Periodontal Surgery Involving Modified Widman Flap Procedure and Connective Tissue Graft for Generalized Aggressive Periodontitis: A Case Report.

We report a case of generalized aggressive periodontitis (AgP) requiring periodontal treatment including flap surgery and ridge augmentation. The patient was a 39-year-old woman who presented with the chief complaint of pus discharge from tooth #36. No other obvious signs of gingival inflammation were observed. Periodontal examination revealed multiple sites with a probing depth of ≥10 mm. Radiography showed pro-nounced bone defects in the maxillary incisors and molar region. Real-time PCR was used to detect Aggregatibacter actinomycetemcomitans, Porphyromonas gingivalis, and Tannerella forsyt

Periosteoplasty for soft tissue closure and augmentation in preprosthetic surgery: a surgical report.

Soft tissue closure is a critical factor in preprosthetic surgery and implant placement. In large transverse or vertical bone augmentations, there is often insufficient or very thin mucosa available. Soft tissue dehiscences and bone or implant exposure may result. Based on the application of a periosteal flap, the presented method solves this problem. The flap is prepared in the neighboring area and folded into the corresponding surgical area. It not only provides good soft tissue coverage but also results in soft tissue augmentation. Periosteoplasty has been successfully applied in over 60 pa

[Interaction between orthodontic tooth movement and hydroxyapatite ceramics].

A pilot study examined the clinical and histological effects of orthodontic tooth movement in hydroxyapatite-augmented ridges in animal models. Responses observed at the interface between the clinical root and the ceramo-osseous regeneration were negative: impairment of periodontal function had led to immobility and root resorption. On the basis of this experience one must advice against using this approach in humans at the present time. Further studies will be conducted to elucidate the relevant clinical questions.

Variation in bucco-palatal maxillary sinus width does not permit a meaningful sinus classification.

OBJECTIVES: The bucco-palatal sinus width (SW) appears as relevant factor for graft consolidation after maxillary sinus (MS) floor augmentation. The present study aimed to assess (a) SW at different height levels of posterior teeth, (b) possible factors influencing SW, and (c) whether a simple/meaningful sinus classification based on SW is possible. METHODS: The following parameters were recorded on computed tomographies of 76 edentulous and 86 partially edentulous maxillary quadrants displaying 383 tooth sites in total: (a) alveolar ridge height, (b) -area, (c) -width 2 mm apical to the alve

Morphological and cortical bone assessment of the edentulous posterior mandible using CBCT Implications for implant planning and mandibular cortical index evaluation.

BACKGROUND: This study aimed to investigate the relationship between posterior mandibular ridge morphology, cortical bone characteristics, and the Mandibular Cortical Index (MCI), and to assess their potential impact on the complexity of dental implant placement. Understanding these parameters is essential to optimize implant planning, minimize surgical complications, and improve clinical outcomes, particularly in regions where anatomical variations pose increased risks. METHODS: In this retrospective observational study, 100 cone-beam computed tomography (CBCT) scans of edentulous mandibles

Horizontal ridge augmentation using native collagen membrane vs titanium mesh in atrophic maxillary ridges: Randomized clinical trial.

BACKGROUND: Several techniques have been proposed to reconstruct deficient alveolar ridges including bone blocks, ridge splitting and guided bone regeneration (GBR). GBR has been successfully established in restoring horizontal bone deficiency. However, yet still there is a debate regarding the ideal barrier for GBR. PURPOSE: To evaluate the quantity and the quality of the bone gained using collagen membrane with 1:1 mixture of autogenous and anoraganic bovine bone mineral compared to titanium mesh with the same mixture of bone for GBR of horizontally deficient maxillary ridges. MATERIALS AN

Alveolar Bone Regeneration Using 3D-Printed, Patient-Specific, Resorbable PCL Scaffold: Prospective Cohort Study.

