Invasive amebiasis: an update on diagnosis and management.

In its invasive form, the trophozoite is responsible for clinical syndromes, ranging from classical dysentery to extraintestinal disease with emphasis on hepatic amebiasis. Abdominal pain, tenderness and diarrhea of watery stool, sometimes with blood, are the predominant symptoms of amebic colitis. Besides the microscopic identification of Entamoeba histolytica, diagnosis should be based on the detection of specific antigens in the stool or PCR associated with the occult blood in the stool. Amebic dysentery is treated with metronidazole, followed by a luminal amebicide. The trophozoite reaches…

Epidemiology and treatment of amebiasis in Hué, Vietnam.

Hué, a city of about 300,000 inhabitants in Central Vietnam, is known as a "hot spot" for diseases caused by Entamoeba histolytica and in particular for amebic liver abscess (ALA), thus representing an excellent site for studies on the epidemiology and treatment of E. histolytica infections. Accordingly, during the last few years, various studies on this topic have been performed in Hué in close collaboration among the Hué Medical College, the Hué Central Hospital and the Bernhard Nocht Institute for Tropical Medicine, Hamburg, Germany. These studies comprised (i) comprehensive analyses of a c

Cutaneous amebiasis in a child and review of the literature.

Cutaneous amebiasis is a rare manifestation of infection with the parasite Entamoeba histolytica. We describe this infestation in a 9-year-old Mexican immigrant who presented with diarrhea and perianal ulcers. To our knowledge, this is only the tenth child with cutaneous amebiasis reported in the literature, and only the second from the United States. We review previous reports and emphasize the importance of a high index of clinical suspicion for early diagnosis and treatment to prevent potentially serious morbidity and even mortality.

[Utility and limitations of laboratory diagnosis of amebiasis].

Entamoeba histolytica is an invasive and pathogenic protozoan parasite that causes amebiasis. It must be distinguished from Entamoeba dispar, a nonpathogenic commensal parasite of the human gut lumen that is morphologically identical to Entamoeba histolytica. Diagnosis of amoebic colitis currently requires combination of microscopic examination of stool specimens with another technique allowing positive identification of the two species. Stool culture followed by zymodem analysis is considered as gold standard but is not applicable in routine practice. Detection of specific Entamoeba histolyti…

Gender distribution in asymptomatic and invasive amebiasis.

OBJECTIVE: The majority of individuals infected by the protozoan parasite Entamoeba histolytica experience subclinical infections. However, a small proportion of parasitized individuals develop severe invasive disease such as amebic dysentery or amebic liver abscess. Invasive amebiasis affects predominantly men; the usual explanation for this has been that men have a higher rate of asymptomatic infections and therefore experience a higher rate of invasive disease. To date, there is no convincing evidence of an increased rate of asymptomatic infection of men as compared with women. The purpose …

Cerebral amebiasis in the acquired immunodeficiency syndrome.

Rare cases of cerebral amebiasis have been described in AIDS patients. We report the case of a 46 year-old homosexual man with AIDS who developed an intermittent amnesia and a right palpebral ptosis. The cerebrospinal fluid contained 169 cells (75% lymphocytes). The patient died five days after hospitalization. Necropsy revealed thrombosis of small vessels of the periventricular regions as well as necrosis and hemorrhage of the periventricular tissue, cerebellum and brainstem. The inflammatory process was scarce and composed mainly of CD-68 positive macrophages. In these regions as well as in …

[Diagnostic certainty in intestinal amebiasis in Managua].

Three surveys were carried out in Managua. Nicaragua. In the first one it was found that 71% of the intestinal amebiasis cases of obligatory notification had no comparable diagnostic foundations; in the second one, 11.7% of the laboratory diagnoses as Entamoeba histolytica referred by 4 health centers were correct; and in the third one, it was obvious that only 9.4% of the doctors knew well what intestinal amebiasis is. The results suggest that intestinal amebiasis is being diagnosed excessively in Managua.

[Urinary amebiasis: a parasitosis of exceptional presentation].

