OBJECTIVE: To compare the effect of the medical and surgical treatment of pediatric chronic rhinosinusitis (CRS). METHOD: Seventy-two cases of pediatric CRS were randomly divided into medical group (35 cases) and surgical group (37 cases). The patients in medical group received a 12-week course of clarithromycin, alkaline nasal douche and intranasal mometasone furoate. The patients in surgical group underwent adenoidectomy or (and) tonsillectomy or ESS. All patients underwent pre- and post-treatment assessments of visual analogue score (VAS) and nasal endoscopy. The assessments of CT were arr…

Adenotonsillectomy in children: indications and contraindications.

Adenoidectomy and tonsillectomy are the most common major operations done on children. The indications for tonsillectomy in certain clinical situations are constantly being debated in the literature and among professionals. We studied the efficacy (or lack of it) of adenotonsillectomy for chronic tonsillitis (recurrent throat infections), oral nasal obstruction, peritonsillar abscess, elimination of a bacterial carrier state, biopsy, and prevention of tongue thrusting with resultant anterior open bite. Adenoidectomy has been advocated in the literature for the treatment of nasal obstruction, s…

Mandibular form and position related to changed mode of breathing--a five-year longitudinal study.

A five-year follow-up study was performed on 26 children treated for nasal obstruction by adenoidectomy, who exhibited a changed mode of breathing postoperatively. They were compared with a control group matched according to age and sex. Lateral skull radiographs were used to examine mandibular morphology. The mandibular outline was registered using 36 digitized points. This method of portraying growth changes provides a valuable complement to isolated measurements. The technique revealed a more anterior direction of symphyseal growth in the adenoidectomy group following surgery as well as som…

Submucous cleft palate and the general practitioner.

Submucous cleft palate refers to a situation where the soft palate is largely composed of mucosa with little or no muscle. The defect is often not obvious on inspection of the mouth and pharynx. There is considerable clinical variation, with speech ranging from normal or minimal nasality to severe nasality and defective articulation. Many patients who have latent submucous cleft palate have the condition unmasked by an adenoidectomy because the adenoid pad had served as a compensatory factor in effecting palatopharyngeal closure. All physicians who perform tonsillectomy and adenoidectomy shoul…

A solitary tonsil can cause severe obstructive sleep apnea.

Hypertrophy of the tonsils and adenoids is the most common cause of pediatric obstructive sleep apnea. Bilateral tonsillectomy, most commonly performed with adenoidectomy, is the accepted treatment for obstructive sleep apnea. We report the unusual case of a child who underwent unilateral tonsillectomy and adenoidectomy at another institution and subsequently presented to us with persistent severe obstructive sleep apnea and a diagnosis of attention deficit hyperactivity disorder. The adenoids were not obstructing the choanae. The remaining tonsil was removed and the patient's sleep apnea reso…

Incidence of second surgery following pediatric adenotonsillar surgery: a population-based cohort study.

The aim of the study is to evaluate the incidence and risk factors of a second surgery of the adenoids or tonsils for hypertrophy in children who have already undergone surgery for the same condition. This is a retrospective study (2004-2013) based on data from the National patient registry in Sweden. A total of 41,401 children underwent a first surgery of the adenoids or tonsils during the studied period. The most commonly performed (first) surgical procedure was adenoidectomy followed by adenotonsillotomy, adenotonsillectomy, tonsillotomy, and tonsillectomy. A total of 4459 patients underwen…

[Middle ear ventilation with the Otovent latex membrane system].

Epidemiological studies have shown that up to 80% of all pre-school children develop Eustachian tube dysfunction with middle ear effusion and corresponding conductive hearing loss. The therapy of choice for hyperplastic adenoids is adenoidectomy, often in combination with paracentesis. The objective of this study was to determine whether regular middle ear aeration (Otovent-system) could obviate the need for paracentesis. To this end treatment of 146 children from four to ten years old was conducted within the guidelines of a study protocol. In one group of children, Otovent-supported middle e…

Tonsillectomy in the allergic child.

Edema is the chief factor in enlargement of tonsils of children with allergic disease. In the absence of infection tonsillectomy and adenoidectomy are contraindicated in the allergic child except when obstruction is present. Infection produces one of two distinct patterns when it influences allergic states, the pattern depending upon the nature of the infectious disease. Chronic infection of the upper respiratory tract in the allergic child produces a pattern simulating that of acute respiratory infection. If, in the presence of infected tonsils and adenoids, management of the allergic state do…

Otitis media with effusion and tubal tonsil (video).

Carbon dioxide laser surgery was performed on children aged 5 to 11 years, on whom either adenoidectomy or tonsilloadenoidectomy had been performed to restore their hearing. We examined 177 ears 18 to 24 months after surgery. Thirty ears (17%) were diagnosed as having otitis media with effusion. Analysis of the endoscopic findings revealed 51% marked tubal tonsil hypertrophy and 33% adenoid tissue hypertrophy. Subjects were operated upon in the dorsal position for a duration of point one second at an amplitude of 15 watts. Six months after surgery, most patients' hearing had returned to almost…

Comparative study on the transport and deposition of dust mite fecal particles in the upper airway of Asian children with adenoid hypertrophy before and after surgery.

Adenoidectomy is a routine treatment for adenoid hypertrophy (AH), but its effect on allergen deposition in the upper airways remains unclear. In this study, anatomically realistic upper airway models of eight Asian children with AH and their post-operative counterparts were reconstructed. Based on experimentally validated models, the transport trajectories of dust mite fecal particles were simulated and tracked using a computational fluid dynamics-discrete phase model (CFD-DPM). The results demonstrated that as particle diameter increased, the nasal septum and vestibule gradually became major…

Atypical bacteria in adenoids and tonsils of children requiring adenotonsillectomy.

CONCLUSIONS: The results of this study suggest that atypical bacteria may be involved not only in acute upper airway diseases but also in recurrent infections requiring adenoidectomy and/or tonsillectomy. Therefore, their identification, followed by an appropriate treatment, should be considered. OBJECTIVE: Although viruses and group A beta-haemolytic streptococci (GABHS) represent the most frequent bacterial aetiological agents of paediatric upper respiratory tract infections (URTIs), chlamydia and Mycoplasma pneumoniae have also been found in acute tonsillopharyngitis. Nevertheless their re…

Lidocaine and risk of postoperative vomiting in children undergoing tonsillectomy: a randomised clinical trial.

The dose-response of intravenous lidocaine in preventing postoperative vomiting (POV) in children remains unclear. This study investigated whether intravenous lidocaine dose-dependently decreased POV risk within 24 h postoperatively in children undergoing tonsillectomy (with or without adenoidectomy) without severe complications. Patients aged 3-12 years (American Society of Anesthesiologists grade I-II) scheduled for elective tonsillectomy (with or without adenoidectomy) were enroled from December 2021 to March 2022. They were randomly grouped according to the lidocaine dose (A [0 mg kg-1], B…

Association of Long-Term Risk of Respiratory, Allergic, and Infectious Diseases With Removal of Adenoids and Tonsils in Childhood.

IMPORTANCE: Surgical removal of adenoids and tonsils to treat obstructed breathing or recurrent middle-ear infections remain common pediatric procedures; however, little is known about their long-term health consequences despite the fact that these lymphatic organs play important roles in the development and function of the immune system. OBJECTIVE: To estimate long-term disease risks associated with adenoidectomy, tonsillectomy, and adenotonsillectomy in childhood. DESIGN, SETTING, AND PARTICIPANTS: A population-based cohort study of up to 1 189 061 children born in Denmark between 1979 and…
