A Mucorales infection of an external iliac artery stent in a renal transplant patient is described. Following a kidney transplantation operation and an acute rejection of the transplant, an aneurysm developed at the site of the arterial ligation and three stents were inserted. Two months following insertion of the stents, recurrent episodes of peripheral thromboembolism occurred. Histological examination of the thrombus revealed hyphae characteristic of mucormycosis. A combination of surgical and medical treatment with intravenous liposomal amphotericin B was applied, which led to complete cur

The presence of a mass in a jejunal free flap that causes dysphagia less than two years after a pharyngolaryngectomy for carcinoma usually indicates tumour recurrence. We present a case of invasive candidiasis of a jejunal free flap presenting with dysphagia and a mass. To our knowledge this is previously unreported. Such a cause should always be considered in the differential diagnosis, as early recognition and treatment are likely to result in a favourable outcome.

BACKGROUND: Hemipelvic resections for primary malignant bone tumor require reconstruction to restore weight bearing along anatomic axes. However, reconstruction of the pelvic arch remains a major surgical challenge because of the high rate of associated complications. We designed a combined hemipelvic prosthetic system to reconstruct the pelvis and purpose of this investigation was to assess the oncology and functional outcome and complication rate following this procedures.
METHODS: we retrospectively reviewed 18 patients who had primary malignant pelvic tumor resections and reconstructions

BACKGROUND: There is limited data on perioperative management of patients with prior stroke undergoing total hip arthroplasty (THA). This study evaluates timing for THA following stroke. We assessed (1) complications at 90 days, 1 year, and 2 years; (2) timing of stroke and THA; and (3) risk factors for periprosthetic joint infection (PJI).
MATERIALS AND METHODS: We retrospectively analyzed a national database to identify 35,496 THA patients. Cohorts were stratified by time from stroke to surgery: no stroke (n = 20,000), stroke within 6 months (n = 5,535), 12 months (n = 3,165), 18 months (n

BACKGROUND: Albumin is a preoperative risk stratification tool for patients undergoing primary total knee arthroplasty (TKA) and total hip arthroplasty (THA). However, existing literature predominantly evaluates serum albumin levels as a categorical variable using predetermined cut-off values, limiting our ability to identify a data-driven risk threshold. Our study addresses this gap by analyzing albumin as a continuous variable, aiming to establish a more clinically relevant preoperative threshold for identifying patients at increased risk of postoperative complications.
METHODS: A healthcar

We report the diagnosis and clinical course of a 62-year-old patient with non-specific symptoms, such as dry cough, weight loss, increased fatigue, and a long-standing previously operated aortic graft prosthesis. Extensive investigations led to the diagnosis of a chronic infection of the aortic graft caused by Coxiella burnetii (Q-fever), following which an antibiotic therapy with Doxycycline and Hydroxychloroquine was established.

Background: Preventing the early shunt infection is critical for the success of ventriculoperitoneal shunt (VPS) operation. Our goal was to establish a standardized protocol to prevent early shunt infection. Patients and Methods: This was a single-center retrospective study. Patients who received the VPS in Peking Union Medical College Hospital (PUMCH) between August 2012 and June 2022 were enrolled. Data of patients were extracted from the PUMCH hydrocephalus database. An evidence-based protocol to prevent early shunt infection was established, implemented, and supervised strictly throughout

BACKGROUND: Vancomycin and tobramycin have traditionally been used in antibiotic spacers. In 2020, our institution replaced tobramycin with ceftazidime. We hypothesized that the use of ceftazidime/vancomycin (CV) in antibiotic spacers would not lead to an increase in treatment failure compared to tobramycin/vancomycin (TV).
METHODS: From 2014 to 2022, we identified 243 patients who underwent a stage I revision for periprosthetic joint infection. The primary outcome was a recurrent infection requiring antibiotic spacer exchange. We were adequately powered to detect a 10% difference in recurren

INTRODUCTION: Management of bone and joint infections (BJI) requires prolonged and high-dose antibiotic therapy to achieve target concentrations in bone tissue. However, these therapies often lead to adverse effects in patients who are frequently fragile, with multiple comorbidities and associated medications. The decision to treat these complex cases is made during a multidisciplinary team meeting at the reference centre for complex osteoarticular infections (CRIOAC).
MATERIAL AND METHODS: Elaborated by a pharmacist during CRIOAC meetings, a single-centre before-and-after comparative study o

