Radiographic evaluation of children with nasopharyngeal obstruction due to the adenoid.

The adenoidal-nasopharyngeal ratio (ANR) was proposed as a convenient and practical method to evaluate adenoidal enlargement. We analyzed cephalometric radiographs of 150 children divided into 6 subgroups according to clinical assessment (possible adenoidectomy candidates [PACs] and controls) and age. Our purpose was to investigate the predictive reliability of different ANRs calculated on the same reference line. Each of the ANRs described in this study presented statistically significant differences between PACs and control groups, and highly significant correlations with clinical symptoms o

[Post-obstructive pulmonary edema after adenoid-amygdalectomy].

A two-years-old boy with Down's Syndrome and a small interauricular communication was given priority scheduling for adenoidectomy and tonsillectomy to correct subacute obstruction of the upper airway. The only noteworthy event during surgery was an SpO2 of 92% during mechanical ventilation with FiO2 of 0.5 from no apparent cause. The patient suffered acute respiratory insufficiency due to the post-obstructive pulmonary edema in the hours immediately following surgery. The incidence of post-obstructive pulmonary edema is higher than might be expected, as symptoms can be interpreted as aspiratio

The microbial ecology and immunology of the adenoid: implications for otitis media.

The nasopharyngeal tonsil, or adenoid, is a major inductive site for the synthesis of J-chain-positive B cells that may migrate to other are … that there is a difference in the profiles of cytokine secretion by Th1 and Th2 lymphocytes in the adenoids in both otitis-prone, as well as nonotitis-prone children. We have suggested that if this specific immune system does not protect the host from invasion by potential pathogens, there are other modalities of therapy to protect the nasopharynx from co …

Expression of adhesion molecules on the microvasculature of the pharyngeal tonsil (adenoid).

We have examined the vasculature of 12 human adenoids and the expression and distribution of four endothelial adhesion molecules, ICAM-1, VCAM-1, P-selectin and E-selectin, in tissue sections using histology, immunocytochemistry and immunoelectron microscopy (IEM). The connective tissue septa and septula contained arterioles, veins and efferent lymphatics. Branches of arterioles supplied lymphoid follicles and divided into sub- and intraepithelial capillary plexuses which drained into interfollicular venules, mostly high endothelial venules (HEV), before joining larger veins. No afferent lymph…

[Very rare case of adenoid cystic carcinoma of the hypopharynx].

We experienced a case of adenoid cystic carcinoma (cribriform type) of the hypopharynx treated with radiation followed by total laryngectomy and bilateral radical neck dissection. There have been no reports of adenoid cystic carcinoma of the hypopharynx in the last 30 years. In this case, the primary tumor and lymph node metastases, of the neck responded well to radiation therapy. Residual disease of the primary tumor and lymph node metastases, after a dose delivery of 50 Gy, were removed by radical surgery.

Adenoid cystic carcinoma of the sublingual gland--an unusual presentation.

A case of adenoid cystic carcinoma of the sublingual salivary gland presenting as recurrent submandibular salivary gland swelling is described. The obstructive symptoms were due to involvement of the ipsilateral submandibular duct by the tumour. A review of the literature suggests that this is the first such case to be reported. The importance of intra-oral examination when investigating swellings of the neck is emphasised.

[Influence of adenoidal vegetation on rhinopharyngeal obstruction: cephalometric study].

In this paper, the Authors propose a new method of cephalometric visualization of adhaenoids, in order to verify the amount of them as cause of an obstructive pathology. In this evaluation, related to a group of 100 individuals, was found an inversely proportional relation between adhaenoid's volume and angolar value of FHBoPA angle.

The penetration of erythromycin into Waldeyer's ring--tonsil and adenoid tissue.

Erythromycin ethylsuccinate was given orally (30-60 mg/kg/day) to 72 otherwise healthy children for different intervals before adenoidectomy. The concentration of erythromycin was assessed in homogenized tissue, in whole blood and in plasma, making it possible to calculate blood-free tissue levels. The tissue levels (1.5-2.0 mg/l) were the same as the plasma levels. Six patients with chronic tonsillitis were given 1000 mg erythromycin ethylsuccinate via a gastric tube, and tonsillectomy à froid was performed three to five hours later. This unusual mode of administration was chosen to avoid ton

Intra-arterial cisplatin treatment of adenoid cystic carcinoma.

Adenoid cystic carcinoma (ACC) is unique among salivary gland tumors in both its natural history and in its response to nonsurgical treatment methods (ie, radiation and chemotherapy). The chemotherapeutic agent, cisplatin, seems to be unique in its ability to affect ACC. It might be that ACCs of the minor salivary glands are especially vulnerable to the intra-arterial method of administering this drug, because there are accessible feeding vessels to most locations in which these tumors occur. We have treated four patients with advanced ACC with intra-arterial cisplatin. The responses suggest t

Primary pulmonary adenoid cystic carcinoma presenting with ground-glass opacity nodules coexisting with male breast adenoid cystic carcinoma: A case report.

Adenoid cystic carcinoma (ACC) of the lung is a rare subtype of lung cancer characterized by tumor cells forming cystic or glandular structures. It predominantly arises in segmental or larger bronchi, typically presenting as solid nodules or masses on imaging. Previous studies describe it as a slow-growing, low-grade malignancy associated with favorable prognosis and low metastatic potential. A 29-year-old male underwent surgical resection for breast nodules, pathologically confirmed as ACC of the breast. During hospitalization, chest CT revealed multiple bilateral ground glass nodules (GGN),

Association of Long-Term Risk of Respiratory, Allergic, and Infectious Diseases With Removal of Adenoids and Tonsils in Childhood.

IMPORTANCE: Surgical removal of adenoids and tonsils to treat obstructed breathing or recurrent middle-ear infections remain common pediatric procedures; however, little is known about their long-term health consequences despite the fact that these lymphatic organs play important roles in the development and function of the immune system. OBJECTIVE: To estimate long-term disease risks associated with adenoidectomy, tonsillectomy, and adenotonsillectomy in childhood. DESIGN, SETTING, AND PARTICIPANTS: A population-based cohort study of up to 1 189 061 children born in Denmark between 1979 and…

Association of Pneumococcal Carriage and Expression of Foxp3+ Regulatory T Cells and Th17 Cells in the Adenoids of Children.

BACKGROUND: Pneumococcal carriage in the nasopharynx is a primary means of transmission and a necessary prerequisite for pneumococcal disease. OBJECTIVES: We analyzed the relationship between expressions of Foxp3+ regulatory T (Treg) cells and Th17 cells, and pneumococcal carriage in the adenoids of children who were either positive or negative for pneumococci. METHODS: We collected adenoidal tissue and nasopharyngeal swab samples from children undergoing an adenoidectomy. Adenoidal mononuclear cells were isolated, cultured and then stimulated with culture concentrated supernatant (CCS) obta…
