term dynamics of asynchronous biological system models. Specifically, we extend the notion of [Formula: see text]-equivalence developed for graph dynamical systems to support systematic analysis of all possible attractor configurations that can be generated when varying the asynchronous update order (Macauley and Mortveit in Nonlinearity 22(2):421, 2009). We extend earlier work by Veliz-Cuba and Stigler (J Comput Biol 18(6):783-794, 2011), Goles et al. (Bull Math Biol 75(6):939-966, 2013), and others by comparing long-term dynam

Utility of 18F-FDG PET(/CT) in patients with systemic and localized amyloidosis.

PURPOSE: Amyloidosis is a group of diseases characterized by deposition of fibrils and this deposition may be localized or systemic. The presence of giant cells is typical of localized AL amyloidosis in contrast to systemic amyloidosis. Because of this presence of giant cells we hypothesize that (18)F-fluorodeoxyglucose (FDG) positron emission tomography/computed tomography (PET/CT) may show uptake in localized amyloidosis but not in systemic amyloidosis. The aim of the study was to evaluate the utility of (18)F-FDG PET/CT in distinguishing systemic amyloidosis from localized amyloidosis. MET

Edge-assisted adaptive offloading algorithm for 3D object detection tasks.

Multimodal 3D object detection is crucial for autonomous systems but suffers from high delay due to significant computational demands. To address this, we propose an edge computing-assisted framework that balances load between terminal devices and edge servers. We introduce dynamic threshold tuning and resolution-adaptive offloading algorithms to optimize performance. Experimental results demonstrate that our approach significantly reduces delay by minimizing offloading frequency while maintaining high accuracy, achieving a superior delay-accuracy trade-off. Furthermore, the framework exhibits

Six-Loop Gravitational Interactions at the Sixth Post-Newtonian Order.

We compute the gravitational interaction of two coalescing compact objects at sixth post-Newtonian order in the static limit, employing the diagrammatic approach within the effective field theory framework of general relativity. The calculation requires the evaluation of six-loop Feynman diagrams that are mapped onto two-point integrals with a gauge-theory-like structure, which are computed here for the first time. The resulting seventh-order contribution in Newton's constant is finite in three space dimensions. This result provides the most technically demanding missing ingredient for the det

The error theory of LLM consciousness: there is no evidence that standard LLMs are conscious.

Are chatbots AI zombies or conscious minds? I suspect they are AI zombies, at least when implemented on today's standard hardware. I argue that today's Large Language Mode's (LLM's) consciousness-like behaviors do not suggest they are conscious, because there is an error theory - a theory explaining why they behave as if they are conscious in absence of actual felt experience. LLMs function as "crowdsourced neocortices" - as they scale up, they come to mirror human conceptual structures, leading to human-like behaviors, including those involving consciousness. By contrast, bio-computers, quant

Phase diversity improves retinal image quality in adaptive optics scanning light ophthalmoscopy.

The quality of retinal images is compromised by aberrations that remain uncorrected even in confocal adaptive optics imaging. This study demonstrates phase diversity (PD), a computational imaging technique, to address residual aberrations and enhance image quality in adaptive optics scanning laser ophthalmoscopy (AOSLO). By using images of the same object obtained with and without deliberately added aberrations, PD computes and compensates for the effects of existing residual aberrations beyond those corrected by a closed-loop AO system. Experimental validation demonstrates that PD improves vi

Case of stage 0 pulmonary sarcoidosis pathologically diagnosed via transbronchial lung cryobiopsy.

A 45-year-old male was diagnosed with arrhythmia during a routine health examination. Findings from different modalities, such as echocardiography and radiography, were consistent with cardiac involvement in sarcoidosis. There was no ocular involvement or superficial lymph node enlargement. A chest computed tomography scan did not reveal any pulmonary lesions or bilateral hilar lymphadenopathy. To pathologically diagnose systemic sarcoidosis, transbronchial lung cryobiopsy was performed. Results showed pathological evidence of noncaseating epithelioid granulomas. Herein, we present a case in w

Complex Pulmonary Arteriovenous Fistula Presented With Ventricular Tachycardia and Type 2 Acute Myocardial Infarction.

