Interlocking screws placed with freehand technique and uni-planar image intensification: the "dip-stick" technique.

OBJECTIVE: To report our experience with a novel alternative method of freehand interlocking of intramedullary nails. This method requires the use of only anterior-posterior image intensification and an intramedullary guide wire to verify screw placement. Our results are compared with historical results in the literature. METHODS: A total of 815 patients were treated using this technique from January 2008 to December 2012; 603 patients had fractures of the tibia and 212 had fractures of the femur. RESULTS: The mean duration of surgery for tibial shaft fractures was 55.6 minutes (range 42-60

Functional results of displaced proximal humerus fractures in children treated by elastic stable intramedullary nail.

PURPOSE: Displaced proximal humerus fractures within the pediatric population can be treated by elastic stable intramedullary nailing (ESIN). The main objective of our study is to evaluate functional outcome of the displaced proximal humeral fractures treated by ESIN within the pediatric group using a standardized evaluation scale. The secondary goal is to compare functional outcome of epiphyseal and metaphyseal injuries and functional outcome of children younger and older than 10 years of age. MATERIALS AND METHODS: From March 2010 to December 2011, 27 children had been treated surgically us

The management of isolated closed femoral shaft fractures in a district hospital in Malawi.

UNLABELLED: We prospectively studied the inpatient management of closed femoral shaft fractures in adults over 10 months at Ntcheu District Hospital, Malawi. RESULTS: We had 20 patients, who stayed for an average of 46 days in hospital. The average time to apply traction was 2.3 days. The average time from admission to X-ray was 1.1 days. We recorded 11 complications: seven pinsite infections; two leg length discrepancies of >2 cm; one delayed union; and one malunion. Owing to logistics, we were unable to survey late complications. Check X-rays (available in 17/20 patients) at discharge showe

The validity of ESIN method of osteosynthesis compared to other active surgical methods of treatment of diaphyseal fractures of long bones in children and adolescents.

We investigated the relationship of efficiency in the application of the ESIN method of intramedullary osteosynthesis and other active surgical methods in the treatment of diaphyseal fractures of long bones in children and adolescents. The study comprised 100 subjects treated by elastic stable intramedullary osteosynthesis (ESIN - group A) and 50 subjects in whom other active surgical methods were applied (group B). The following criteria of efficiency of treatment were applied: 1. length of perioperative hospitalization, 2. time elapsed since the operation until the beginning of loading of th

Early intramedullary nailing of lower extremity fracture and respiratory function in polytraumatized patients with a chest injury: a retrospective study of 61 patients.

BACKGROUND: The optimal treatment of diaphyseal fractures of the lower extremities in patients who also have serious chest injuries is not known. PATIENTS AND METHODS: We retrospectively evaluated the effect of an early intramedullary nailing (IMN) of femur or tibia fractures on respiratory function in 61 consecutive polytraumatized patients with unilateral or bilateral pulmonary contusion (thoracic AIS=3) admitted to our trauma intensive care unit between January 2000 and June 2001. 27 patients had a diaphyseal fracture of at least one long bone of the lower extremity, which was treated with

Open exchange locked nailing in humeral nonunions after intramedullary nailing.

Humeral nonunions after cannulated intramedullary nailing have been difficult to reconstruct. In the current study, 23 consecutive patients were treated by open exchange locked nailing with bone grafting. There were 16 men and seven women with a mean age of 46.2 years. The nonunions followed humeral locked nailing in eight patients, Seidel nailing in 13, and Küntscher nailing in two. The average nonunion time was 14.7 months. The nonunions, located at the proximal (1/3) in four humeri, middle (1/3) in 15, and distal (1/3) in four, were antegrade nailed in 10 and retrograde nailed in 13. Ninete

Long-term residual musculoskeletal deficits after femoral shaft fractures treated with intramedullary nailing.

BACKGROUND: Locked intramedullary (IM) nailing has been recognized as one of the most reliable methods for treatment of femoral shaft fractures. Although IM nails are routinely used in the treatment of these fractures, the long-term effects of retained IM nails are unknown. METHODS: Seventeen patients with radiographically documented healed fracture of femoral diaphysis after locked IM nailing technique were evaluated at a follow-up of at least 18 months postoperatively. All patients had returned to their preinjury level of function and activity before the study. The bone mineral densities (B

Femoral fracture at the proximal end of an intramedullary supracondylar nail: a case report.

