From Pansinusitis to Cerebritis Due to Eikenella corrodens.

Meningitis is a rare but possible complication of sinusitis. We present a case of a 21-year-old woman with a history of fever, headache and nasal obstruction who presented at the emergency department with psychomotor agitation. Orotracheal intubation and invasive mechanical ventilation were given to protect airway. Blood analysis showed leukocytosis and elevated C-reactive protein. Cerebral and maxillofacial computed tomography (CT) demonstrated pansinusitis with gas foci more prominent in the left frontal sinus with an area of bone rarefaction on the posterior wall with possible communicati

[Language disorders in acute cerebellitis: beyond dysarthria].

INTRODUCTION: Acute cerebellitis is one of the main causes of cerebellar syndrome in infancy. Among the wide range of manifestations, headache and ataxia being the most predominant, we can find other less frequent, although nonetheless interesting, ones, such as language disorders, which go beyond the well-known cerebellar dysarthria. The different combinations in which the symptoms can appear, especially when not accompanied by ataxia, make the condition a real challenge for the clinician. CASE REPORTS: Two patients, aged 2 and 4 years, with clinical features, lab tests and neuroimaging resu

Treatment for acquired apraxia of speech: examination of treatment intensity and practice schedule.

PURPOSE: The authors designed this investigation to extend the development of a treatment for acquired apraxia of speech (AOS)--sound production treatment (SPT)--by examining the effects of 2 treatment intensities and 2 schedules of practice. METHOD: The authors used a multiple baseline design across participants and behaviors with 4 speakers with chronic AOS and aphasia. Accuracy of production of trained and untrained words in phrases served as the dependent measure. Participants received 4 permutations of SPT (i.e., intensive-blocked, intensive-random, traditional-blocked, and traditional-r

Acute bilateral cerebral infarction in the presence of neuromyelitis optica spectrum disorder: A case report.

RATIONALE: Neuromyelitis optica spectrum disorders (NMOSDs) are inflammatory demyelinating disorders of the central nervous system; they are characterized by severe optic neuritis and transverse myelitis. Intravenous methylprednisolone pulse (IVMP) therapy is an effective treatment that is administered to patients in the acute phase of NMOSD; this therapy has achieved remarkable results in clinical practice. However, there are no reports on NMOSD patients who have experienced an acute bilateral cerebral infarction while undergoing IVMP treatment. PATIENT CONCERNS: We report on a 62-yr-old wom

[A case of progressive multifocal leukoencephalopathy presenting white matter MRI lesions extending over the cerebral cortex and a marked decrease in cerebral blood flow on SPECT, and associated with HTLV-I infection].

We report a 47-year-old woman with progressive multifocal leukoencephalopathy (PML). She was a carrier of HTLV-I virus, and developed subacute right hemiparesis and marked motor aphasia. She had a malignant lymphoma in the left neck and basal cell carcinoma in the right inguinal region. Three months after the onset, she became unable to walk because of the right leg weakness or to speak because of motor aphasia. Magnetic resonance imaging (MRI) revealed multifocal T2-high lesions in the white matter of the left frontal lobe, and a brain biopsy revealed demyelinating pathology. A biopsy of the

Improved language in chronic aphasia after self-delivered iPad speech therapy.

Self-delivered speech therapy provides an opportunity for individualised dosage as a complement to the speech-therapy regime in the long-term rehabilitation pathway. Few apps for speech therapy have been subject to clinical trials, especially on a self-delivered platform. In a crossover design study, the Comprehensive Aphasia Test (CAT) and Cookie Theft Picture Description (CTPD) were used to measure untrained improvement in a group of chronic expressive aphasic patients after using a speech therapy app. A pilot study (n = 3) and crossover design (n = 7) comparing the therapy app with a non-la

Primary progressive non-fluent aphasia: a case study.

A case study of a 65 year old man is described with an eight-year history of progressive primary non-fluent aphasia accompanied by agrammatism, phonemic paraphasias and mild spelling dysgraphia. His naming ability, however, has remained at an exceptionally high level and there has been no evidence of impairment of word or sentence comprehension. Non-verbal skills and memory functions have also been preserved within the range of his very high premorbid level of abilities. Single photon emission computed tomography was consistent with bifrontal hypoperfusion. We argue that the selective language

Phonology and neuropsychology of the English past tense.

The double dissociation between the regular and irregular past tense in English has been explained in terms of dual and single mechanism accounts. In previous research we have argued that problems with the regular past tense in patients with left inferior frontal damage arise from morpho-phonological parsing difficulties [Trends in Cognitive Science 2 (1998) 428]. This claim has recently been challenged by a single mechanism connectionist account which argues that a general phonological processing deficit causes the poor performance on the regular past tense, with morphological factors playing

Progressive nonfluent aphasia and subsequent aphasic dementia associated with atypical progressive supranuclear palsy pathology.

We describe a right-handed man who developed progressive nonfluent aphasia and apraxia of speech beginning at age 71. By age 74 he had behavioral changes, aphasic dementia, as well as mild parkinsonism; extraocular movements were normal except for mild limitation of upgaze. Serial neuropsychometric testing and single photon emission computed tomography (SPECT) scans showed progressive changes reflecting left>right cerebral hemisphere dysfunction. Neuropathologic examination revealed findings characteristic of progressive supranuclear palsy (PSP) except that the cortical pathology was more wide

Language organisation in left perinatal stroke.

