Prevalence and Mechanisms of Carbapenem Resistance Among Acinetobacter baumannii Clinical Isolates in Egypt.

The increasing number of carbapenem-resistant Acinetobacter baumannii clinical isolates is a major concern, which restricts therapeutic options for treatment of serious infections caused by this emerging pathogen. The aim of this work is to assess the antimicrobial resistance profile and identify the molecular mechanisms involved in carbapenem resistance in A. baumannii isolated from different clinical sources in Mansoura University Hospitals, Egypt. Antimicrobial susceptibility testing has shown that resistance to carbapenem has dramatically increased (98%) with concomitant elevated levels of

Multiple roles for a novel RND-type efflux system in Acinetobacter baumannii AB5075.

Colony opacity phase variation in Acinetobacter baumannii strain AB5075 is regulated by a reversible high-frequency switch. Transposon mutagenesis was used to generate mutations that decreased the opaque to translucent switch and a gene encoding a predicted periplasmic membrane fusion component of a resistance-nodulation-cell division (RND)-type efflux system was isolated. This gene was designated arpA and immediately downstream was a gene designated arpB that encodes a predicted membrane transporter of RND-type systems. A nonpolar, in-frame deletion in arpA resulted in a 70-fold decrease in t

Sheltering effect and indirect pathogenesis of carbapenem-resistant Acinetobacter baumannii in polymicrobial infection.

The role of carbapenem-resistant Acinetobacter baumannii (CRAb) in polymicrobial infection remains elusive. Having observed the ability of CRAb to shelter other susceptible bacteria from carbapenem killing, we sought to determine the factors contributing to this sheltering effect by transforming different recombinant plasmids into recipient A. baumannii cells. The sheltering effects of CRAb were reproduced in recipient A. baumannii cells that highly expressed carbapenem-hydrolyzing class D β-lactamases (CHDLs) through their associated strong promoter. With the use of Western blot analysis and

Sulbactam-based therapy for Acinetobacter baumannii infection: a systematic review and meta-analysis.

BACKGROUND: A number of studies have reported on the effectiveness of sulbactam-based therapies for Acinetobacter baumannii infection; however, there is little evidence that sulbactam-based therapies are more or less effective than alternative therapies. Unfortunately, there is a distinct lack of high quality data (i.e., from randomized controlled trials) available on this issue. Therefore, we conducted a systematic review and meta-analysis comparing the efficacy of sulbactam-based and non-sulbactam-based regimens in the treatment of A. baumannii infection. METHODS: We searched PubMed, MEDLIN

OXA-143, a novel carbapenem-hydrolyzing class D beta-lactamase in Acinetobacter baumannii.

A carbapenem-resistant Acinetobacter baumannii strain was isolated in Brazil in 2004 in which no known carbapenemase gene was detected by PCR. Cloning experiments, followed by expression in Escherichia coli, gave an E. coli recombinant strain expressing a novel carbapenem-hydrolyzing class D beta-lactamase (CHDL). OXA-143 showed 88% amino acid sequence identity with OXA-40, 63% identity with OXA-23, and 52% identity with OXA-58. It hydrolyzed penicillins, oxacillin, meropenem, and imipenem but not expanded-spectrum cephalosporins. The bla(OXA-143) gene was located on a ca. 30-kb plasmid. After

Colistin methanesulfonate against multidrug-resistant Acinetobacter baumannii in an in vitro pharmacodynamic model.

Using an in vitro pharmacodynamic model, a multidrug-resistant strain of Acinetobacter baumannii was exposed to colistin methanesulfonate alone and in combination with ceftazidime. Pre- and postexposure colistin sulfate MICs were determined. A single daily dose of colistin methanesulfonate combined with continuous-infusion ceftazidime prevented regrowth and postexposure MIC increases.

Catheter-Associated Vancomycin-Resistant Enterococcus faecium Ventriculitis and Multidrug-Resistant Acinetobacter baumannii Pneumonia With Subsequent Acinetobacter Ventriculitis: A Case Report.

Ventriculitis is associated with cerebrospinal fluid (CSF) shunts, and rare microorganisms associated with infection include vancomycin-resistant Enterococcus (VRE) faecium and Acinetobacter baumannii. Both organisms are known to cause nosocomial infections, and the emergence of multidrug-resistant (MDR) strains presents a treatment challenge. There is a lack of consensus on antimicrobial agent selection for ventriculitis involving VRE faecium or MDR A. baumannii, which are life-threatening conditions. We present a case of a 59-year-old male presenting with CSF catheter-associated VRE faecium

Multifaceted roles of carboxyl compounds in antibiofilm potency and resistance modulation in Acinetobacter baumannii.

