Femoral shaft fractures in eldery patients - An epidemiological risk analysis of incidence, mortality and complications.

OBJECTIVES: This work aimed at answering the following research questions: (1) What is the incidence of femoral shaft fractures in the geriatric population in the U.S.? (2) What is the rate of mortality, mechanical complications, nonunion and infection, and what are the associated risk factors? STUDY DESIGN AND SETTING: In this cross-sectional study, femoral shaft fractures occurred between January 1, 2009, and December 31, 2019, were identified from Medicare records. Rates of mortality, nonunion, infection, and mechanical complications were calculated with the Kaplan-Meier method with Fine a

Infrapatellar versus suprapatellar approach for intramedullary nailing of the tibia: a systematic review and meta-analysis.

BACKGROUND: Intramedullary nailing (IMN) is a conventional technique for the treatment of tibial shaft fractures. It has been suggested that the suprapatellar (SP) approach holds advantages over the traditional infrapatellar (IP) approach. Current literature lacks adequate data to provide robust clinical recommendations. This meta-analysis aims to determine the efficacy of infrapatellar versus suprapatellar techniques for IMN. METHODS: An up-to-date literature search of the Embase, Medline, and registry platform databases was performed. The search was conducted using a predesigned search stra

Developmental Patterns of the Greater Trochanter in Skeletally Immature Patients.

BACKGROUND: Trochanteric entry femoral nailing in immature patients is controversial. Although it has become increasingly accepted in pediatric patients aged 8 years and older, complications related to physeal violation have been reported. To date, the appropriate minimum age for safe antegrade nailing in skeletally immature patients remains to be unknown. The purpose of this investigation was to determine the degree of trochanteric bony and cartilage growth with increasing age to provide normative growth values. METHODS: A total of 408 serial digitized radiographs of the left hip in 45 healt

Suprapatellar versus infrapatellar approaches in the treatment of tibia intramedullary nailing: a retrospective cohort study.

BACKGROUND: Tibial shaft fractures are routinely managed with intramedullary nailing (IMN). An increasingly accepted technique is the suprapatellar (SP) approach. The purpose of this study was to compare the clinical and functional outcomes of knee joint after tibia IMN through an suprapatellar (SP) or traditional infrapatellar (IP) approach. METHODS: Retrospective analysis was performed in patients with tibial shaft fractures that were treated with IMN through a SP or IP approach between 01/01/2014 and 31/12/2016. The clinical and functional outcomes of the knee were assessed with the Hospit

Advantages and disadvantages of the prone position in the surgical treatment of supracondylar humerus fractures in children. A literature review.

INTRODUCTION: Supracondylar humerus fractures are the most common elbow injuries in children. The widely adopted approach for Gartland III extension type consists of closed reduction and percutaneous pinning; the pin configuration can be lateral or crossed in relationship with the habit of the surgeons. Iatrogenic injury of the ulnar nerve is the most common risk during the insertion of the medial pin. The aim of this study was to analyze advantages and disadvantages of percutaneous pinning with the patient in prone position. MATERIALS AND METHODS: A literature review of the period 2005-2017

Provider factors associated with intramedullary nail use for intertrochanteric hip fractures.

BACKGROUND: Intramedullary nails provide no clear outcomes benefit in the majority of patients with intertrochanteric hip fracture, yet their use in the United States continues to increase. Non-patient factors that are associated with intramedullary nail use among Medicare patients have not been examined. The goal of this study was to identify the surgeon and hospital characteristics that were associated with the use of intramedullary nails compared with plate-and-screw devices among elderly Medicare patients with intertrochanteric hip fractures. METHODS: Medicare beneficiaries who were sixty

Treating segmental bone defects: a new technique.

Repair of segmental bone defects is an enormous challenge for foot and ankle surgeons. This report describes the case of an unusual technique for managing segmental bone voids about the ankle, namely the use of a cylindrical titanium cage packed with bone graft, and an intramedullary nail. The technique was successful at bridging a large segmental defect, and biopsy specimens procured from the length of the healed graft confirmed osseous consolidation.

