Maxillofacial fractures, especially those of the mandible, pose a significant risk in microgravity environments because astronauts experience progressive bone loss during long-duration flights because of skeletal unloading. In this study, we explored the biomechanical response of the mandibular angle to high-impact trauma caused by gravity and microgravity. A human mandibular model was subjected to a force of 2000 N at an angle of 45°, which was directed posterosuperiorly at the right-angle region with simulations comparing healthy and osteoporotic bone (bone loses its density in long flights …

Interesting novel and somewhat perplexing physiologic and pathologic neuro-ocular findings have been documented in astronauts during and after long duration space flight (LDSF). These findings collectively have been termed the "space flight-associated neuro-ocular syndrome" (SANS). The National Aeronautics and Space Administration (NASA) in the United States has meticulously and prospectively documented the clinical, ultrasound, optical coherence tomography imaging, and radiographic findings of SANS including unilateral and bilateral optic disc edema, globe flattening, choroidal and retinal fo…

Cardiovascular changes occur during spaceflight. Because of the loss of hydrostatic pressure there is, during weightlessness, a fluid shift of extra- and intra-vascular volumes to the upper part of the body. This provokes objective and subjective symptoms, especially in the first days of spaceflight. The fluid shift is perceived by baroreceptors as a relative central hypervolaemia and induces neuro-hormonal mechanisms which provoke a real hypovolaemia. Cardiovascular deconditioning exists during spaceflight with the following symptoms when astronauts return to earth: decreased orthostatic tole…

Several consistent medical problems have been encountered by astronauts during space flights. These include vestibular dysfunction, weight loss, increase in height, upward fluid shift, anemia, cardiovascular deconditioning, muscle atrophy, and bone loss. Almost all of these alterations can be attributed to the absence of gravitational force. Most are adaptive in nature and therefore reversible, but readaptation after returning to earth may cause further problems (e.g., in the case of vestibular dysfunction). The most recalcitrant and disturbing of all these problems is the relentless bone loss…

Spaceflight associated neuro-ocular syndrome (SANS) alters the vision of astronauts during long-duration spaceflights. Previously, the current authors have discussed the similarities and differences between SANS and idiopathic intracranial hypertension to try to elucidate a possible pathophysiology. Recently, a theory has been advanced that SANS may occur secondary to failure of the glymphatic system caused by venous dilatation within the brain and optic nerves. There is recent evidence to suggest glymphatic obstruction occurs in childhood hydrocephalus, multiple sclerosis and syringomyelia du…

The new era of space exploration is increasing the astronaut's number and diversity in low orbit and beyond. The influx of such a diverse crew population will also increase the need for medical technologies to ensure safe and productive missions. Such a need represents a unique opportunity to innovate and develop diagnostics and treatment tools to meet future needs. Historically, terrestrial regulatory oversight of biomedical design processes was considered separate from spaceflight regulatory processes because it did not address spaceflight constraints. These constraints challenge the creativ

Recordings of horizontal nystagmus were obtained on 16 male subjects exposed to repeated patterns of horizontal angular acceleration, consta … free-fall. This fact likely accounts for the disorientation and dizziness sometimes experienced by astronauts when moving their heads in the early phases of orbital flight and again after splashdown. The implications of the present findings, both for the etiology and for the treatment of space motion sickness, are discussed.

OBJECTIVE: To investigate the mechanisms of ocular injuries in astronauts due to gravity deficit by examining changes in retinal microcirculation and visual electrophysiology in macaques subjected to simulated weightlessness.
METHODS: The head-down recumbency of macaques was used to simulate the movement of blood to the side of the head that occurs without microgravity. Head-down recumbency was performed with the head tilted downwards at a recommended angle of 10°. The macaques in the control group were similarly tethered to the rope but could be held in a normal position. The whole experimen…