Retrograde intramedullary supracondylar nails have been added to the orthopedic armamentarium for treatment of distal femoral fractures. Major complications of this new technique have not been noted. We report a case of a femoral fracture at the proximal end of a retrograde intramedullary supracondylar nail. We hypothesize that this complication may be due to the proximal end of the nail acting as a stress riser, with the cortical holes drilled for the interlocking screws compounding this effect.

Complications following the use of the Marchetti flexible intramedullary nail.

Twenty-seven patients, with eight femoral and 20 tibial fractures, were treated with the Marchetti/Vincenzi flexible intramedullary nail between March 1993 and May 1994 in a District General Hospital. Clinical and radiological reviews were performed on all patients except for one who died. There was a high complication rate in both groups (63.5 per cent femoral and 43 per cent tibial). Particularly common was coronal mal-alignment. Other complications included non-union, external rotation, and fracture shortening with proximal migration of the nail. These data suggest that flexible intramedull

Concomitant post-traumatic ocular and cerebral fat embolism syndrome and thrombotic pulmonary embolism: A case report.

RATIONALE: Fat embolism syndrome (FES) is composed of a triad of symptoms, including respiratory distress, neurologic deficit, and petechiae. Respiratory distress usually presents first before the other symptoms. Thrombotic pulmonary embolism (TPE) is a differential diagnosis of FES. Trauma is a risk factor for both diseases; however, co-occurrence is rare. PATIENT CONCERNS: A 35-year-old male patient presented with altered consciousness, focal neurologic deficit, and respiratory distress after a left femoral subtrochanteric fracture and subsequent open reduction and internal fixation with an

Sciatic nerve compression by preexisting heterotopic ossification during general anesthesia in the dorsal lithotomy position.

This report documents the 4th patient reported with sciatic nerve compression by heterotopic ossification and the 1st case occurring during general anesthesia. A 25-year-old woman developed right sciatic nerve dysfunction after a 30-minute dilatation and curettage in the dorsal lithotomy position. Postoperative x-rays revealed a large area of heterotopic ossification above the right greater trochanter. The patient had sustained a traumatic midshaft fracture of the right femur seven years previously, which required intramedullary nail fixation. It is postulated that during the D and C in the do

Augmented reality enhanced physical simulation for femoral nailing training.

PURPOSE: Current femoral nailing simulators face a trade-off between delivering procedural guidance and realistic haptic feedback. This study aimed to address this limitation by developing and evaluating an augmented reality (AR)-enhanced physical simulator that combines real surgical instruments with AR-based visualization. The system provides spatial feedback on fluoroscopic imaging orientation and implant placement within hidden anatomical structures, supporting self-directed learning. METHODS: A hybrid simulator integrating a synthetic femur, real surgical instruments, and an AR interface

The Effect of Transphyseal Rigid Tibial Nailing on Proximal Tibial Morphology in Pediatric Patients With at Least 1 Year of Growth Remaining.

INTRODUCTION: Tibial shaft fractures are common in the pediatric population. As patients approach skeletal maturity, rigid intramedullary nailing of these fractures is a potentially advantageous option, allowing rigid fixation and earlier mobilization and weight-bearing. However, potential injury to the open proximal tibial physis in patients with remaining growth remains a concern. This study sought to determine the rate of growth disturbance following rigid, transphyseal tibial nailing of tibial shaft fractures in patients with open physes. METHODS: This was a retrospective study of skeleta

Helical Blade Versus Lag Screw Fixation in the Cephalomedullary Nailing of Geriatric Hip Fractures.

BACKGROUND: While some prior research has shown helical blades to have higher risks of fixation failure and cut-out than lag screws in the cephalomedullary nailing of intertrochanteric femoral fractures, other studies have not demonstrated any such differences. The purpose of this study was to compare the performance of helical blade and lag screw fixation among older patients with a hip fracture treated with cephalomedullary nailing and to determine whether the relative performance of these 2 fixation methods varies on the basis of patient characteristics. METHODS: This retrospective cohort

Management of Transverse Fracture of Shaft of Femur by Intramedullary Interlocking Nail with Dynamic Locking.

Fractures of the shaft of the femur are among the most common fractures encountered in orthopaedic practice. The goal in treating a femoral shaft fracture is to promote union, mobilize the patient and restore the function of the limb. The aim of the study was to evaluate the effectiveness of primary dynamization with intramedullary interlocking nail for the treatment of femoral shaft fracture (AO type 32-A3). This prospective study was conducted in National Institute of Traumatology and Orthopaedic Rehabilitation (NITOR), Dhaka, Bangladesh from July 2017 to June 2019. Fifty (50) patients with
