Smoking-associated factors in myocardial infarction and unstable angina: do gender differences exist?

The aim of this study was to investigate demographic and psychological characteristics associated with smoking in patients with acute coronary syndrome (myocardial infarction or unstable angina). Interviews were conducted with 348 consecutive hospitalized patients with acute coronary syndrome and included questions about demographic characteristics, coffee consumption, heart disease risk perception, economic status, alcohol consumption, depression, anxiety, and stress. Female group multivariate analysis showed that smoking in females was significantly and negatively associated with age, heart

Tongxinluo (Tong xin luo or Tong-xin-luo) capsule for unstable angina pectoris.

BACKGROUND: Tongxinluo capsule is a medicine consisting of traditional Chinese herbs and insects used for cardiovascular diseases in China and some other Asian countries. To date the evidence of its effect has not previously been subject to systematic review, making it difficult to derive robust conclusions about its actual benefits, and indeed, possible harms. OBJECTIVES: To assess systematically the effects of tongxinluo capsule in people with unstable angina pectoris. SEARCH STRATEGY: We searched the Cochrane Central Register of Controlled Trials (CENTRAL) on The Cochrane Library, Issue 4

Best evidence topic report. Clopidogrel plus aspirin or aspirin alone in unstable angina.

A 37-year-old HIV-positive Gambian woman presented with spastic weakness of the right leg six years after receiving treatment for tuberculous meningitis (TBM). Magnetic resonance imaging (MRI) of the spine showed a multiloculated syrinx in the thoracic spinal cord extending from the T2 to the T11 level. Syringomyelia and syringobulbia have been reported as complications of TBM. We describe the first case of syringomyelia as an uncommon late complication of treated TBM in the setting of HIV infection. Early recognition of this rare entity may avoid irreversible neurological sequelae.

[Platelets and platelet microparticles glycoprotein IIb/IIIa complex in patients with unstable angina].

UNLABELLED: Platelet activation is known to play a key role in pathogenesis of acute coronary syndrome (ACS). Platelet activation leads to increased numbers of glycoprotein (GP) IIb/IIIa complex on the platelet membrane. On the other side antiplatelet therapy in patient with unstable angina can suppress the actions of platelet. THE AIM: To estimate by flow cytometry GP IIb/IIIa expression on the platelets and platelet microparticles membrane. MATERIAL AND METHODS: The study involved 21 patients with unstable angina, before (group B1) and after treatment (group B2). All subjects received aspi

Skeletonized bilateral internal mammary arteries for non-elective surgical revascularization in unstable angina.

OBJECTIVE: The aim of this study was to evaluate the feasibility, safety and outcome of skeletonized bilateral internal mammary arteries (BIMA) in patients with unstable angina (UA) undergoing non-elective myocardial revascularization. METHODS: Between January 1997 and December 2003, 758 patients, mean age 62+/-12 years, underwent non-elective coronary artery bypass grafting (CABG) for unstable angina. Two hundred and five (27%) were operated emergently and 503 (73%) urgently. BIMA were employed in 320 (42%) patients (Group B) and isolated left IMA and/or saphenous vein grafts in the remainin

Sympathetic neural hyperactivity and its normalization following unstable angina and acute myocardial infarction.

Impaired autonomic function occurs after AMI (acute myocardial infarction) and UA (unstable angina), which may be important prognostically. However, the pattern of sympathetic nerve hyperactivity has been investigated only after AMI. We aimed to quantify central sympathetic output to the periphery in patients with UA, investigate its progress over time relative to that after uncomplicated AMI and to explore the mechanisms involved. Muscle sympathetic nerve activity (MSNA) assessed from multiunit discharges and from single units (s-MSNA) was obtained in matched patients with UA ( n =9), AMI ( n

Risk Stratification in Patients with Unstable Angina and Non-ST-elevation Myocardial Infarction.

Risk stratification in acute coronary syndromes is important both for prognosis and for treatment. Consistently, using any of a variety of clinical predictors of risk, patients at highest risk for poor outcomes derive the greatest benefit from aggressive therapy with early coronary angiography, glycoprotein IIb/IIIa antagonists, or low molecular weight heparins. By contrast, patients at low risk may be managed conservatively without long-term impact on their risk of death or myocardial infarction. Several clinical and laboratory parameters have been identified as independent, powerful predicto

Diffusion of troponin testing in unstable angina patients: adoption prior to guideline release.

BACKGROUND: We examined diffusion of troponin testing in Medicare patients with unstable angina before the release of year 2000 American College of Cardiology/American Heart Association guidelines recommending measurement in all patients with acute coronary syndromes. METHODS: We identified unstable angina admissions from Medicare administrative files for 22 Alabama hospitals over two time periods: 03/1997-02/1998 and 01/99-12/1999. Data were obtained from chart abstraction. Patients not confirmed for unstable angina by a clinical algorithm were eliminated. RESULTS: For 1997-1998, 1116 (87.7

Relationship between CRP and clinical course of unstable angina depends on assay method.