BACKGROUND: Carbon dioxide (CO₂) toxicity could be catastrophic for astronauts. Suppressing metabolism by lowering body temperature decreases CO₂ production and may facilitate rescue in the event of a crippled ship. Lowering body temperature requires shivering suppression. We evaluated dexmedetomidine to facilitate cooling of healthy individuals.METHODS: Following consent, we administered a 1 mcg · kg-1 bolus of dexmedetomidine followed by continuous infusion (0.5-1.4 mcg · kg-1 · h-1) for 3 h of cooling. We cooled subjects using a bolus of 30 cc · kg-1 of 4°C saline followed by surface coolin…

We carried out an extensive series of measurements on astronauts in the orbiting laboratory known as SpaceLab in the 1990s. Many aspects of pulmonary function are affected by gravity, so it was not surprising that many changes were found. However, overall gas exchange remained efficient. Some of the findings such …

Astronauts coming back from long-term space missions present with different health problems potentially affecting mission performance, involving all functional systems and organs and closely resembling those found in the elderly. This review points out the most recent advances in the literature in areas of expertise in which specific research groups were particularly creative, and as they relate to aging and to possible benefits on Earth for disabled people. The update of new findings and approaches in space research refers especially to neuro-immuno-endocrine-metabolic interactions, optic ner…

The space adaptation syndrome is one of the more vexing problems confronted by our nation's astronauts during their journeys. This syndrome may be a variant of motion sickness, although this possibility has been questioned. Physostigmine, a centrally active cholinesterase inhibitor which increases brain acetylcholine, was found to cause a motion sickness-like syndrome--in psychiatric patients and normals--including nausea, emesis, malaise, dysphoria, increases in serum ACTH, beta-endorphin, cortisol, and prolactin, Neostigmine, a non-centrally acting cholinesterase inhibitor, and saline placeb…

Previous studies have revealed that muscle atrophy emerges as a significant challenge faced by astronauts during prolonged missions in space. A loss in muscle mass results in a weakening of skeletal muscle strength and function, which will not only contribute to a decline in overall physical performance but also elevate the risk of various age-related diseases. Skeletal muscle atrophy in the microgravity environment is thought to be associated with changes in energy metabolism, protein metabolism, calcium ion homeostasis, myostatin levels, and apoptosis. Modulating some pathways could be a pro…

Long-duration spaceflight exposes astronauts to various stressors that can alter human physiology, potentially causing immediate and long-term health effects. These stressors can damage biomolecules, cells, tissues, and organs, leading to adverse outcomes. Developing adverse outcome pathways (AOPs) relevant to radiation exposure can guide research priorities and inform risk assessments of future space exploration activities. Through expert consultation, we developed an AOP network linking 18 key events (KEs) to four non-cancer outcomes: learning and memory impairment, bone loss, abnormal vascu…

BACKGROUND: Dust exposure is a well-known occupational hazard for terrestrial workers and astronauts alike and will continue to be a concern as humankind pursues exploration and habitation of objects beyond Earth. Humankind's limited exploration experience with the Apollo Program indicates that exposure to dust will be unavoidable. Therefore, NASA must assess potential toxicity and recommend appropriate mitigation measures to ensure that explorers are adequately protected. Visual acuity is critical during exploration activities and operations aboard spacecraft. Therefore, the present research …

Main osteoporosis definitions and some results of bone tissue research in Russian astronauts, patients, and healthy subjects, using modern osteodensitometry, are presented. Bone mineral density (BMD) was regularly decreased at lower segments of skeleton. In the skull bone and some other sites of upper part of skeleton, a tendency was revealed for an increase of the bone mineral content (BMC). The mean value of bone loss was within the normal range and not correlated with duration of space flight; it revealed a high individual variability and in some cases was clinically qualified as local oste…

When manipulating objects, our brain continuously adjusts grip force (GF) to the variations in load force (LF) generated by our own movements. GF-LF coordination provides a window into the predictive capabilities of the brain. To better understand how gravity influences these predictions, we analyzed grip dynamics and movement kinematics in astronauts (two females, nine males) manipulating objects on the ground (in 1G) and during spaceflight in a stable weightless environment (in 0G). We found that the imprint of gravity remains visible in the way we manipulate objects even after months of liv…
