Fatal thrombosis of antithrombin-deficient mice is rescued differently in the heart and liver by intercrossing with low tissue factor mice.

BACKGROUND: We previously reported that the targeted disruption of murine antithrombin (AT) gene resulted in embryonic lethality before 16.5 gestational days (gd) because of severe cardiac and hepatic thrombosis. OBJECTIVE AND METHODS: To investigate the influences of lowered tissue factor (TF) activity upon hypercoagulation of AT-/- embryos, we crossed AT+/- with low TF (mTF-/- hTF+) mice to yield homozygous AT-deficient mice with the extremely low TF activity, that is expressed from the inserted human TF mini gene. RESULTS: AT-/- embryos either with 50% TF (AT-/- mTF+/- hTF+) or with low (

There is no effect on remodeling of vascular wall 4 weeks after local delivery of antithrombin in a porcine model of coronary overstretching.

BACKGROUND: Antithrombin III is an effective endogenous inhibitor of thrombin with antiproliferative and anti-inflammatory capabilities. Systematic administration of direct thrombin inhibitors as well as of antithrombin has proven effective at reducing formation of neointima after injury to vessel wall in various animal models. Local delivery of antithrombin attenuates deposition of platelets after balloon injury to porcine coronary vessels. OBJECTIVE: To test our hypothesis that local delivery of antithrombin affects remodeling of vessel wall after balloon injury to the left anterior descend

Prothrombin fragment F1 + 2 and thrombin-antithrombin III complex are useful markers of the hypercoagulable state in atrial fibrillation.

It is well known that atrial fibrillation (AF) is one of the most important diseases that predispose patients to thrombosis. We have attempted to identify patients with AF in the hypercoagulable state by measuring molecular markers such as thrombin-antithrombin III complex (TAT) and prothrombin fragment 1 + 2 (PTF) and determining the effect of antithrombotic therapy on these markers; 83 patients with AF were studied. Increased levels of plasma TAT and PTF were more frequently observed in patients with AF and associated mitral stenosis than in patients with AF alone. In cases of AF without mit

The quantitative association of plasma endotoxin, antithrombin, protein C, extrinsic pathway inhibitor and fibrinopeptide A in systemic meningococcal disease.

We have evaluated the quantitative relationship between lipopolysaccharide (LPS, endotoxin), fibrinopeptide A (FPA), antithrombin (AT), protein C (PC) and extrinsic pathway inhibitor (EPI) in plasma from 39 consecutively admitted patients with systemic meningococcal disease (SMD). The most severely ill patients with fulminant meningococcal septicemia (n = 13, 6 dead) had significantly (p less than 0.01) higher plasma levels of LPS and FPA and lower levels of PC and AT on admission as compared with the less severe clinical presentations (n = 26, 1 dead). The levels of EPI on admission were sign

Severe acquired antithrombin III deficiency in relation to hepatic and renal insufficiency and intrauterine fetal death in late pregnancy.

Acute fatty liver (AFLP) is a rare complication of late pregnancy. The maternal and fetal mortality has earlier been reported to be about 75%, but during the last decade a reduced mortality to about 30 and 50%, respectively, has been reported in the literature. Disseminated intravascular coagulation (DIC) has been suggested as a contributing cause to the high mortality. The treatment of DIC has long been under debate, and recently the administration of antithrombin III (AT) concentrate in addition to other supportive treatment has been reported successful. This paper presents the survival of 1

A novel risk prediction model for post-treatment ischemic stroke recurrence using plasma antithrombin III and thromboelastography.

BACKGROUND: The primary aim of this investigation was to build and test a novel risk prediction model incorporating plasma antithrombin III (ATIII) activity and thrombelastography (TEG)-derived parameters. This approach seeks to enhance the ability to stratify the risk of stroke recurrence among ischemic stroke (IS) patients receiving dual antiplatelet therapy (DAPT). METHODS: In this prospective cohort study, 200 consecutive patients diagnosed with non-cardiogenic IS were recruited during a one-year period (May 2024 to May 2025). All participants had their ATIII activity, TEG parameters, and

SERPINC1 p.M313T Variant Induces Aberrant O-Glycosylation and Leads to Conformational Instability-Related Transient Antithrombin Deficiency.

Congenital antithrombin (AT) deficiency, primarily caused by SERPINC1 variants, is a major risk factor for venous thromboembolism (VTE). We previously reported the SERPINC1 p.M313T variant in three VTE probands with normal AT activity and antigen levels. This study aims to elucidate its pathogenetic mechanism.Thrombin generation test (TGT), thermal stability, native-urea PAGE, in vitro protein expression, and enzymatic assays were performed. Glycosylation analysis was conducted using glycosidase treatment, and structural analysis was performed through molecular dynamics simulation.AT in proban

Rare inherited antithrombin deficiency presenting as spontaneous cerebral venous thrombosis with concomitant parenchymal hemorrhage: a case report.

