OBJECTIVE: To investigate the effects of EEG suppression and anesthetics on variability of electrical stimulus thresholds during functional cortical motor mapping, and the possible influences of age, lesion location and pathology.
METHODS: Multivariate regression analysis was performed to study these relationships in 60 cases of successful mapping using the monopolar multipulse train technique.
RESULTS: An increase in the length of EEG "flats" by 1s produced an increase in stimulus threshold by 1.08 mA (p=0.0004). Administration of TIVA (total intravenous anesthesia) or inhalational agents p

Mitochondrial encephalopathy, lactic acidosis, and stroke-like episodes (MELAS) is a type of mitochondrial disease that is characterized by stroke-like seizures. For these patients, serious, unexpected complications have occurred during and following anesthetic exposure. Provision of anesthesia is challenging, including the choice of anesthetic agents. We here report a case of general anesthesia management for a patient with MELAS. A 46-year-old woman was diagnosed with MELAS at the age of 40. She subsequently underwent cochlear implantation for hearing loss. Anesthesia was induced with midazo

Myasthenia gravis, a disorder of neuromuscular transmission, presents a unique challenge to the perioperative anesthetic management of morbidly obese patients. This report describes the case of a 27-year-old morbidly obese woman with a past medical history significant for myasthenia gravis and fatty liver disease undergoing bariatric surgery. Anesthesia was induced with intravenous agents and maintained with an inhalational and balanced intravenous technique. The nondepolarizing neuromuscular blocker Cisatracurium was chosen so that no reversal agents were given. Neostigmine was not used to an

Pena-Shokeir syndrome is a rare, early lethal disease. It is characterized by fetal growth restriction; craniofacial deformities, for example micrognathia and microcephaly; multiple ankyloses; and pulmonary hypoplasia. For patients with this syndrome, maintenance of airway and control of perioperative respiratory complications are important for anesthetic management. We report 3 pediatric cases of Pena-Shokeir syndrome undergoing tracheostomy and arthrolysis under general anesthesia using sevoflurane, nitrous oxide, fentanyl, and vecuronium bromide. Anesthetic procedures including mask ventila

Status epilepticus (SE) refractory to benzodiazepines and other antiepileptic agents is managed with intravenous anesthetic compounds, such as thiopental, propofol or midazolam. These drugs display quite different pharmacodynamic and pharmacokinetic properties, but have not been prospectively compared to date. Their use is clearly advocated for the treatment of generalized convulsive SE, whereas partial-complex, or absence SE are generally managed less aggressively, in consideration of their better prognosis. The most important aspect seems to be related to the correct use of these anesthetics

The losses of righting reflex produced by various doses of phenobarbital in mice at 1 atm O2 versus 1 atm O2 plus 102 atm He were determined. The resulting dose-response curve at pressure gave an ED50 that was 64 per cent larger than the ED50 at 1 atm. This increment is essentially the same as that found for gaseous anesthetics under similar test conditions. The quantitative similarity of the results of pressure reversals of barbiturate and inhalational anesthetics suggests that the mechanisms or sites of action of these agents are similar. However, the dose-response curve at 103 atm was steep

Drug discrimination procedures were used to compare the discriminative stimulus properties of ETOH and volatile inhalants. Mice were trained to discriminate between i.p. injections of ETOH (1 g/kg) and saline in a two-lever operant task. Stimulus generalization was examined after 20-min inhalation exposures to toluene (300-5400 ppm), halothane (500-8000 ppm) and 1,1,1,-trichloroethane (TCE; 125-14000 ppm). Oxazepam (10-30 mg/kg i.p.) also was tested. Concentration- or dose-related increases in ETOH-lever responding were observed with toluene, halothane, TCE and oxazepam. Shared discriminative

Stents are inserted for severe stenosis of the trachea owing to malignant tumors, but an incorrectly positioned stent can cause airway obstruction. Here, we report the anesthetic management of a patient undergoing removal of an incorrectly positioned stent. A 56-year-old man had a stent inserted in the trachea for stenosis caused by a tumor. One month later, he was scheduled for reinsertion of a stent for tracheal stenosis owing to growth of the tumor, but the stent was inserted in the left main bronchus interfering ventilation to the right lung. Therefore, removal of the stent under general a