Irreducible fracture-dislocation of the ankle caused by an entrapped medial malleolus at the syndesmosis.

A 55-year-old fisherman sustained an unusual and irreducible fracture-dislocation of the ankle joint. An entrapped medial malleolar fragment was discovered by computed tomography between the distal tibia and lateral malleolus. The fragment was removed during an emergent open reduction, during which deltoid ligament repair and internal fixation of the bimalleolar fracture were undertaken. At the 18-month follow-up visit, the patient was working normally and was asymptomatic. At previous visits, he demonstrated a full range of ankle motion and had normal radiograph.

Distal locking of tibial nails : a new device to reduce radiation exposure.

UNLABELLED: The indications for intramedullary nailing have expanded to include most tibial shaft fractures. Nail design has improved since their first introduction, but distal locking remains a difficult part of the procedure, resulting in radiation exposure to the patient and the surgeon and increased operation time. To address these issues, we describe an alternative surgical technique using a newly designed distal targeting device that consists of a proximally mounted aiming arm, and we report the preliminary data from its use in all tibial shaft fractures amenable to surgery for a 2-year

Is there a gluteus medius tendon injury during reaming through a modified medial trochanteric portal? A cadaver study.

OBJECTIVES: The purpose of this cadaveric study was to evaluate whether there is any damage to the gluteus medius tendon when reaming through a modified medial trochanteric portal for antegrade intramedullary femoral nailing. METHODS: Ten cadaver hips were used in this study. A guidewire was placed in the modified medial trochanteric portal using the assistance of C-arm fluoroscopy and a 14-mm reamer was advanced over the wire. After the reaming was complete, each hip was dissected and the gluteus medius muscle and tendon were inspected to evaluate the amount of intrasubstance and medial tend

Surgical therapy of bone metastases.

The treatment of patients with cancer epitomizes the importance of using a collaborative team approach to optimize patient care. Physician team members most commonly are radiation oncologists, general surgeons, surgical oncologists, thoracic surgeons, neurosurgeons, and orthopedic surgeons. When patients are receiving chemotherapy, their medical oncologist frequently takes responsibility for coordinating care among the various team members and initiating consultations with necessary providers. When patients develop bone metastases or chemotherapy-induced bone loss (CTIBL), the orthopedic surge

Pulmonary dysfunction in patients with femoral shaft fracture treated with intramedullary nailing.

BACKGROUND: This study was undertaken to determine whether alveolar dead space increases during intramedullary nailing of femoral shaft fractures and whether alveolar dead space predicts postoperative pulmonary dysfunction in patients undergoing intramedullary nailing of a femoral shaft fracture. METHODS: All patients with a femoral shaft fracture were prospectively enrolled in the study unless there was evidence of acute myocardial infarction, shock, or heart failure. Arterial blood gases were measured at three consecutive time-periods after induction of general anesthesia: before intramedul

Fracture of the femoral shaft in children: advantages of the descending medullary nailing.

In a 7-year period, 70 children aged 5-15 years (mean 9.2 years) were treated for a fracture of the femoral shaft (73 fractures: 71 closed and 2 open) by flexible medullary nailing, either ascending (12 cases) or descending (61 cases). Although the classic treatment is use of the ascending route described by the Nancy team, the subtrochanteric descending route was preferred for its simplicity. Noninvolvement of the knee by the insertion of the nail allows earlier autonomy in children. Results were satisfactory and no complications related to the method were observed. Biomechanical principles a

Reduction and osteosynthesis of subcapital fractures of the femoral neck: possible repercussions on post-fracture hemarthrosis of the hip.

Intracapsular hip pressure was measured in undisplace or less displaced subcapital fractures of the femoral neck before and after open reduction and internal fixation with three-flanged nails. After reduction and osteosynthesis, intracapsular pressure increased in three out of five cases. The result is discussed in relation to the possible role of intracapsular pressure sustained by hemarthrosis in the pathogenesis of post-traumatic vascularity of the femoral head.