Inherited antithrombin deficiency (IATD) is a rare thrombophilia that predisposes individuals to venous thromboembolism (VTE), though its presentation as spontaneous cerebral venous thrombosis (CVT) is exceedingly uncommon. We report the case of a 27-year-old woman with IATD who developed CVT complicated by bilateral frontal lobe hemorrhage. The initial manifestation of intracranial hemorrhage posed a significant diagnostic and therapeutic dilemma, as it often contraindicates anticoagulation. This case highlights that in the context of CVT, concomitant hemorrhage should not be considered an ab

Structural characterization of novel polysaccharides from Sargassum fusiforme with dual anticoagulant activity targeting thrombin and antithrombin III.

The polysaccharide from Sargassum fusiforme, an edible brown algae, exhibits anticoagulant activity and benefits cardiovascular health, yet structural studies are limited. Herein, crude polysaccharide from S. fusiforme (CPSF) was extracted and isolated, which demonstrated anticoagulant activity when orally administered. A polysaccharide fractionation map of S. fusiforme was established. Bioassay-guided fractionation yielded homogeneous and bioactive polysaccharides from CPSF. Notably, structurally novel SF4B (6.37 × 106 Da) displayed dual target anticoagulant activity by directly inhibiting th

The unexpected clot: unilateral brachial artery occlusion unmasking antithrombin III deficiency; case report and therapeutic considerations.

BACKGROUND: Antithrombin III (AT III) deficiency is a rare autosomal dominant thrombophilia that typically predisposes carriers to venous thromboembolism. Arterial thrombosis is uncommon but may occur in severe quantitative or qualitative defects, particularly when compounded by exogenous risk factors such as estrogen exposure. CASE PRESENTATION: We report a 47-year-old woman on combined oral contraceptives who presented with acute left upper-extremity ischemia due to complete brachial artery occlusion. Emergency embolectomy was performed, but early re-thrombosis occurred. Laboratory studies s

Antithrombin III at admission as a new predictive factors for mortality in ECMO for cardiogenic shock caused by acute myocardial infarction.

Extracorporeal membrane oxygenation (ECMO) is a technique increasingly used in the practice of intensive therapy for extracorporeal gas exchange and/or circulatory support in patients with acute respiratory and/or cardiac failure, when conventional treatment modalities are ineffective. We aimed to investigate the association of antithrombin III with clinical outcomes in patients with cardiogenic shock caused by acute myocardial infarction undergoing ECMO. We retrospectively studied patients with acute myocardial infarction complicated by cardiogenic shock undergoing ECMO in four hospitals in C

Efficacy and safety of antithrombin or recombinant human thrombomodulin in the treatment of disseminated intravascular coagulation: A systematic review and meta-analysis.

OBJECTIVE: Multiple organ damage is a hallmark of the highly lethal condition known as disseminated intravascular coagulation (DIC). The efficacy and safety of recombinant human soluble thrombomodulin (rhTM) and antithrombin (AT) in DIC is still debatable. Therefore, we used a fixed-effects model to conduct a comprehensive evaluation and meta-analysis to examine the safety and efficacy of AT or rhTM administration for treating DIC. METHODS: Up until September 2024, the databases of the Cochrane Library, Embase, Web of Science, PubMed, and CNKI were searched for pertinent papers that satisfied

How we treat severe inherited antithrombin deficiency: lessons from cases homozygous for the Budapest 3 variant.

BACKGROUND: Antithrombin deficiency represents one of the most severe inherited thrombophilias. Albeit a rare disorder, available knowledge suggests that antithrombin deficiency is underestimated due to the limitations of current diagnostic algorithms. The high clinical variability of this patient population may be another cause of underdiagnosis. Heterozygous type I (quantitative) variants are normally associated with a severe thrombophilic phenotype, while heterozygous type II (qualitative) variants are heterogeneous, including heparin-binding site defects, which are mild/moderate and the mo

Thrombin Generation Decrease After LMWH Administration in an Antithrombin-Deficient Pregnant Woman With a Homozygous HBS II Mutation.

The cases of antithrombin (AT)-deficient pregnant women with a homozygous HBS II mutation are relatively rare and are accompanied by an increased thrombophilic risk, which is manifested by increased thrombin generation (TG). It is very difficult to ensure their prophylactic treatment during pregnancy. We aimed to determine the utility of the thrombin generation assay (TGA) and anti-factor Xa (anti-FXa) test to monitor the effects of a prophylactic dose of low-molecular-weight heparin (LMWH) in a 28-year-old woman with homozygous AT deficiency caused by mutation c.391C > T#, (p.Leu131Phe†) in t

Prothrombin Complex Concentrate-induced Disseminated Intravascular Coagulation Can Be Prevented by Coadministering Antithrombin in a Porcine Trauma Model.

BACKGROUND: The risk of thromboembolic complications with prothrombin complex concentrates (PCCs) appears low when used for reversal of vitamin K antagonists but might be different in other indications (e.g., trauma). A difference in risk could arise from the plasma ratio of pro- versus anticoagulant proteins. This study used a porcine trauma model to investigate combined treatment with PCC and antithrombin. The hypothesis was that antithrombin can modulate prothrombotic effects and prevent adverse events of PCC. METHODS: Nine treatment groups (n = 7 per group) were included: control (placebo
