BACKGROUND: Anaphylactic reactions during a barium enema have been attributed to allergy to latex on the barium enema device. The observation of anaphylaxis during barium enema without latex exposure or latex allergy led to the performance of an allergy skin test to the barium enema solution.
METHODS: Individual components of the barium enema solution were obtained for double-blind skin testing. A RAST to identify specific IgE antibodies to the skin test active agent was established.

To verify production of nitric oxide (NO) in anaphylaxis, we measured NO in peripheral tissue in anaphylactic rabbits using an NO-sensitive electrode. Rabbits were sensitized to horse serum, which was later administered over 10 s into the systemic circulation to induce anaphylaxis. Blood pressure (BP), central venous pressure (CVP), heart rate, and NO were recorded continuously for 80 min after antigen challenge. Idiopathic urticaria is a common and usually self-limited illness that is occasionally associated with life-threatening idiopathic anaphylaxis. A case is presented in which urticaria existed for 30 years before tests that revealed penicillin allergy as the probable cause. Our findings suggest that evaluation for penicillin allergy may be useful in some cases of urticaria.

Latex products have recently been identified as the cause of severe intraoperative anaphylactic reactions. We have identified a group of pediatric patients who appear to be at increased risk for such reactions. Fifteen patients with either spina bifida or congenital urologic abnormalities experienced 19 intraoperative anaphylactic reactions. All patients had frequent previous exposures to rubber materials since infancy as part of their management and/or investigative procedures. To establish a useful laboratory protocol to investigate possible cases of fatal anaphylaxis, we measured mast-cell-derived tryptase levels and allergen-specific immunoglobulin E (IgE) antibody levels in sera obtained prior to or within 24 h after death from 19 anaphylaxis victims. Immediate allergic reaction to natural latex of Hevea brasiliensis has been well documented. We report 17 cases, all in women. Twelve had an anaphylactic reaction during surgery and five had contact urticaria. Reactions to prick tests done through a washed surgical glove and with natural latex were positive in 15 of 17 patients and in 16 of 16 patients, respectively; specific IgE was found in 12 of 17 patients. In 206 control subjects, results of a prick test through a surgical glove were negative.
