Large language models are enabling a new generation of health agents, yet their ability to address the diverse needs of individuals in everyday health settings remains underexplored1,2. Here we developed a comprehensive personal health agent that reasons over multimodal data from consumer wellness devices and medical records to provide personalized health support. Through analysis of web search and health forum queries, together with user-centred research involving end users and health experts, we identified four key user journeys: general health knowledge, personal data interpretation, wellne

BACKGROUND: Workplace mistreatment in graduate medical education is typically measured using instruments with validity evidence, yet these tools may inadequately capture context-dependent experiences that vary by frequency, source, and setting.
OBJECTIVE: To examine response frequency distributions from 2 mistreatment instruments used with residents and fellows whereby ambiguous ("grey") experiences appeared distinct from explicit mistreatment, and to assess the differential associations of each with resident well-being and burnout.
METHODS: Using data collected in 2024 from the Workplace Ps

Spinal metastases are common in the course of a neoplasm and involve multiple pathophysiological mechanisms. The venous vascularization of the spine plays an important role in the onset and development of these secondary lesions. Their progression leads to evolving risks of fracture and neurotoxicity, all of which compromise quality of life and survival. Understanding these harmful developments requires a knowledge of the bony and ligamentar structures and of the vertebral contents, including particularities of its venous drainage.

The cerebral operculum comprises the cortical folds of the frontal, central, parietal, and temporal lobes that overlie the insula. Although historically described as a simple anatomical "cover," the operculum is now recognized as a morphologically complex periSylvian region embedded within multiple cortico-cortical and cortico-subcortical networks. In this review, a comprehensive synthesis of opercular organization is provided, integrating topographic anatomy, structural connectivity, and vascular supply. The topographic anatomy of the operculum is described in relation to periSylvian sulcal l…

The brainstem connects the forebrain to the spinal cord. It consists of three layers, the ventral basis, the intermediate tegmentum, and the dorsal tectum. Caudo-rostrally, the brainstem is divided into the medulla oblongata or myelencephalon, the metencephalon, and the mesencephalon. The tegmentum is the most conserved part of the brainstem and contains the motor and sensory nuclei of cranial nerves (CN) III-XII. They are arranged in a mediolateral direction, and, unlike the spinal cord, their afferent and efferent fibers are not macroscopically separated. The tegmental reticular formation, t

The present narrative review delves into the multifaceted roles of the cerebellum, highlighting its significance beyond traditional motor functions to encompass cognitive and behavior-related processes, as evidenced by advancements in functional neuroimaging. We provide a comprehensive summary of the cerebellum's embryological development, intricate microanatomy, macroanatomy, and vascular anatomy of the cerebellum, revealing how these aspects contribute to its unique circuitry and operational capabilities. A non-systematic literature search was conducted using the PubMed database, focusing on…

An in-depth understanding of the anatomy of the craniocervical junction (CCJ) is indispensable in skull base neurosurgery. In this paper, we discuss the osteology of the occipital bone, the atlas (C1) and axis (C2), the ligaments and the muscle anatomy of the CCJ region and their relationships with the vertebral artery. We will also discuss the trajectory of the vertebral artery and review the anatomy of the jugular foramen and lower cranial nerves (IX to XII). The most important surgical approaches to the CCJ, including the far lateral approach, the anterolateral approach of Bernard George an…

The glossopharyngeal nerve is a complicated and mixed nerve including sensory, motor, parasympathetic, and visceral fibers. It mediates taste, salivation, and swallowing. The low cranial nerves, including IXth, Xth, and XIth, are closely related, sharing some nuclei in the brainstem. The glossopharyngeal nerve arises from the spinal trigeminal nucleus and tract, solitary tract and nucleus, nucleus ambiguous, and inferior salivatory nucleus in the brainstem. There are communicating branches forming a neural anastomotic network between low cranial nerves. Comprehensive knowledge of the anatomy o…

PURPOSE: Endoscopic third ventriculostomy (ETV) is a surgical procedure that can lead to complications and requires detailed preoperative planning. This study aimed to provide a more accurate understanding of the anatomy of the third ventricle and the location of important structures to improve the safety and success of ETV.
METHODS: We measured the stereotactic coordinates of six points of interest relative to a predefined stereotactic reference point in 23 cadaver brain hemi-sections, 200 normal brain magnetic resonance imaging (MRI) scans, and 24 hydrocephalic brain MRI scans. The measurem…

BACKGROUND: The purpose of this study was to quantify the articular surfaces of the naviculocuneiform (NC) joint to help clinicians better understand common pathologies observed such as navicular stress fractures and arthrodesis nonunions.
METHODS: Twenty cadaver NC joints were dissected and the articular cartilage of the navicular, medial, middle, and lateral cuneiforms were quantified by calibrated digital imaging software. Statistical analysis included calculating the mean cartilage surface area dimensions of the distal navicular and proximal cuneiform bones. Length measurements on the nav

The spinous processes act as a lever for attachments of muscles and ligaments. Spinal imaging is commonly performed as a diagnostic test for pain and radiculopathy. A myriad of incidental or unexpected findings, both potentially asymptomatic and symptomatic, may be encountered during the interpretation of these images, which commonly comprise radiographs, Computed Tomography (CT) and Magnetic Resonance Imaging (MRI). Isolated lesions of the spinous process, although less common, are some of the lesions that may be encountered and can present a diagnostic dilemma. These can range from congenita
