Hearing in nonprofessional pop/rock musicians.

OBJECTIVE: The purpose of this study was to evaluate the hearing and subjective auditory symptoms in a group of nonprofessional pop/rock musicians who had experienced repeated exposures to intense sound levels during at least 5 yr of musical activity. DESIGN: An evaluation of both ears in 42 nonprofessional pop/rock musicians included pure-tone audiometry in the conventional and extended high-frequency range, the measurement of uncomfortable loudness levels, and an assessment of tinnitus and hypersensitivity to sound. Exclusion criteria were (a) the occurrence of acoustic trauma, (b) excessiv…

Prevalence of hearing disorders in Singapore military conscripts: a role for routine audiometry screening?

AIM: To determine the prevalence of hearing disorders in Singapore military conscripts and describe the profile of hearing loss and associated risk factors. METHODS: Population-based descriptive analysis of randomised cohort of 818 Singapore military conscripts presenting for medical examination, using self-administered questionnaire and pure tone audiometry performed for both ears. Main outcome measures are prevalence rate of hearing loss and relative risk of hearing loss by demographic factors and surrogates for noise exposure. RESULTS: Prevalence of hearing loss was 36.7 per 1,000 (95% CI

The natural history of embouchure dystonia.

Focal task-specific dystonias are unusual disorders of motor control, often affecting individuals who perform complex repetitive movements. Musicians are especially prone to develop these disorders because of their training regimens and intense practice schedules. Task-specific dystonia occurring in keyboard or string instrumentalists usually affects the hand. In contrast, there have been few descriptions of musicians with task-specific dystonia affecting the muscles of the face and jaw. We report detailed clinical observations of 26 professional brass and woodwind players afflicted with focal…

Quantitative, clinically relevant acoustic measurements of focal embouchure dystonia.

BACKGROUND: Focal embouchure dystonia impairs orofacial motor control in wind musicians and causes professional disability. A paucity of quantitative measures or rating scales impedes the objective assessment of treatment efficacy. OBJECTIVES: We quantified specific features of focal embouchure dystonia using acoustic measures and developed a metric to assess severity across multiple domains of symptomatic impairment. METHODS: We recruited 9 brass musicians with and 6 without embouchure dystonia. The following 4 domains of symptomatic dysfunction in focal embouchure dystonia were identified:…

Drumming-associated anthrax incidents: exposures to low levels of indoor environmental contamination.

Two fatal drumming-related inhalational anthrax incidents occurred in 2006 and 2008 in the UK. One individual was a drum maker and drummer from the Scottish Borders, most likely infected whilst playing a goat-skin drum contaminated with Bacillus anthracis spores; the second, a drummer and drum maker from East London, likely became infected whilst working with contaminated animal hides. We have collated epidemiological and environmental data from these incidents and reviewed them alongside three similar contemporaneous incidents in the USA. Sampling operations recovered the causative agent from

A combination of constraint-induced therapy and motor control retraining in the treatment of focal hand dystonia in musicians: a long-term follow-up study.

UNLABELLED: Focal hand dystonia (FHD) in musicians is a painless, task-specific motor disorder characterized by involuntary loss of control of individual finger movements. The aim of this study was to investigate the long-term effects of a combined behavioral therapy intervention aimed at normalizing finger movement patterns. METHODS: Eight musicians with FHD had taken part in the 1-year study involving intensive constraint-induced therapy and motor control retraining at slow speed as the interventions. Four of these subjects volunteered to take part in this 4-year follow-up. A quasi-experime…

Exercise DVD effect on musculoskeletal disorders in professional orchestral musicians.

BACKGROUND: Professional musicians report a high prevalence of performance-related musculoskeletal disorders (PRMDs). Excessive muscle tension and fatigue have been reported as important factors contributing to PRMDs. AIMS: To evaluate feasibility and effectiveness of a specific exercise programme delivered via a digital video disc (DVD) targeting PRMDs and associated risk factors. METHODS: Volunteers from eight Australian symphony orchestras undertook two or more sessions per week over 12 weeks. Questionnaires were administered pre- and post-intervention with items including the frequency a…

Wolfgang Amadeus Mozart--controversies regarding his illnesses and death: a bibliographic review.

More has been written about Mozart's illnesses and death than for any other composer. An exploration of PAMA's Bibliography of Performing Arts Medicine provides the data for this review. The bibliography contained 136 entries that pertained to Wolfgang Amadeus Mozart. Of these, 81 were available to the author, either in printed or electronic copy. In order to provide a clearer historical perspective on this topic, this review assembles information pertaining to illnesses and other medical problems that occurred during Mozart's life as well as those purportedly contributing to his death.

[Voice diagnostics in professional sopranos: application of the protocol of the European Laryngological Society (ELS)].

The European Laryngological Society (ELS) has published a protocol for the evaluation of vocal function. Using this protocol, we evaluated 17 female sopranos, of whom 11 had laryngostroboscopic findings of organic alterations and six did not. Singers with increased subjective impressions of hoarseness, or of whom the examiner had increased perceptions of hoarseness, showed significant differences regarding impairment of the maximum phonation time, the frequency range, and the dysphonia severity index. No such differences were found regarding the presence of alterations of the vocal folds. Thes

Articulatory configuration and pitch in a classically trained soprano singer.

Previous studies suggest that singers modify articulation to avoid that the pitch frequency F0 exceeds the normal value of the first formant F1(Normal). Using magnetic resonance imaging at a rate of 5 frames/s, articulation was analyzed in a professional soprano singing an ascending triad pattern from C4 to G5 (262-784Hz) on the vowels /i, e, u, o, a/. Lip and jaw opening and tongue dorsum height were measured and analyzed as function of pitch. Four or five semitones below the pitch where F0=F1(Normal) the tongue dorsum height was reduced in /i, e, u, a/, whereas in /o/ the lip opening was wid

Treatment effectiveness for patients with a history of repetitive hand use and focal hand dystonia: a planned, prospective follow-up study.

Recent studies show that rapid, nearly simultaneous, stereotypical repetitive fine motor movements can degrade the sensory representation of the hand and lead to a loss of normal motor control with a target task, referred to as occupational hand cramps or focal hand dystonia. The purpose of this prospective follow-up study was to determine whether symptomatic patients in jobs demanding high levels of repetition could be relieved of awkward, involuntary hand movements following sensory discriminative retraining complemented by a home program of sensory exercises, plus traditional posture, relax

Paving the way for speech: voice-training-induced plasticity in chronic aphasia and apraxia of speech--three single cases.

Difficulties with temporal coordination or sequencing of speech movements are frequently reported in aphasia patients with concomitant apraxia of speech (AOS). Our major objective was to investigate the effects of specific rhythmic-melodic voice training on brain activation of those patients. Three patients with severe chronic nonfluent aphasia and AOS were included in this study. Before and after therapy, patients underwent the same fMRI procedure as 30 healthy control subjects in our prestudy, which investigated the neural substrates of sung vowel changes in untrained rhythm sequences.
