Transpositional microsurgery in multiple digital amputations.

In 34 of the 62 patients treated for complete multiple digital amputations, the severed part was not replanted in its anatomical position, but in the place of the most useful stump. This procedure is defined as transpositional digital microsurgery and refers to the transposition and replantation of any digit to another stump which plays a more significant role in the function of the hand. Twenty-eight patients had transposition of a digit, while six patients underwent thumb transposition. All except six of the transposed digits survived, while all of the thumbs survived the surgical procedure.

Prosthetic fitting in a patient with a transtibial amputation due to a congenital vascular malformation of the right leg.

BACKGROUND: The problems of prescribing a prosthesis for a young girl with severe congenital vascular malformation deformity leading to a transtibial amputation. CASE DESCRIPTION AND METHODS: Due to the high risk of recurrent bleeding and limitations regarding full weight bearing of the stump, a normal socket fitting process was not possible. Using a multidisciplinary approach, a prosthesis was designed to enable full weight bearing in a flexed knee position with ischial tuberosity support to prevent full weight bearing on the tibial part of the stump. FINDINGS AND OUTCOMES: After training a

Post-traumatic lower-limb amputation: Indications, techniques and results.

This study presents a review of traumatic lower-limb amputation, addressing 6 questions. (1) In what circumstances is amputation necessary in traumatology? (2) Are scores and results analyses contributive to decision-making for emergency amputation? Amputation can be primary, when directly caused by the trauma, salvage if limb conservation would be life-threatening, a necessity if functional prognosis is poor, or else necessitated by a mass-casualty context. Score systems and results analysis can help identify patients who would benefit from amputation or salvage, but applications are controve

FGF8 induces bone and joint regeneration at digit amputation wounds in neonate mice.

Due to increases in vascular diseases, the incidence of limb loss is predicted to more than double in the next quarter century. Therefore, developing a greater understanding of the latent regenerative capacity in mammals is a significant and growing goal. Mammals, including humans and mice, have limited regenerative capacity following limb amputation, with regenerative responses restricted to amputations transecting the distal digit tip (P3). Unlike P3, amputations of the adjacent skeletal segment, the middle phalanx, P2, are non-regenerative and result in bone truncation and soft tissue scar

Indications for amputations in Ile-Ife, Nigeria.

This is a retrospective study of limb amputations in Ile-Ife, Nigeria during a thirteen-year period (1987-1999). 82 patients were studied with a mean age of 35 +/- 22 years. 63 of the patients were adults, while 19 patients were children aged 15 years and below. Trauma was indicated as a reason for amputation in 60 patients (73.4%). Road traffic accident with peripheral vascular compromise was the single most common reason for amputation (41.7%). Forty-seven of the 82 patients had lower limb amputations, while the others were in the upper limbs. There was delay in performing amputation in some

Modified tibiofibular synostosis vs. Burgess stump in a bilateral amputee: a case report.

Despite trans-tibial amputation being the most common major amputation, consensus on optimal stump management remains limited. Cohort studies comparing Ertl-tibiofibular synostosis (TFS) and Burgess-type stumps (BTS) have yielded conflicting results. We present the case of a bilateral trans-tibial amputee with a modified TFS on the right and a conventional BTS on the left. Instead of traditional myodesis, a myoplastic sling was created attaching residual tendons distal to the bone bridge to improve tendon sliding and perceptive feedback. Outcomes were assessed via load distribution, temporospa

Targeted Muscle Reinnervation in the Upper Extremity Amputee: A Technical Roadmap.

Targeted muscle reinnervation (TMR) offers the potential for improved prosthetic function by reclaiming the neural control information that is lost as a result of upper extremity amputation. In addition to the prosthetic control benefits, TMR is a potential treatment for postamputation neuroma pain. Here, we present our surgical technique for TMR nerve transfers in transhumeral and shoulder disarticulation patients.

Recent trends in lower extremity amputation. Surgery and rehabilitation.