AIM: To evaluate clinical feasibility of 3D-printed, patient-specific, polycaprolactone (PCL) resorbable scaffolds for staged alveolar ridge augmentation. MATERIALS AND METHODS: Ten patients requiring staged bone augmentation underwent digital planning to virtually reconstruct the defect to its original contour. Patient-specific medical-grade PCL scaffolds were then fabricated using 3D printing and sterilised using γ irradiation. Following surgical access of the defect site, the scaffolds were loaded with a composite graft of autogenous bone, deproteinised bovine bone mineral and injectable p…

A Novel Approach for Horizontal Augmentation of Posterior Maxilla Using Ridge Split Technique.

Over the years, there have been a series of innovative approaches to the alveolar bone augmentation techniques. These have led to the modifications of the existing methods and the establishment of more efficient ways to obtain sufficient bone mass that is necessary for the implant procedure. The aim of this paper is to propose a novel augmentation technique, and to investigate its efficacy, particularly during the healing process of the maxillary alveolar bone prior to the implant treatment. During the application of the proposed methodology, first, the ridge-split technique was applied to the

Clinical outcomes of implants in atrophic ridges augmented with collagenated xenogeneic or autogenous bone: 4-year follow-up of a randomized clinical trial.

This study aimed to evaluate the long-term outcomes of dental implants placed in alveolar ridges augmented with collagenated xenogeneic bone blocks (CXBB) or autogenous bone blocks (ABB). In this non-interventional study, 20 patients (CXBB: n = 11; ABB: n = 9) from a previous randomized controlled clinical trial were followed for four years. These patients had undergone lateral alveolar ridge augmentation with either CXBB or ABB, with implants placed after a 30-week healing period. The following parameters were assessed: implant survival and success rates, peri-implant clinical parameters, Pin…

Evaluation of the Effectiveness of the Socket Preservation Technique Using Allogeneic and Xenogeneic Materials-A Preliminary Study.

BACKGROUND: The socket preservation technique involves filling the bone defect created after tooth extraction with a bone substitute material. This helps to reduce bone resorption of the post-extraction alveolar ridge. Various types of bone substitute biomaterials are used as augmentation materials, including autogeneic, allogeneic, and xenogeneic materials. The purpose of this study was to evaluate changes in alveolar ridge dimensions and alterations of optical bone density in sockets grafted with two different biomaterials. Additionally, bone biopsies taken from the grafted sites underwent h

U-Shaped Bone Splitting and Osteotome Techniques for Narrow Alveolar Ridge in Implant Surgery.

PURPOSE: Bone splitting and osteotome are common ridge expansion choices for patients with a narrow alveolar ridge requiring dental implant restoration. Despite the effectiveness of the 2 techniques in horizontal ridge augmentation, they could also result in severe complications, which is thus considered a contraindication. MATERIALS AND METHODS: A modification of the bone splitting and the osteotome techniques is proposed hereby as an alternative. RESULTS: The modified procedure reduces the tension of labial bone plate by creating a partial bone fracture at the most depressed area of labial

Atrophic Anterior Mandible Treated With Sandwich Osteotomy Without Mini-Screws and Mini-Plates: A Case Report With 7 Years of Follow-Up.

Restoring the anterior mandible may be challenging due to both insufficient height and width of the edentulous alveolar ridge; thus, this case report aimed to treat anterior mandibular atrophy by using the inlay technique without the use of mini-screws or mini-plates to stabilize the augmented bone fragments. A 19-year-old patient who lost his anterior teeth in an accident was treated with a horizontal osteotomy performed 4 mm from the alveolar ridge, with two oblique cuts made using an ultrasonic instrument, and the final phase of the osteotomy was performed with a lever for dental extraction

Local residual ridge augmentation with solid hydroxyapatite blocks: Part II--Correction of local resorption defects in 50 patients.

Replacement of anterior teeth with fixed or removable prostheses is often a compromise, because the resorbed residual ridge in the area of missing teeth cannot be ideally restored functionally and esthetically at the same time. To address this problem, 67 blocks of hydroxyapatite were placed subperiosteally to improve residual ridge resorption defects subsequent to loss of anterior teeth in 50 patients. The implants were evaluated clinically and radiographically for 6 months to 7 years after implantation. Results indicate that the suggested approach improves the esthetic results and the progno