The escalating threat of multidrug-resistant (MDR) Acinetobacter baumannii is primarily driven by its robust biofilm-forming capacity and complex antimicrobial resistance mechanisms. This review comprehensively explores the pivotal role of the carboxyl functional group in modulating biofilm-associated virulence, quorum sensing (QS), and resistance pathways in A. baumannii. We emphasize the critical regulatory functions of the BfmRS two-component system and the AbaI/R QS system in biofilm formation, antibiotic resistance, and immune evasion. A diverse range of carboxyl-functionalized compounds,

Characterizing the in vitro and in vivo effect of bicarbonate on azithromycin activity against Acinetobacter baumannii.

Carbapenem-resistant Acinetobacter baumannii (CRAB), a Gram-negative bacterial pathogen, has been identified by Centers for Disease Control (CDC) as the top priority pathogen for which new antibiotics are needed. We found that the addition of bicarbonate at physiologically normal levels found in the blood (23.8 mM) increased susceptibility of A. baumannii clinical isolates (n = 63) to azithromycin and resulted in MIC50 shift from 64 mg/L in CAMHB to 1-2 mg/L in CAMHB + bicarbonate (23.8 mM) or RPMI-1640 respectively. To characterize in vivo efficacy in murine blood and lung infection models, m

Genomic landscape of nosocomial Acinetobacter baumannii: A comprehensive analysis of the resistome, virulome, and mobilome.

Acinetobacter baumannii (A. baumannii) is a major multidrug-resistant pathogen, posing serious threats in the healthcare settings. This study provides a comprehensive genomic analysis of nosocomial A. baumannii whole-genome sequences retrieved from NCBI Genome database. Multilocus sequence typing and capsule typing were performed to investigate the clonal diversity. The genomes were characterized to identify antimicrobial resistance genes (ARGs), virulence factors, and mobile genetic elements. Further, pangenome analysis was conducted to examine the core and accessory genomes of A. baumannii.

A conserved hub protein required for peptidoglycan remodeling and cell division in Acinetobacter baumannii.

Gram-negative bacteria build a multilayered cell envelope in which the peptidoglycan layer is sandwiched between an inner membrane of glycerophospholipids and an asymmetric outer membrane composed of glycerophospholipids and lipopolysaccharide (LPS). Acinetobacter baumannii, however, synthesize lipooligosaccharide (LOS), an LPS variant lacking O-antigen. Although LPS/LOS is typically essential, A. baumannii can survive without LOS, offering the opportunity to examine how the Gram-negative envelope remains stable in the absence of this major glycolipid. We previously found that the peptidoglyca

Pharmacokinetics of sulbactam-durlobactam in patients with Acinetobacter baumannii ventriculitis: a report of two cases.

Two adult patients with ventriculitis due to Acinetobacter baumannii were treated with intravenous sulbactam-durlobactam as a part of a combination regimen. Penetration into the central nervous system was estimated (by cerebrospinal fluid [CSF]/serum area under the curve [AUC] ratios at steady state) for sulbactam (0.47-0.64) and durlobactam (0.27-0.3), respectively. In view of mostly constant CSF concentrations, single CSF levels at steady state may offer a more convenient and clinically applicable approach for monitoring therapy.

Impact of AbaI mutation on virulence, biofilm development, and antibiotic susceptibility in Acinetobacter baumannii.

The quorum sensing (QS) system mediated by the abaI gene in Acinetobacter baumannii is crucial for various physiological and pathogenic processes. In this study, we constructed a stable markerless abaI knockout mutant (ΔabaI) strain using a pEXKm5-based allele replacement method to investigate the impact of abaI on A. baumannii. Proteomic analysis revealed significant alterations in protein expression between the wild type (WT) and ΔabaI mutant strains, particularly in proteins associated with membrane structure, antibiotic resistance, and virulence. Notably, the downregulation of key outer me

Lipocalin-2 is an essential component of the innate immune response to Acinetobacter baumannii infection.

Acinetobacter baumannii is an opportunistic pathogen and an emerging global health threat. Within healthcare settings, major presentations of A. baumannii include bloodstream infections and ventilator-associated pneumonia. The increased prevalence of ventilated patients during the COVID-19 pandemic has led to a rise in secondary bacterial pneumonia caused by multidrug resistant (MDR) A. baumannii. Additionally, due to its MDR status and the lack of antimicrobial drugs in the development pipeline, the World Health Organization has designated carbapenem-resistant A. baumannii to be its priority