Pulmonary arteriovenous fistula (PAVF) is a rare congenital vascular malformation primarily manifested as dyspnea, migraine, ischemic stroke, hemoptysis, and nervous system complications. However, in our case, an 18-year-old male patient with PAVF presented with sudden onset of ventricular tachycardia and type 2 acute myocardial infarction as initial symptoms. A diagnosis was achieved through pulmonary artery computer tomography angiography (CTA) and three-dimensional (3D) computed tomography (CT) reconstruction, revealing a complex and giant PAVF. Following multidisciplinary team (MDT) consul

Syphilis With Multiple Pulmonary Nodules and Bone Lesions Detected on Computed Tomography and 18F Fluorodeoxyglucose-Positron Emission Tomography/Computed Tomography.

Syphilis can cause a wide range of systemic manifestations, such as papular rash, malaise, weight loss, muscle aches, generalized lymphadenopathy, and meningitis. However, pulmonary involvement in patients with secondary syphilis is thought to be relatively rare. Moreover, bone involvement in patients with secondary syphilis is also considered rare, and only a few cases of involvement of lung and bone in such patients have been reported. In this paper, we report a case of secondary syphilis with pulmonary involvement in the form of multiple nodules with low attenuation areas, lymphadenopathy a

Asymptomatic but functional paraganglioma of the posterior mediastinum.

A 72-year-old woman was referred to our hospital because of a posterior mediastinal tumor. On the basis of detailed imaging tests, including (123)I-metaiodobenzylguanidine single photon emission computed tomography-computed tomography, and elevated values of catecholamines in the plasma and urine, the tumor was diagnosed as a functional mediastinal paraganglioma even in the absence of symptoms. After preoperative blood pressure control, surgical resection was performed. During the operation, the systemic blood pressure increased transiently as a result of surgical manipulation of the tumor. So

Detection of masses in mammograms via statistically based enhancement, multilevel-thresholding segmentation, and region selection.

A method for automatic detection of mammographic masses is presented. As part of this method, an enhancement algorithm that improves image contrast based on local statistical measures of the mammograms is proposed. After enhancement, regions are segmented via thresholding at multiple levels, and a set of features is computed from each of the segmented regions. A region-ranking system is also presented that identifies the regions most likely to represent abnormalities based on the features computed. The method was tested on 57 mammographic images of masses from the Mini-MIAS database, and achie

An acoustic analysis of the effects of surgical therapy on voice quality.

An automated computer system was used to measure changes in vocal quality resulting from laryngeal surgery. Five acoustic parameters were computed and seven listeners made judgments on vowel samples recorded before and after surgery from 37 patients. Presurgery and postsurgery changes in the five acoustic measures were compared with the perception of change in vocal harshness. A multiple regression analysis showed that four of the five acoustic measures accounted for a large amount of the variance in the judgment of change of voice quality following surgery. The measures may provide a quick an

Development of a system that generates structured reports for chest x-ray radiography.

OBJECTIVES: Radiology reports are typically made in narrative form; this is a barrier to the implementation of advanced applications for data analysis or a decision support. We developed a system that generates structured reports for chest x-ray radiography. METHODS: Based on analyzing existing reports, we determined the fundamental sentence structure of findings as compositions of procedure, region, finding, and diagnosis. We categorized the observation objects into lung, mediastinum, bone, soft tissue, and pleura and chest wall. The terms of region, finding, and diagnosis were associated wi

Reliability of computer-assisted periacetabular osteotomy using a minimally invasive approach.

BACKGROUND: Periacetabular osteotomy (PAO) is the treatment of choice for younger patients with developmental hip dysplasia. The procedure aims to normalize the joint configuration, reduce the peak-pressure, and delay the development of osteoarthritis. The procedure is technically demanding and no previous study has validated the use of computer navigation with a minimally invasive transsartorial approach. METHODS: Computer-assisted PAO was performed on ten patients. Patients underwent pre- and postoperative computed tomography (CT) scanning with a standardized protocol. Preoperative preparat