BACKGROUND: The CRP concentration has been identified as a cardiovascular risk factor in healthy volunteers and in patients with stable and unstable angina pectoris. There is little information about the comparability of the results of different CRP assays and it has also not been evaluated whether different assays give different associations with risk. METHODS: We studied this in the APRAIS study, a study on 211 patients with unstable angina pectoris who were admitted to hospital and in whom we studied the association between the CRP concentration on admission and the in-hospital clinical co

Long-term outcome in patients with unstable angina treated by coronary balloon angioplasty.

UNLABELLED: Comparison of balloon angioplasty results in 472 patients with stable angina (SA) and 158 patients with unstable angina (UA) in 5-year follow-up is reported. Clinical success rate did not differ significantly, while periprocedural complications rate was higher in UA group (22.3 vs. 11.1%, P<0.001). During follow-up UA patients demonstrated higher: restenosis rate (48.5 vs. 30.4%, P<0.001), incidence of myocardial infarction (8.8 vs. 3.0%, P=0.004), although cardiac mortality did not differ significantly (2.2 vs. 1.6%). Reintervention rate in patients with unstable angina resultant

Percutaneous coronary intervention after subcutaneous enoxaparin pretreatment in patients with unstable angina pectoris.

BACKGROUND: Subcutaneous low-molecular-weight (LMW) heparins can effectively replace unfractionated heparin in patients with unstable angina or non-Q-wave myocardial infarction. However, the optimal anticoagulation strategy for these patients when they require cardiac catheterization is still unclear. Therefore, we evaluated a new and simple strategy of anticoagulation in these patients. METHODS AND RESULTS: A total of 451 consecutive patients with unstable angina/non-Q-wave myocardial infarction were treated for at least 48 hours with subcutaneous injections of enoxaparin (1 mg [100 IU]/kg e

Prior aspirin use in unstable angina predisposes to higher risk: the aspirin paradox.

BACKGROUND: Aspirin is the most widely prescribed agent used in prevention of coronary thrombosis. Thus, many patients presenting with acute coronary syndromes have a history of aspirin usage. METHODS: To assess response to medical therapy in patients taking aspirin prior to admission with an acute coronary syndrome, we reviewed outcomes data from the Efficacy and Safety of Subcutaneous Enoxaparin in Non-Q-wave Coronary Events (ESSENCE) and Platelet Receptor Inhibition in Ischemic Syndrome Management in Patients Limited by Unstable Signs and Symptoms (PRISM-PLUS) studies. RESULTS: Patients w

Unstable angina and non-ST-segment elevation myocardial infarction: perspectives on combination therapy.

We reviewed recent clinical data on the management of unstable angina and non-ST segment elevation myocardial infarction. We concentrated on the use of new therapies, particularly in combination with both older agents and other new methods, in order to present health care providers with an overview of available treatment options. The clinical trials reviewed herein provide strong evidence and proof of principle that combination therapies targeting 1) platelet function (aspirin, thienopyridines, and GP IIb/IIIa antagonists), 2) the coagulation cascade (unfractionated heparin and low-molecular-w

The association between coronary microvascular dysfunction and HbA1c in patients with unstable angina pectoris.

BACKGROUND: Angiographic microvascular resistance (AMR) is an effective parameter for assessing coronary microcirculatory function, and studies have indicated its association with adverse cardiovascular outcomes. However, the relationship between glycemic metabolic status and AMR levels requires further investigation. METHODS: A total of 291 patients with unstable angina who underwent percutaneous coronary intervention (PCI) in the Cardiology Department of the Second Affiliated Hospital of Xuzhou Medical University from December 2022 to December 2023 were consecutively included, Based on the

Development and validation of diagnostic and prognostic models for heart failure in unstable angina patients.

OBJECTIVE: To develop diagnostic models to predict initial heart failure (HF) hospitalisation in patients with unstable angina (UA) and prognostic models to predict rehospitalisation for recurrent or new-onset HF after discharge, aiming to enhance early identification and patient care strategies. METHODS: We retrospectively analysed data from 12 857 acute coronary syndrome patients (January 2015 to March 2023). After screening, 7092 UA patients were included and randomly divided into a training cohort (4964) and a validation cohort (2128). Logistic regression and least absolute shrinkage and

Efficacy of ticagrelor and clopidogrel in treating unstable angina and their effects on serum inflammatory factors.

To evaluate the comparative efficacy of Ticagrelor and Clopidogrel in treating patients with coronary heart disease and unstable angina, as well as their effects on serum inflammatory factors, thereby providing a solid foundation for future clinical diagnosis and treatment. The frequency of angina attacks in the Ticagrelor group was lower than in the Clopidogrel group (P<0.05). The duration of angina and shortness of breath were also shorter in the Ticagrelor group (P<0.05). The levels of C-reactive protein (CRP), interleukin-6 (IL-6), interleukin-18 (IL-18), matrix metalloproteinase-9 (MMP-9)
