Worldwide, hypoxic-ischemic encephalopathy (HIE) is a major cause of neonatal mortality and morbidity. To better understand the mechanisms contributing to brain injury and improve outcomes in neonates with HIE, better preclinical animal models that mimic the clinical situation following birth asphyxia in term newborns are needed. In an effort to achieve this goal, we modified our nonhuman primate model of HIE induced by in utero umbilical cord occlusion (UCO) to include postnatal hypoxic episodes, in order to simulate apneic events in human neonates with HIE. We describe a cohort of 4 near-ter

The use of 2-D strain speckle tracking echocardiography in B-mode images from tissue Doppler (TD) recordings was compared with its use in regular gray-scale (GS) recordings in healthy and asphyxiated neonates at days 1-3. Single-cycle and three-cycle longitudinal deformation indices were assessed. Median B-mode frame rates were 38/s (TD) and 77/s (GS). Systolic indices were similar for TD and GS recordings. Diastolic indices were lower for TD recordings. Single-cycle and three-cycle indices were equal. TD and GS recordings were similar in reproducibility, except for single-cycle inter-observer

Birth asphyxia, which causes hypoxic-ischemic encephalopathy (HIE), accounts for 0.66 million deaths worldwide each year, about a quarter of the world's 2.9 million neonatal deaths. Animal models of HIE have contributed to the understanding of the pathophysiology in HIE, and have highlighted the dynamic process that occur in brain injury due to perinatal asphyxia. Thus, animal studies have suggested a time-window for post-insult treatment strategies. Hypothermia has been tested as a treatment for HIE in pdiglet models and subsequently proven effective in clinical trials. Variations of the mode

BACKGROUND: Neonates after perinatal asphyxia are at increased risk of thrombocytopenia. The correlation between perinatal asphyxia and the risk and severity of early-onset thrombocytopenia is not well known.
OBJECTIVE: To estimate the incidence, severity and risk factors for early-onset thrombocytopenia in neonates after perinatal asphyxia.
METHODS: We included all newborns (gestational age ≥ 36 weeks) admitted to our neonatal nursery due to perinatal asphyxia in this retrospective study. We collected platelet counts that were obtained within the first 48 h of life to estimate the incidence

OBJECTIVE: To explore the capacity of somatosensory evoked potentials (SEP) to assess maturation processes in the development of the nervous system, and the characteristics of SEP in healthy full-term infants and full-term newborns with perinatal asphyxia and their follow up until the age of 14 months.
MATERIALS AND METHODS: SEP were studied in 21 healthy full-term infants and 38 full-term newborns with perinatal asphyxia. The children with asphyxia were studied longitudinally until they were 14 months old. To assess the SEP we measured the latency of the P15, N20 and P25 components, the ampl

Subcutaneous fat necrosis is an uncommon disease that may be complicated with potentially fatal hypercalcemia or with nephrocalcinosis. We report on the case of a patient with a history of significant perinatal asphyxia, hospitalized for a urinary tract infection. Lesions of subcutaneous fat necrosis were noted, with asymptomatic hypercalcemia at 3.9mmol/L. A renal ultrasound was performed and showed echogenic medullary pyramids bilaterally, consistent with nephrocalcinosis and left nephrolithiasis. The treatment of hypercalcemia included hyperhydration, a diuretic and corticosteroids. Progres

With an annual birth rate of 12‰, or 16 millions, of all population (1.34 billions), and an implementation of universal healthcare policy for all rural residents in recent years, China is undergoing a dramatic and profound transition in perinatal and neonatal healthcare as a part of the global campaign for reduction in mortality of children under 5 years old. This review describes recent development in neonatal-perinatal medicine, with special emphasis on general neonatal-perinatal care, respiratory and intensive care, neurological and infectious diseases, for a comprehensive view of the trend

