[Magnetic resonance tomography in acoustic neurinomas].

The possibilities of MR and CT were compared in respect of neurinomas of the acoustic nerve (n. acusticus). The comparison showed that MR detection of tumours in the posterior of the cranial cavity is the more sensitive method, especially in respect of neurinomas of the acoustic nerve, although CT enables the use of the small-level technique with or without the addition of a contrast substance, as well as the application of pneumocisternography. The sensitivity of MR can be increased further by adding a contrast medium. Whereas in diagnosis of extrameatal neurinomas of the n. acusticus, CT and

The pathogenesis of stereocilia abnormalities in acoustic trauma.

Stereocilia abnormalities develop in anesthetized guinea pigs within two minutes of exposure to loud sound (3 kHz, 125 dB SPL). Displacement, fracture, fusion and membrane rupture were observed well before the development (after 10 or more minutes) of fine structural changes to other cytoplasmic organelles within the hair cells. Thus stereocilia abnormalities can arise very rapidly due to the direct effects of noise on their microfilaments, cross linkages and limiting membranes. Such abnormalities are thus not necessarily secondary to changes in the body of the affected hair cells.

The acoustic reflectometer as a screening device: a comparison.

Sixty children (120 ears) were evaluated using reflectometry, pure tones, immittance, otologic examination, and pure-tone air and bone conduction thresholds. Comparisons were made between the various screeners. Reflectometry was found to be the least sensitive and the least specific of the procedures. Results were then examined within each diagnostic category to determine test agreement with the otologists' judgments. The reflectometer categorization was found to be inconsistent with the otologic findings. Finally, the reflectometer was compared to each of the other screener results and pure-t

Management of acoustic neuromas, 1978-1983.

The surgical aspects of 162 consecutive procedures for removal of acoustic neuromas, performed from 1978 through 1983, are reviewed. Nearly all of the procedures were done through a retrosigmoid suboccipital craniectomy. Most used the combined skills of a neurosurgeon and an otological surgeon. Total tumor removal was accomplished in 98% of cases. There have been two recurrences and one postoperative death. The facial nerve was preserved in 81% of procedures. Facial function returned in nearly all of these patients, but the degree of return was variable. The cochlear nerve was preserved in 55

A review of acoustic tumors: 1979-1982.

This is a review of the English literature relative to acoustic tumors from 1979 through 1982. I have selected what I consider the most important articles. Many other significant papers are not discussed. The material is presented in ten categories: diagnosis, histopathology, surgical approaches, facial nerve preservation and repair, von Recklinghausen's disease and bilateral tumors, nonacoustic cerebellopontine angle tumors, instrumentation, basic research, complications of surgery, and miscellaneous topics. In each section the articles are briefly reviewed, and sometimes a personal point of

High-frequency sensitization of the acoustic reflex.

High-frequency sensitization of the acoustic reflex was studied in 10 normal-hearing subjects. The procedure involved pairing a subreflex 6000 Hz sensitizing tone with activating frequencies of 500, 1000, and 2000 Hz. Both threshold and growth of amplitude of the acoustic reflex were examined. Acoustic reflex threshold was reduced by an average of 10 to 12 dB in the presence of a 6000 Hz sensitizing tone, an amount significantly less than previously reported in similar investigations. Variations in the method used to adjust the presentation level of the high-frequency sensitizing tone probably

Acoustic neurinoma presenting as subarachnoid hemorrhage.

A 49-year-old man who had been suffering from disturbance of hearing for 5 years suddenly developed severe headache, nausea, and vomiting. Computed tomography (CT) scans both with and without contrast medium enhancement, performed on the day of admission, showed a nearly round high-density area in the left cerebellopontine angle. A left transaxillary vertebral angiogram showed no aneurysm or arteriovenous malformation. Hemorrhage from a tumor in the left cerebellopontine angle was suspected. CT scan without contrast medium enhancement, performed on the 17th day after onset, showed only a small

Clinical findings in patients with acoustic neurinoma.

Herein we review the symptoms, physical findings, and test results in 131 patients with acoustic neurinoma. The earliest symptoms are unilateral hearing loss, tinnitus, and dysequilibrium. As the disease progresses, facial numbness, facial weakness, and headaches become more prominent. Physical findings other than hearing loss are uncommon; the most frequently observed are a decreased corneal reflex, nystagmus, and facial hypoesthesia. Routine audiometry provides objective information about the hearing loss. Some degree of asymmetry is found in the pure-tone tests. In addition, the speech disc

Effect of the acoustic reflex on the spondee threshold.