Right-hemispheric organisation of language has been observed following early left-sided brain lesions. The role of the site of damage is still controversial, as other aspects influence the pattern of speech organisation including timing of the lesion and the presence of epilepsy. We studied a group of 10 term-born children homogeneous for timing/type of lesion and clinical picture. All subjects had left perinatal arterial stroke, right hemiplegia, normal cognitive functions and no or easily controlled epileptic seizures. In half the patients, the lesion clearly involved Broca's area, in the ot

[Accidental fall and word finding problems in heart valve disease].

Abscesses of the spleen are rare and, if untreated, lead to death. The usual mode of development is by hematogenous spread of infection. Treatment options are splenectomy, percutaneous drainage and in selected cases an antibiotic therapy. We report a case of a splenic abscess due to Streptococcus bovis treated with antibiotics only.

Acute methotrexate neurotoxicity: findings on diffusion-weighted imaging and correlation with clinical outcome.

BACKGROUND AND PURPOSE: Acute lymphocytic leukemia (ALL) is a common malignancy of childhood treated with methotrexate (MTX), which is associated with acute neurotoxicity. We evaluated diffusion-weighted (DW) and conventional MR images in children with ALL and acute MTX-induced neurotoxicity, with clinical correlation. METHODS: Five patients aged 12-15 years underwent fluid-attenuated inversion recovery (FLAIR), T2-weighted fast spin-echo and gradient-echo, T1-weighted gadolinium-enhanced spin-echo, and DW imaging within 24 hours of symptom onset. Records were reviewed for the temporal relati

Neurological Manifestations of Aortic Dissection: A Scoping Review.

Aortic dissection (AD) is the most prevalent aortic pathology, with an incidence of 2.6-3.5 cases per 100,000 inhabitants per year with mortality rates as high as 90% at 3 months without proper management. Despite the presence of typical symptoms, it has been reported that up to 38% of cases go unnoticed in the initial evaluation, either due to additional symptomatology or the absence of classic symptoms. In 28% of cases the diagnosis is made post-mortem, highlighting the severity and importance of timely diagnosis. The main goals of this study were to identify the primary neurological manifes

Primary angiitis of the central nervous system mimicking tumor-like lesion--case report.

A 60-year-old man presented with a rare case of primary angiitis of the central nervous system mimicking a tumor-like lesion and manifesting as slight disorientation, left hemiparesis, and motor aphasia. Computed tomography showed multiple low density lesions in the left frontal lobe, brain stem, and right parietal lobe. Magnetic resonance images revealed a slightly enhanced mass lesion in the right parietal lobe with surrounding brain edema. Serum, cerebrospinal fluid, and other image examinations did not show any abnormal findings, so surgical removal of the right parietal mass was performed

Verbal short-term memory and cerebellum: evidence from a patient with congenital cerebellar vermis hypoplasia.

The impairment of phonological short-term memory has been reported in adults with cerebellar lesions. At the same time, a role of the cerebellum in speech production has been hypothesized. Cerebellar malformations have been related to developmental problems and language acquisition in children. We describe a 5-year-old male child with cerebellar vermis hypoplasia who presented a severe linguistic deficit. On language testing, verbal production was almost absent, while comprehension was partially spared. Digit span was markedly reduced. An extensive examination of phonological short-term memory

[Reversible cortical atrophy secondary to anti-NMDA receptor antibody encephalitis].

INTRODUCTION: Anti-N-methyl-D-aspartate (NMDA) receptor antibody encephalitis was initially described as a paraneoplastic syndrome associated to ovarian teratomas. Yet, an increasing number of reports are being published involving cases of young women and children with signs and symptoms of an autoimmune encephalopathy, in 40-50% of the cases secondary to a viral infection. Clinically, it is characterised by a progressive picture of psychiatric manifestations, convulsive seizures, dyskinesias and dysautonomias. One neuroimaging finding that has received little attention is reversible cortical

Spontaneous social behaviors discriminate behavioral dementias from psychiatric disorders and other dementias.

OBJECTIVE: Changes in social behavior are often the first symptoms of neurodegenerative disease. Patients with frontotemporal lobar degeneration (FTLD) often go undiagnosed, or are misclassified as psychiatric patients, because in the absence of cognitive deficits, nonexperts fail to recognize these social changes as dementia symptoms. The object of this study was to improve screening for behavioral dementias in primary care and mental health settings by quantifying spontaneous social behaviors specific to FTLD. METHOD: In a university hospital dementia clinic, examiners blind to subject diag

The case is presented of a 51 year-old woman, proposed for endoscopic endonasal transsphenoidal resection of a hypophyseal macroadenoma diagnosed in the context of a stroke suffered 10 weeks before the date of the surgery. During this time, she had been treated with antiplatelet drugs, which were withdrawn 5 days before the surgery. The surgical procedure was performed without any incidents. On the second day after the surgery, the patient had an ischaemic infarction of the left cerebellar hemisphere, with signs of hydrocephaly and a posterior haemorrhagic transformation, with brain death 5 da
