Electrophysiological characteristics of Chagas disease.

OBJECTIVE: Chagas disease has become a global problem due to changing migration patterns. An electrophysiological study is generally indicated for assessing sinus node function, conduction through the atrioventricular node and His-Purkinje system, in addition to evaluating the mechanisms of arrhythmia. The aim of this study was to describe the characteristics of electrophysiological study findings in patients with Chagas disease. METHODS: A retrospective descriptive study of 115 consecutive patients with Chagas disease undergoing an electrophysiological study over the last three years in a te…

Comparison of cardiovascular effects of a new calcium channel blocker AH-1058 with those of verapamil assessed in blood-perfused canine heart preparations.

The cardiovascular effects of AH-1058, a novel calcium channel blocker, were examined in comparison with those of verapamil using canine isolated, blood-perfused papillary muscle, atrioventricular node, and sinoatrial node preparations. Intravenous administration of AH-1058 (20, 50, and 100 microg/kg) or verapamil (20, 50, and 100 microg/kg) to the blood-donor dog induced negative inotropic, dromotropic, and chronotropic effects and a coronary vasodilator action in cross-circulated isolated heart preparations, simultaneously inducing the same cardiac effects in the blood-donor dog. The order o…

The use of radiofrequency energy in pediatric cardiology.

The use of radiofrequency energy for the treatment of supraventricular tachycardia in pediatric patients has gained widespread acceptance, especially for tachyarrhythmias associated with palpitations, dizziness, presyncope or syncope, cardiomyopathy, and cardiac arrest. Ablation of the substrate supporting atrioventricular reentry, atrioventricular node reentry, and automatic atrial tachycardia yields a 90%-98% success rate with low incidence (< 1%) of complications and adverse side-effects. Ablation of intra-atrial reentry, including atrial flutter and fibrillation, appears to be promising an…

Cooling for Hypoxic Ischemic Encephalopathy: From Evidence to Practice.

The cardiac conduction system (CCS) develops asymmetrically along the body axes. In heterotaxy syndrome - resulting from aberrant left-right axis formation - atrial and atrioventricular conduction defects can cause life-threatening arrhythmias. However, the developmental mechanisms regulating the atrioventricular conduction system (AVCS) disposition and integrity remain unclear. To investigate the etiology of AVCS malformations in laterality defects, we analyzed CCS development and function in mouse mutants for Cryptic and Lefty1, which are key regulators of Pitx2 in the left-right axis format

Autonomic sinus node dysfunction and its treatment.

The clinical, electrocardiographic, pharmacologic, electrophysiologic and Holter monitoring findings are described in four patients with aut … blockade with electrophysiological studies exclude the intrinsic involvement of the sinoatrial and atrioventricular node. Holter monitoring is the best method for assessing the autonomic neurovegetative component of dysrhythmias. Cardiac electrophysiologic effects of moricizine hydrochloride.

Moricizine is a class I antiarrhythmic drug. In preclinical studies, it produces a concentration-dependent decrease in the maximal rate of p … patients, moricizine has minimal effects on the normal sinus node, slows conduction in the atrium, atrioventricular node, His-Purkinje system and ventricular myocardium and has little effect on the atrial and ventricular refractoriness. The intensity of moricizine action on the atrioventricular node, His-Purkinje system and JT interval are dose-related. Co-administrat …

Radiofrequency catheter ablation of a right atriofascicular (Mahaim) and two atrioventricular (Kent) accessory pathways in a single session.

This report describes the case of a 19-year-old patient with Ebstein's anomaly, who had an unusual combination of a right atriofascicular (Mahaim) and two ipsilateral right atrioventricular (Kent) accessory pathways participating in three types of antidromic and orthodromic reciprocating tachycardias in the absence of retrograde conduction over the bundle of His-atrioventricular node axis. All three pathways were ablated in a single session using temperature-guided radiofrequency current.

Verapamil in arrhythmia.

The antiarrhythmic effects of verapamil were observed before it was appreciated that it was a calcium ion-antagonist. Intravenous verapamil is highly effective in the termination of paroxysmal reciprocating atrioventricular tachycardia, whether associated with preexcitation or involving the atrioventricular node alone. It consistently slows and regularises the ventricular response in atrial fibrillation, and usually increases the degree of AV-nodal block in atrial flutter though it occasionally induces a return to sinus rhythm. Given orally it is useful for the prophylaxis of atrioventricular …

When is surgery indicated for the control of supraventricular tachycardia?

The surgical treatment of supraventricular tachycardias, if the experience with the ones caused by Kent bundles is excluded, is only now developing. Recent reports have described eight patients who had ablation, excision, or exclusion of an area of myocardium that contained an automatic focus. One instance of partial atrioventricular node interruption has been reported for correction of an atrioventricular nodal reentry tachycardia. The largest group of patients subjected to a direct operation had interruption of atrioventricular conduction. Twenty-seven patients, most of whom had enhanced atr…

A very unusual wide QRS tachycardia: VT or SVT?