BACKGROUND: Birth asphyxia is one of the commonest causes of neonatal morbidity and mortality in developing countries. Together with prematurity and neonatal sepsis, they account for over 80% of neonatal deaths.
AIM: To determine the incidence and mortality rate of birth asphyxia in Warri Niger Delta of Nigeria.
MATERIALS AND METHOD: Recovery of case notes of all the newborn babies seen from January 2000 to December 2007 at Central Hospital Warri and GN children's Clinic, Warri, was undertaken. They were analyzed and those with birth asphyxia were further analyzed, noting the causes, severit

INTRODUCTION: In the last few years the use of cardiac troponin I and T, as diagnostic and prognostic factors of ischemic myocardial injury both in adult and neonatal medicine has been of great interest.
OBJECTIVE: The objective of our research was to investigate the significance of cardiac troponin I (cTnl) as an early indicator of the presence and severity of hypoxic-ischemic encephalopathy (HIE) in newborns.
METHODS: We analyzed 55 term newborns with HIE diagnosed based on clinical findings and ultrasonographic examination of the central nervous system. Serum concentration of cTnl-ultra w

Hypothermia is used for its neuroprotective effect in perinatal asphyxia. Mechanical ventilation is often used as a supportive therapy for severe asphyxiated infants who can present various degrees of respiratory failure. Animal studies demonstrated a protective effect of cooling on the lungs due to reduced ventilatory requirements. Even if actual knowledge on the effects of hypothermia and rewarming on respiratory parameters during mechanical ventilation is limited, nevertheless human studies seem to demonstrate that hypothermia is safe and does not cause significant changes in the level of r

Each year, 814,000 neonatal deaths and 1.02 million stillbirths result from intrapartum-related causes, such as intrauterine hypoxia. Almost all of these deaths are in low- and middle-income countries, where women frequently lack access to quality perinatal care and may delay care-seeking. Approximately 60 million annual births occur outside of health facilities, and most of these childbirths are without a skilled birth attendant. Conditions that increase the risk of intrauterine hypoxia--such as pre-eclampsia/eclampsia, obstructed labor, and low birth weight--are often more prevalent in low r

Celebrated for disproving the traditional view that lack of oxygen at birth (perinatal asphyxia) contributes significantly to cerebral palsy, a 1986 New England Journal of Medicine article by Karin Nelson and Jonas Ellenberg engineered a new consensus in the medical community: that lack of oxygen at birth rarely causes cerebral palsy. We demonstrate that the article's central argument relies on straightforwardly fallacious statistical reasoning, and we discuss significant implications--e.g. how carefully fetuses are monitored during labor and delivery, expert testimony in malpractice cases, an

OBJECTIVE: To assess usefulness of high-dose early phenobarbital therapy for prevention of hypoxic-ischemic encephalopathy (HIE) secondary to perinatal asphyxia (PNA).
MATERIAL AND METHODS: By means of a randomized clinical trial, asphyxiated full-term or post-term newborn infants were divided in two groups: Group A was the experimental group, while group B was the control group. Infants in group A received phenobarbital, 40 mg/kg, during the first 60 min after birth. Infants on group B received phenobarbital at conventional doses, only if there was clinical evidence of seizures; otherwise, t

Perinatal asphyxia is a major topic in neonatology. It has a high impact on neonatal mortality and morbidity and neurological and intellectual development of the infant. Hypoxia triggers compensatory mechanisms in the fetus and the newborn aimed to sustain adequate oxygen transport to the vital organs. Long periods of hypoxia and poor perfusion cause failure of mentioned compensative mechanisms and lead to irreversible damage in tissues. No clear-cut physiologic parameters exist which allow for an early identification of neonatal infants who are either at risk to develop brain damage or posthy

In this prospective study proton magnetic resonance spectroscopy (1H MRS) was used to test the hypothesis that lactate can be detected later than 1 mo after birth in the brains of infants who display severe neurodevelopmental impairment 1 y after transient perinatal hypoxia-ischemia. Data were obtained from three groups of infants: 1) eight infants suffering birth asphyxia followed by perinatal encephalopathy and abnormal neurodevelopmental outcome at 1 y of age (defined as major neurologic impairment, Griffiths quotient <85%, and low optimality score); 2) 10 infants with signs of perinatal hy