With the use of modern prosthetic appliances and the initiation of early ambulation, satisfactory function can be expected in the vast majority of below-knee amputees. The rehabilitation of the above-knee amputee is more difficult and the performance of these patients does not compare favorably with that of the below-the-knee amputee. Immediate postsurgical fitting appears to have resulted in a higher percentage of successful below-knee amputations in patients with peripheral vascular disease. The rigid dressing provided by the temporary prosthesis applied to the stump immediately after the am

Dermatologic Conditions Following Limb Loss.

The use of prostheses in individuals with limb loss increases the risk of maceration, friction, and pressure-induced skin injuries due to the vulnerability of the residual limb and the close contact with the socket. Poorly fitting prostheses exacerbate these issues, especially for those with immunosuppression or vascular conditions. Skin pathologies disrupt daily prosthetic limb use and impact the independence of those with limb loss. Preventive measures, including proper prosthesis socket fitting and meticulous skin care, are crucial. This review explores residual limb-site skin pathologies a

[The necessity for treatment paths for lower limb prosthetics : All that glitters is not gold!].

BACKGROUND: Frequently occurring problems in the fitting of leg prostheses are either, for a prosthetic fitting, insufficiently resilient leg stumps or insufficiently adapted prostheses. For this reason, there has been a demand at the political level to establish treatment pathways and a quality standard for the care of people after leg amputations. QUESTION: How can a joint, interdisciplinary approach to the provision of aids be developed? Does interdisciplinary care of amputees lead to an improvement in the quality of care? MATERIAL AND METHOD: A compendium "Quality standard in the field o

A novel, low-cost transradial socket fabrication method using mass-producible components and expanding rigid foam.

BACKGROUND: Millions of people in low- and middle-income countries lack access to prosthetic care. A well-fitting, durable socket is important for prosthesis comfort and function, but conventional fabrication techniques require highly trained clinicians and specialized equipment. OBJECTIVES: To increase access to prosthetic care by developing a simple, low-cost socket fabrication method that does not require specialized equipment or electricity, and can be performed by persons with minimal prosthetic training. STUDY DESIGN: Socket fabrication methods and socket function were evaluated in a p

Structural testing of lower-limb prosthetic sockets: A systematic review.

A lower-limb prosthetic socket is the custom-made structural element interfacing the residual limb of a person with an amputation to their prosthetic leg comprising off-the-shelf componentry. The socket can be subject to mechanical failure, especially when new fabrication methods and materials are introduced (e.g. 3D printing). Failures can have severe consequences for patients. A systematic review was conducted to collect information about available socket mechanical testing methods, to support the definition of widely accepted guidelines. To this aim the structural testing methods were revie

Amputation Revision Surgery - Refining the surgical approach. Ten years of experience and 250 cases, impressions, outcomes, and thoughts for the future.

INTRODUCTION: The decision for revision amputation surgery requires a multi-disciplinary approach and the evidence on outcome data available in the current literature is limited. The aim of this observational, retrospective study was to investigate in patients undergoing stump revision, whether factors such as pathology and reason for the revision surgery affect surgical 'success' rates as primary outcomes. Secondary outcomes of interest include rehabilitation outcomes and complication rates. METHODS: From December 2008 to November 2018, 250 amputation revision procedures were performed at ou

[Amputee rehabilitation: taking charge, functional assessment and goal setting].

Patients with lower limb amputations need the help of a multi-professional care team. There is no single professional who encompasses the skills required by patients who generally present multiple pathologies: skills in the care of wounds which are often dehiscent, knowledge of prosthetics, considering the fast evolving technologies in the field, psychological skills, essential to help the patient face their somatic conditions and resultant changes in body image. The physiatrist manages a care team which must prioritise treatments and intervention methods, and define practicable objectives in

Replantation of a multiple digit and circular palm amputation: a case report.

A case of successful replantation of multiple digits and circular palm amputation caused by an industrial punch force is reported. The two-level amputation was reconstructed by first replanting the severed fingers to the palmar segment and then connecting the palm segment to the hand stump. An arterial crisis was treated with urokinase, and postoperative functional recovery is described.