We report a case of urinary tract amebiasis localized to the bladder. We reviewed the different ways secondary infection can spread to the urinary tract (rupture of hepatic amebic abscess, perforation in amebic dysentery, or hematogenous spread of amebas) and the primary form of presentation and spread that have been recently focussed upon, i.e. via the "ascending route" in deviant sexual habits of bisexuals and homosexuals, parasites in the anorectal area can colonize the urethra. We discuss the different diagnostic methods and treatment of urinary and intestinal amebiasis, as well as their p…

[Invasive amebiasis as a public health problem].

Amebiasis, an infection of humans with the protozoan Entamoeba histolytica, has a wide distribution in Mexico. The lumenal, asymptomatic infection, as measured by the presence of cysts in stools has been recorded from 2.4% at Ometepec, Guerrero to over 55% at Mixquic, D.F., but only a small percentage of those having intestinal infection will develop invasive amebiasis, the main clinical forms being dysentery and liver abscess. In Mexico City, from 0.8 up to 14% of cases of acute diarrhoea in children requiring hospitalization were found to be associated with E. histolytica. Serological survey…

Amebiasis. The ancient scourge is still with us.

Amebiasis is usually contracted in geographic areas where sanitation is poor, but outbreaks can still occur anywhere that drinking water becomes contaminated with sewage. In the majority of persons infected with the parasite, colonization of the intestine is asymptomatic. In others, symptoms of gastrointestinal distress can appear within a week. In rare cases, extra-intestinal amebiasis can cause abscesses in the liver or elsewhere. Many questions about the disease course in different patients remain to be answered. Diagnosis can be made through symptom identification; findings of right-upper-…

Cutaneous amebiasis of the face: a case report.

This paper describes a case of cutaneous amebiasis of the face in a 17-month-old girl. About four weeks prior to admission she had diarrhea with high suspicion of intestinal amebiasis. Fifteen days later she presented with edema of the right lower eyelid and epiphora. There was a vesicle in the inner angle of the eye which spontaneously drained, causing loss of the underlying tissue. The ulcer developed rapidly, destroying periorbital tissue. Both cytology and biopsy of the ulceration border showed abundant trophozoites of Entamoeba histolytica. Treatment was specific therapy and surgery.

[A case of a single pleural localization of amebiasis].

The diagnostic of isolated pleural amebiasis is exceptional even in endemic countries. The reported observation illustrates this eventuality. The relation of this clinical factor leads to a general review on the pleuro-pulmonary manifestations of amebiasis, their mechanism and their treatment.

Sonographic features of hepatic amebiasis in childhood.

We report the ultrasonographic features of hepatic amebiasis in six children. The spectrum of change ranges from a typical abscess to early "hepatitis." Ultrasonography is the primary imaging modality in patients suspected of having this disease and is excellent in localizing disease. This is important in terms of abscess drainage and in recognizing complications such as diaphragmatic perforation and empyema.

Nitroimidazoles in the treatment of trichomoniasis, giardiasis, and amebiasis.

Nitroimidazoles have been extensively evaluated in the treatment of trichomoniasis, giardiasis, liver and intestinal amebiasis. The most widely used are metronidazole, tinidazole, ornidazole, and secnidazole. Tinidazole, ornidazole, and secnidazole have a much longer half-life than metronidazole, allowing single-dose or once daily administration. Nitro-5-imidazoles remain extremely effective drugs for treating protozoans, Trichomonas vaginalis, Entamoeba histolytica, and Giardia intestinalis. Although all are suspected of potential carcinogenicity, they are the drugs of choice for treating pro…

Cutaneous amebiasis of the eyelid with extension into the orbit.

A 4-month-old girl was admitted to hospital with extensive ulceration and erosion of the left eyelid where 17 days earlier the skin had been broken when hit by a ball. Despite antibiotic therapy the lesion expanded to involve the orbit. Biopsy of the eyelid on the 4th day in hospital, and examination of the eye and attached tissues removed on the 6th, failed to detect amebae until after the infant's death due to a blood transfusion 2 weeks later. Re-examination of the tissues revealed abundant amebae with characteristic morphology of Entamoeba histolytica in subcutaneous tissues of the biopsy