There is concern for increased risk of adverse events, particularly periprosthetic joint infection (PJI), following total joint replacement (TJR) in patients with hidradenitis suppurativa (HS) because of a compromised skin barrier and bacterial colonization of lesions. We used the TriNetX health research database to identify patients who had undergone TJR with (n = 1760) and without (n = 1760) HS matched by age, sex, ethnicity, race and risk factors for PJI. Multivariate analysis was performed and revealed that 90-day risk of PJI, reoperation, wound dehiscence, delayed wound healing, emergency

BACKGROUND: We explore the incidence of periprosthetic infections post-total knee arthroplasty (TKA) in morbidly obese patients who achieved weight loss. Current American Academy of Orthopaedic Surgeons guidelines suggest a preoperative body mass index (BMI) below 40 for TKA. This study assesses infection risks in patients initially who had a BMI of 40-50 who reduced their BMI to under 35 at varying intervals prior to surgery.
METHODS: We reviewed a national, all-payer database, PearlDiver, for patients undergoing primary TKA. Patients were stratified based on initial BMI of 40 to 50 and redu

BACKGROUND: Synovial leukocyte count analysis is an important tool in the diagnosis of PJI. However, results can be poor if metallosis is present. The issue of metallosis was established for some rotating hinge knee arthroplasty designs.
METHODS: This was a retrospective single-center analysis that included 108 patients who had a single-design metal-on-metal rotating hinge knee arthroplasty who underwent revision surgery and had prior synovial fluid analysis performed. The diagnostic accuracy of leukocyte count and the percentage of polymorphonuclear neutrophiles (% PMN) were investigated.

OBJECTIVE: The ability of D-dimer to diagnose periprosthetic joint infection (PJI) before revision hip or knee arthroplasty is still controversial, and the differences in diagnostic ability between serum- or plasma-based assays of D-dimer and fibrin (fibrinogen) degradation product (FDP) are uncertain. The prospective parallel study was performed to determine the ability of D-dimer to diagnose PJI before revision hip or knee arthroplasty, and the differences in diagnostic ability between serum- or plasma-based assays of D-dimer and FDP.

A man in his 60s, with a medical history of gout, underwent total knee arthroplasty of his right knee followed by expeditious rehabilitation. Seven months after surgery, he was referred to the emergency ward with sudden onset of pain and swelling of his right knee accompanied with fever. Further inquiry revealed no trauma, infection or skin lesions besides a tongue bite several weeks earlier. An impaired range motion of the knee was seen on physical examination along with a tachycardia. Laboratory studies showed a C reactive protein of 345 mg/L, after which a debridement, antibiotics and impla

BACKGROUND: The aim of the study is to assess the diagnostic value of preoperative conventional radiographs for diagnosing infection associated with internal fixation devices.
METHODS: We prospectively collected data of patients undergoing removal of internal fixation devices for any reason. Infection was diagnosed in case of purulence, sinus tract, positive histopathology and/or positive peri-implant tissue or sonication fluid culture. In radiographs radiolucent lines, implant breakage or displacement, or periosteal reaction were assessed. White blood cell count (WBC) and serum C-reactive p

AIMS: Metagenomic next-generation sequencing (mNGS) is useful in the diagnosis of infectious disease. However, while it is highly sensitive at identifying bacteria, it does not provide information on the sensitivity of the organisms to antibiotics. The purpose of this study was to determine whether the results of mNGS can be used to guide optimization of culture methods to improve the sensitivity of culture from intraoperative samples.
METHODS: Between July 2014 and October 2019, patients with suspected joint infection (JI) from whom synovial fluid (SF) was obtained preoperatively were enroll

OBJECTIVE: In spite of the progress in antimicrobial and surgical therapy, infective endocarditis (IE) is still associated with a high morbidity and mortality. IE is characterized by bacterial biofilms of the endocardium, especially of the aortic and mitral valve leading to their destruction. About one quarter of patients with formal surgery indication cannot undergo surgery. This group of patients needs further options of therapy, but due to a lack of models for IE prospects of research are low. Therefore, the purpose of this project was to establish an in vitro model of infective endocarditi
