Aniseikonia and stereoacuity were measured in patients with unilateral aphakia, most of whom were postoperative senile cataract cases. The New Aniseikonia test was used to evaluate aniseikonia and the Titmus Stereotest to measure stereoacuity. Ninety cases were studied, 57 (63.3%) of which had intraocular lens (IOL) implants; 27 (30%) had extended-wear soft contact lenses; and six (6.7%) had spectacle lenses. The records of all adult patients at Louisiana State University Eye Center, New Orleans, who underwent epikeratophakia for aphakia with commercially prepared tissue since February 1984 were reviewed. The epikeratophakia lenticule was maintained in 37 (92.5%) of 40 patients; complications required the removal of three tissue lenses. The average refractive error three months after surgery was +0.67 +/- 1.97 diopters; 25 (90%) of 28 patients were within 3 D of emmetropia. A light and electron microscope studies of the lenses in ak/ak and +/+ mice at different stages of embryogenesis have shown that the earliest effect of the mutant gene ak at the subcellular level was the destructive change of the cell organelles in the early invaginating placode. As a result, the formation of the basal membrane of the lens placode cells and, later on, the lens capsule development were affected. The destructive changes of the cells were accompanied by a weakening of intercellular contacts and the transport of the degenerating cells into the lens vesicle cavity.

This paper reports the study over a 2-year period of forty patients with uniocular aphakia fitted with spherical extended wear hydrophilic lenses: Sauflon 85 per cent and Duragel 72 per cent. The patients' ages ranged from 17 to 87 years (mean 52). They were unable to handle a daily wear soft lens or tolerate a hard lens. The success rate was 85 per cent.

This system of soft lens fitting in conjenction with the backward sloping corneal incision proved satisfactory in 84% of cases and provided good fill-in vision during the postoperative period--that is, until a decision about the type of long-term optical correction was made in each case.

In considering the theoretical ideal for an artificial intraocular lens implant to be used in the correction of aphakia it has always been taken for granted that Nature's bioconvex lens is best. But there is some evidence that other configurations may be an improvement. We also have to decide how large it should be and whether it should be placed in front of the iris or behind it. Moreover we have to consider the management of eyes which are already aphakic when first seen and cataractous eyes requiring lens extraction before the pseudophakos can be inserted.

Seventeen cases of unilateral aphakia secondary to acquired cataracts and dislocated lenses were presented. Final visual acuity was 20/70 or better in 12 of our 17 patients. Binocular single vision has been maintained in four cases. Ten patients had a posttherapy tropia. Early surgery and treatment particularly in the young child is stressed in order to maintain normal binocular single vision. Where once traumatized eyes were felt to be "sick" eyes and better left alone, current surgical and therapeutic methods make restoration of vision and binocularity a reasonable goal.