INTRODUCTION: A 13 old girl presented with recurrent wide QRS tachycardia since she was 4. METHODS: An electrophysiologic study was performed. RESULTS: The electrophysiologic study showed that QRS complexes identical to those of the tachycardia could be elicited with premature atrial extrastimuli but with a shorter atrioventricular (AV) delay when the QRS was wide compared with narrow QRS complexes. The tachycardia was ablated at 9 o'clock on the tricuspid annulus demonstrating the presence of an atriofascicular fiber. CONCLUSION: We believe that this atypical behavior can be explained by A

An unusual case of non-reentrant atrioventricular nodal tachycardia.

INTRODUCTION: Although uncommonly encountered, dual atrioventricular nodal non-reentrant tachycardia (DAVNNRT) is a well-described arrhythmia that can manifest in patients with dual atrioventricular nodal pathways physiology. This arrhythmia is characterized on electrocardiogram (ECG) by a single P wave followed by two conducted QRS complexes (so-called "double fire"), and on intracardiac electrograms by a single atrial electrogram followed by two separate His deflections and ventricular electrograms. METHODS/RESULTS: We report a rare case of "triple-fire" atrioventricular non-reentrant tachy

Dual AV nodal non-reentrant tachycardia-induced cardiomyopathy.

Dual atrioventricular (AV) nodal non-reentrant tachycardia (DAVNNT) is rare phenomenon where dual AV nodal physiology allows a single P wave to conduct down both the slow and fast AV node extensions resulting in two ventricular beats. We present a case of DAVNNT manifest as incessant ventricular triplets resulting in tachycardia-inducted cardiomyopathy. We discuss the underlying electrophysiology for this rare phenomenon, how to diagnose this condition and the appropriate treatment for this syndrome.

Case report of paroxysmal atrioventricular block and ventricular arrest in a young pregnant woman: What is the mechanism?

A 30-year-old female patient with 12 weeks of gestation was admitted to our hospital due to dizziness and amaurosis fugax. Moreover, 24 h Holter monitoring showed paroxysmal atrioventricular block (P-AVB) and ventricular arrest. The heart block in the patient was likely a vagally mediated heart block based on the "vagal score." She was not given a pacemaker, and the symptoms and AV nodal conduction were improved following the delivery.

Atrioventricular nodal reentry: evidence supporting an intranodal location.

The exact site of the reentrant circuit in AV nodal reentry remains controversial. While recent ablative techniques have yielded information, the interpretation of which suggests that the atrium is required, other explanations for these interpretations are available. Prior pathophysiological studies with three-dimensional reconstruction of the node suggest that it is a highly anisotropic structure and extends through Koch's Triangle. Data from humans suggesting the atria are not necessary include the presence of AV dissociation during supraventricular tachycardia (SVT), depolarization of atria

[Electrophysiologic properties of cibenzoline in Wolff-Parkinson-White syndrome and atrioventricular nodal reentry tachycardia].

The electrophysiologic effects of the new class-1 antiarrhythmic drug cibenzoline (1.5 mg/kg within 10 min, followed by an infusion of 0.5 mg for 30 min) were investigated in six patients with atrioventricular (av) nodal reentrant tachycardia and nine patients with atrioventricular tachycardia. Sinus cycle length, sinus node recovery time, effective refractory period (ERP) of the atrium and the ventricle as well as the ERP of the av node were not significantly affected by cibenzoline. Retrograde conduction via the av node was prevented by cibenzoline in 6/15 patients, retrograde ERP was increa

[Clinidine-induced bradycardia: studies of the effect on the human sinus node (author's transl)].

Heart rate, atrioventricular conduction, sinus-node recovery time and effectiveness of carotid-sinus massage on heart rate were measured before and every five minutes after intravenous injection of 0.15 mg clonidine (Catapresan) in 11 patients (averaged age 60 years) with symptomatic bradycardia (dizzy spells, syncope, palpitations). Th results indicated the following significant (P less than 0.05) mean maximal effects: heart rate fell by 12% (from 59 to 52/min), capacity of atrioventricular conduction by 9% (from 132 to 121/min), while maximal sinus-node recovery time ("over-drive suppression

Experience sharing of a case of dual atrioventricular nodal non-reentrant tachycardia: Case report.

RATIONALE: Tachycardia is a common arrhythmia in clinical practice, and its pathogenesis is mostly related to reentry. However, there are also a few tachycardia that are not related to reentry. Actively clarifying the pathogenesis of these non-reentry related tachycardia is of great significance for its treatment. PATIENT CONCERNS: A 55-year-old female patient presented with recurrent palpitations with a fastest heart rate of 180 beats/minute 10 years ago. DIAGNOSIS: Dual atrioventricular nodal non-reentrant tachycardia (DAVNNT). INTERVENTIONS: DAVNNT can be cured by radiofrequency ablation

Extremally rare cause of AV block during typical atrial flutter ablation not related to conduction tissue injury.

BACKGROUND: 51-year-old female with extensive prior atrial surgery involving myxoma resection and patch septum repair and prior typical atrial flutter as well as peripatch reentry underwent redo radiofrequency ablation of typical atrial flutter. METHODS: After high density mapping was performed, and gap in the prior typical flutter line was ablated. RESULT: During the ablation transient atrioventricular (AV) block was noted. Subsequent remapping of the right atrium revealed that there was a narrow strip of tissue between the cavo-tricuspid isthmus (CTI) and the coronary sinus (CS) os on whic