Effects of inhalation of several organic solvents on left ventricular contractility were investigated by using left ventricular dp/dt in anesthetized dogs. The first derivative of the left ventricular pressure was obtained by an electronic differentiating circuit and recorded continuously. As organic solvents, 1,1,1-trichloroethane (1,1,1-TCE), trichloroethylene, tetrachloroethylene, toluene, m-xylene and trichloro-trifluoroethane (FC-113) were used in this study. Each solvent was separately inhaled for a period of approximately 2 min. In the acute inhalation experiments, peak dp/dt that was t

UNLABELLED: The choice of anesthetic protocol for canine cesarean section (CS) remains debated, with maternal stability and neonatal outcomes as key concerns. Propofol and alfaxalone are both used for induction, but comparative data on their intraoperative and neonatal effects are limited. The objective of this study was to compare propofol and alfaxalone for induction, each followed by isoflurane maintenance during Cesarean section (CS) in bitches, with respect to maternal hemodynamics, anesthetic requirements, neonatal vitality, and umbilical cord blood parameters.

The authors report four anesthetic experiences of a child with moyamoya disease for two occasions of angiography and bilateral encephalo-duro-arterio-synangiosis (EDAS). For angiography anesthesia was maintained with inhalation of halothane-nitrous oxide-oxygen under spontaneous respiration. During the first angiography PaCO2 was 59.5mmHg and 55.2mmHg and it was 56.0mmHg during the second angiography. For EDAS, anesthesia was managed under controlled ventilation and avoiding hyperventilation. Blood pressure was stable. The patient awoke well. Under anesthetic management of a child with moyamoy

Selected cardiopulmonary variables were measured in anesthetized, male, White Leghorn type chickens before, during, and after 60 min exposure to various inhaled SO2 concentrations ranging from 0 to 5000 ppm. In one series of experiments, birds breathed through their nostrils and mouth; in a second series they breathed through a tracheal cannula. Exposure to 5000 ppm SO2 rapidly caused death in all birds with cannulated tracheae and in 4 of 5 birds with an intact respiratory system; one bird in each series of experiments died when exposed to 1000 ppm. Inhaling 100 ppm SO2 did not alter heart ra

Although inhalant abuse is common, it is one of the most neglected and overlooked forms of substance abuse. Inhalants refer to a wide variety of substances including volatile solvents, aerosols, gases, and nitrites. The mechanism of action of inhalants has not been fully defined. Several molecular targets contribute to the pharmacology, including ion-channel proteins that control neuronal excitability. These agents interact with various receptors and can cause changes in cell-membrane fluidity and nerve-membrane ion channels. Three main pharmacologic categories of inhalants, namely, volatile s

N-methyl-d-aspartate (NMDA) receptors are important in mediating excitatory neurotransmission in the nervous system. They are preferentially inhibited by some general anesthetics and have, therefore, been implied in the mediation of their effects. This review summarizes the main research findings available related to NMDA receptors and their role in anesthesia. The contribution of NMDA receptors to the anesthetized state is discussed separately for each of its components: amnesia, analgesia, unconsciousness and immobility. Anesthetic-induced unconsciousness and immobility have received the mos

Many adjuvant drugs have demonstrated anesthetic-sparing properties when combined with volatile anesthetics. Nitrous oxide is combined with volatile anesthetics to reduce the concentrations of volatile anesthetics required to produce anesthesia. Analgesic doses of opioids clearly reduce the requirement for inhaled anesthetics in both human patients and experimental animals. We performed this study to determine whether the combination of nitrous oxide and morphine decreased isoflurane minimum alveolar anesthetic concentration (MAC) even further in the rat. Fifty-eight female rats were used. The

Given the negative health impacts of climate change, clinicians have a fundamental responsibility to take an active role in mitigating the environmental impact of their practices. Inhaled anaesthetics are potent greenhouse gases, including nitrous oxide (N2O), with their long atmospheric lifetime, high global warming potential, and ozone-depleting properties. However, few clinicians realise that losses from central N2O supply systems account for the vast majority of overall N2O consumption in healthcare. Central N2O supply systems are standard in most facilities, compounding the impact of thes
