Global Disability Trajectories Over the First Decade Following Combat Concussion.

OBJECTIVE: To examine global disability trajectories in US military with and without traumatic brain injury (TBI) over the first decade following deployment to identify risk profiles for better intervention stratification, hopefully reducing long-term cost. SETTING: Patients and participants were enrolled in combat or directly following medical evacuation at the time of injury and followed up every 6 months for 10 years. PARTICIPANTS: There are 4 main groups (n = 475), 2 primary and 2 exploratory: (1) combat-deployed controls without a history of blast exposure "non-blast- control" (n = 143)

Trajectories of physical activity, from young adulthood to older adulthood, and pancreatic cancer risk; a population-based case-control study in Ontario, Canada.

BACKGROUND: There is inconsistent evidence on the association between physical activity and pancreatic cancer risk and few studies have inve … od (1 year prior to questionnaire completion). Physical activity trajectories were identified using latent class analysis. Odds ratios (OR) and 95% confidence intervals (CI) were estimated from multivariable logistic regression adjusted for covariates: age, sex, race, alcohol, smoking, vegetable, fruit and meat consumption, and family history of pancreatic cancer. Classification of gastrointestinal symptom patterns in young adults.

BACKGROUND: The purpose of this study was to identify common gastrointestinal (GI) symptom groups using the Patient-Reported Outcomes Measur … al health of students at a major southeastern university. GI symptom groups were determined using a latent class analysis (LCA) approach. These GI symptom groups were then compared on key psychosocial factors including self-reported mood, anxiety, and health related quality of life (HRQoL) using MANOVA. RESULTS: Three groups were identified based on GI symptom elevati …

Predicting Trajectories of Functional Decline in 60- to 70-Year-Old People.

BACKGROUND: Early identification of people at risk of functional decline is essential for delivering targeted preventive interventions. OBJECTIVE: The aim of this study is to identify and predict trajectories of functional decline over 9 years in males and females aged 60-70 years. METHODS: We included 403 community-dwelling participants from the InCHIANTI study and 395 from the LASA study aged 60-70 years at baseline, of whom the majority reported no functional decline at baseline (median 0, interquartile range 0-1). Participants were included if they reported data on ≥2 measurements of fun

HIV integrase inhibitors block replication of alpha-, beta-, and gammaherpesviruses.

The catalytic site of the HIV integrase is contained within an RNase H-like fold, and numerous drugs have been developed that bind to this site and inhibit its activity. Herpes simplex virus (HSV) encodes two proteins with potential RNase H-like folds, the infected cell protein 8 (ICP8) DNA-binding protein, which is necessary for viral DNA replication and exhibits recombinase activity in vitro, and the viral terminase, which is essential for viral DNA cleavage and packaging. Therefore, we hypothesized that HIV integrase inhibitors might also inhibit HSV replication by targeting ICP8 and/or the

Multiple B vitamins dynamics during pregnancy and the risk of immediate postpartum anemia: a longitudinal birth cohort study.

BACKGROUND: Postpartum anemia is a global health issue linked to maternal mortality and sepsis. While iron deficiency plays a role, its contribution is limited, highlighting the need for better biomarkers. Although some studies suggest potential links between certain B vitamins and anemia, their relationship with immediate postpartum anemia (IPPA) remains unclear. We aimed to explore the longitudinal trajectory patterns of B vitamins (B1, B2, B3, B5, B6, and B9) during pregnancy and their potential associations with IPPA, and to develop a preliminary predictive model based on identified factor

Prenatal and child malaria and child growth: evidence from a Ghanaian pregnancy cohort.

INTRODUCTION: Additional investigation is needed to better understand the associations between prenatal and early child malaria and early childhood growth outcomes. METHODS: We leveraged the Ghana Randomized Air Pollution and Health Study which enrolled 1414 pregnant women from Kintampo, Ghana, and followed the mother-child dyads through the child's first year of life. We defined prenatal malaria exposure by histopathological examination of placenta tissue. We defined early child malaria exposure by active health surveillance over the child's first year of life and malaria rapid diagnostic te

Trajectories of health conditions predict cardiovascular disease risk among middle-aged and older adults: a national cohort study.

BACKGROUND: Most previous studies have focused on the association between health conditions measured at a single time point and the risk of cardiovascular disease (CVD), while evidence regarding the impact of long-term trajectories of health conditions is limited. This study aimed to construct models of health condition trajectories and to evaluate their association with CVD risk and predictive value. METHODS: This study included 2,512 participants aged 45 years and older from the China Health and Retirement Longitudinal Study (CHARLS), who were followed from 2011 to 2018. Trajectories of mul

Trajectories of alcohol screening and brief intervention (ASBI) performance and their associations with long-term performance and alcohol use outcomes: an observational study in a large US integrated healthcare delivery system.

BACKGROUND: Unhealthy alcohol use is a public health problem with significant health, social and economic impacts. Alcohol screening and brief intervention (ASBI) in adult primary care is an evidence-based approach enabling early identification and intervention of unhealthy alcohol use. However, large-scale implementation and sustainment of ASBI in routine clinical practice remains a challenge, and little is known about its population-level impact. Using electronic health record (EHR) data in a large integrated healthcare system in Northern California that implemented systematic ASBI in adult

Association between trajectories of fasting plasma glucose and risk of osteoporosis in non-diabetic and diabetic populations.

INTRODUCTION: Previous studies based on a single measure of fasting plasma glucose (FPG) showed an inconsistent conclusion about the association between FPG and osteoporosis risk. Not accounting for time-varying and cumulative average of FPG over time could bias the true relation between FPG and osteoporosis. Our study aims to investigate the association between the trajectories of FPG and osteoporosis risk for non-diabetic and diabetic populations. METHODS: A total of 18,313 participants who attended physical examinations during 2008-2018 were included. They were free of osteoporosis at thei

Prediction of 2-year clinical outcome trajectories in patients undergoing anterior cervical discectomy and fusion for spondylotic radiculopathy.

OBJECTIVE: Anterior cervical discectomy and fusion (ACDF) is often described as the gold standard surgical technique for cervical spondylotic radiculopathy. Although outcomes are considered favorable, there is little prognostic evidence to guide patient selection for ACDF. This study aimed to 1) describe the 24-month postoperative trajectories of arm pain, neck pain, and pain-related disability; and 2) identify perioperative prognostic factors that predict trajectories representing poor clinical outcomes. METHODS: In this retrospective cohort study, patients with cervical spondylotic radiculo