To determine the effects of the intra-aural muscle reflex on the spondee threshold, 12 normal-hearing adults were tested monaurally with two contralaterally presented stimuli, a 2000 Hz pure tone and a broad-band noise, at two presentation levels: -10 dB and +10 dB re each subject's acoustic reflex threshold for each stimulus. The mean spondee threshold in the +10 dB condition was significantly higher than the mean spondee threshold in the -10 dB condition for both contralateral stimuli. Although some threshold shift in the presence of the +10 dB contralateral broad-band noise was no doubt due

[Acoustic reflexometry in presbyacusis (author's transl)].

The authors have studied the relations between crossed and uncrossed acoustic stapedius reflex thresholds in presbyacusic subjects in order to verify whether ageing produces brain stem auditory lesions. The results showed in 55% of the presbyacusics a brain stem disorder characterized by the absence of one, several or all the acoustic reflexes; very often there was only an abnormal difference between crossed and uncrossed acoustic reflex thresholds.

The biphasic acoustic reflex: a new perspective.

The anatomic, neurologic, and physiologic characteristics of the middle ear structures have created confusion concerning the nature of the intraaural muscle reflex. After reviewing the relevant literature, we correlated this information with our studies of human temporal bones and computer analyses of the acoustic reflex responses of individuals with normal hearing. Our hypothesis is that although the stapedius is the initiator of the reflex and the primary contributor to ossicular chain fixation, the tensor tympani is responsible for the major observed response. The negative deflection presen

A comparison of results--acoustic susceptance and otolaryngology.

A screening study of hearing and middle-ear function was conducted on 2125 preschool children. Results are reported here for 504 of the subjects with static acoustic susceptance. Of these children, 380 had otolaryngological examinations. Static acoustic susceptance reliably provides an estimate of the stiffness component of the middle ear, but it does not predict middle ear abnormalities consistently. Three-year-old female subjects seem to show low values of static acoustic susceptance associated more with developmental factors than with middle ear disease.

Acoustic-reflex dynamics for pulsed signals.

Contralateral acoustic-reflex measurements were taken for 10 normal-hearing subjects using a pulsed broadband noise as the reflex-activating signal. Acoustic impedance was measured at selected times during the on (response maximum) and off (response minimum) portions of the pulsed activator over a 2-min interval as a function of activator period and duty cycle. Major findings were that response maxima increased as a function of time for longer duty cycles and that response minima increased as a function of time for all duty cycles. It is hypothesized that these findings are attributable to the

Teflon laryngoplasty: an acoustical and perceptual study.

A 61-year-old male with a 24-year history of unilateral vocal fold paralysis was evaluated by a speech-language pathologist and an otolaryngologist for 52 weeks following Teflon injection. Tape recordings of the patient's voice were obtained one week prior to and one, eight, 20, and 52 weeks subsequent to Teflon injection. The recorded samples were presented in the backward-play mode to a panel of speech-language pathology graduate students to obtain ratings of the degree of hoarseness, roughness, and pleasantness. Results of the perceptual aspect of this study revealed a general reduction in

Effect of anesthesia on susceptibility to acoustic trauma.

In an effort to ascertain whether differences in susceptibility to noise depend on general condition, awake and anesthetized guinea pigs were given a 4 kHz pure tone overstimulation under identical conditions. Cochlear hair cells were histologically examined four weeks after the noise exposure. The damage was localized in the upper part of the first turn and the lower part of the second turn. One fourth as much damage occurred in the anesthetized group. The distribution of damage in the four rows of sensory cells was different in the two experimental groups. In both groups of animals the damag

The acoustic reflex at low sensation levels.

The acoustic stapedial reflex, which normally occurs at a hearing level of about 80 dB, can be detected at lower levels by means of preactivation. Two techniques are presented, which allow the detection of reflex activity at intensities from 30 to 50 dB HL. It was the authors' intention to find a way to determine the hearing threshold from the stapedius reflex threshold even in patients, in whom the hearing threshold was not horizontal.
