Effect of bariatric surgery on normal and abnormal renal function.

BACKGROUND: Obesity has been associated with hypertension, diabetes mellitus, and metabolic syndrome, risk factors for chronic kidney disease. In addition, obesity has been found to have an independent, negative effect on renal function and the progression of renal insufficiency. METHODS: The serum creatinine (CR) in 813 patients who had undergone obesity surgery from 2003 to 2009 at a large academic medical center and had been followed up for ≥24 months was retrospectively monitored. Renal function, as measured by the CR level, was assessed at baseline and at 6, 12, and ≥24 months of follow-

Bariatric emergencies: what the general surgeon should know.

BACKGROUND: bariatric surgery presented a dramatic increase due to the obesity epidemics and the laparoscopic approach. General surgeons might face acute or chronic complications of bariatric surgery, considering the increasing figures of obesity procedures performed every year in USA, as well as in Europe. AIM: to present the possible surgical emergencies after bariatric surgery. METHODS: laparoscopic adjustable gastric banding is the most widely performed bariatric procedure in Europe. Acute anterior/posterior slippage of the gastric wall is the most frequent complication, and needs emerge

Appropriateness criteria for bariatric surgery: beyond the NIH guidelines.

Careful selection of bariatric patients is critical for successful outcomes. In 1991, the NIH first established patient selection guidelines; however, some surgeons operate on individuals outside of these criteria, i.e., extreme age groups. We developed appropriateness criteria for the spectrum of patient characteristics including age, BMI, and severity of eight obesity-related comorbidities. Candidate criteria were developed using combinations of patient characteristics including BMI: > or =40 kg/m(2), 35-39, 32-34, 30-31, <30; age: 12-18, 19-55, 56-64, 65+ years old; and comorbidities: predi

Complications of bariatric surgery: presentation and emergency management--a review.

INTRODUCTION: The prevalence of obesity surgery is increasing rapidly in the UK as demand rises. Consequently, general surgeons on-call may be faced with the complications of such surgery and need to have an understanding about how to manage them, at least initially. Obesity surgery is mainly offered in tertiary centres but patients may present with problems to their local district hospital. This review summarises the main complications that may be encountered. MATERIALS AND METHODS: A full literature search was carried out looking at articles published in the last 10 years. Keywords for sear

Reproductive outcome after bariatric surgery: a critical review.

BACKGROUND: After many cycles of weight loss and weight gain, more and more morbidly obese patients undergo bariatric surgery, like gastric banding or gastric bypass, as the ultimate treatment for their obesity-problem. Since women of reproductive age are candidates for bariatric surgery, concerns arise regarding the potential impact on future pregnancy. METHODS: English-language articles were identified in a PUBMED search from 1982 to January 2008 using the keywords for pregnancy and bariatric surgery or gastric bypass or gastric banding. RESULTS: The few reported case-control and cohort st

Vitamin D deficiency in preoperative bariatric surgery patients.

BACKGROUND: Obese patients are at risk of hypovitaminosis D. This is particularly concerning for those considering bariatric surgery because of the risk of postoperative nutritional deficiency. We hypothesized that it is necessary to screen for vitamin D deficiency preoperatively and conducted a study to identify the patterns of vitamin D deficiency among prospective bariatric surgery patients. METHODS: A retrospective analysis of available preoperative laboratory values was conducted for all consecutive patients (n = 312) scheduled to undergo bariatric surgery from January 2004 to October 20

Wernicke encephalopathy after bariatric surgery: a systematic review.

OBJECTIVE: To review the clinical essentials of Wernicke encephalopathy (WE) after bariatric surgery. SUMMARY BACKGROUND DATA: An estimated 205,000 bariatric surgical procedures were performed in the United States in 2007. Such procedures may potentially lead to severe nutritional complications. METHODS: Literature searches were performed in Medline, Embase, and abstract collections. Inclusion criteria were WE after bariatric surgery, diagnosed by the presence of two or more of the following signs: mental status changes, eye movement abnormalities, cerebellar dysfunction, and dietary deficie

Histological alterations following bariatric surgery: pilot study.

BACKGROUND: Macroscopic clinical evidence of tissue damaging following bariatric surgery pushed plastic surgeons to presume microscopic alterations as well. METHODS: Five preliminary cases studied randomly, and compared with healthy tissues, confirmed these initial suspects. A deeper and wider study has then been structured. RESULTS: Preliminary results are so evident to encourage us to carry on an estimated 2-3 years multidisciplinary study. CONCLUSIONS: What we want to study is if histological post-bariatric alterations are confirmed, and if these can be considered responsible for higher

Childhood maltreatment and psychiatric morbidity in bariatric surgery candidates.

BACKGROUND: This study used standardized assessments to evaluate the association between childhood maltreatment (i.e., emotional, physical, and sexual abuse and emotional and physical neglect) and Axis I and II psychiatric disorders in patients presenting for bariatric surgery. METHODS: Participants (N = 230) provided demographic information and completed the Childhood Trauma Questionnaire, short form. The Structured Clinical Interview for the DSM-IV was used to assess Axis I clinical disorders and Axis II personality disorders. RESULTS: Approximately 66% of participants had a history of chi

Critical analysis of bariatric procedures in Prader-Willi syndrome.

Prader-Willi syndrome (PWS) is a complex genetic disorder localized to chromosome 15 and is considered the most common genetic cause of the development of life-threatening obesity. Although some morbidities associated with PWS, including respiratory disturbance/hypoventilation, diabetes, and stroke, are commonly seen in obesity, others such as osteoporosis, growth hormone deficiency, and hypogonadism, and also altered pain threshold and inability to vomit, pose unique issues. Various bariatric procedures have been used to cause gastric stasis, decrease gastric volume, and induce malabsorption,

Binge eating in the bariatric surgery population: a review of the literature.

OBJECTIVE: This article reviews the status of the literature addressing clinically significant binge eating in the bariatric surgery patient. The goal is to provide a background that will guide patients, surgeons, and mental health practitioners toward the most successful long-term surgical outcome when binge eating is identified. METHOD: Pubmed and Medline search with subsequent reference list search of identified articles. We searched literature through April 2006 on the influence of binge eating (BE) on surgical outcome. RESULTS: Those with pre-surgical BE are more likely to retain the ea

Short bowel syndrome following bariatric surgical procedures.

BACKGROUND: Short bowel syndrome (SBS) is a potential postoperative complication that is occurring with increasing frequency after bariatric procedures (BP). As long-term follow-up data are available, unusual long-term complications are being identified. Our goal was to examine the mechanisms and outcomes of patients with SBS following BP. STUDY DESIGN: We retrospectively reviewed our SBS database for patients following BP and examined their demographics, indications for resection, clinical course, and outcomes. RESULTS: Eleven of 265 patients had SBS following BP. Mean age was 31 years (ran

Prevention of trocar-wound hernia in laparoscopic bariatric operations.

BACKGROUND: Morbid obesity is a risk for fascial wound dehiscence and incisional hernia after abdominal surgery. The development of minimally invasive surgical techniques has led to a dramatic decrease in these complications. However, laparoscopic surgery may still be followed by trocar-wound herniation. Various methods have been advocated for its prevention. METHODS: The records of 752 patients who underwent laparoscopic bariatric operations (610 mini-gastric bypass and 142 gastric banding) as treatment for morbid obesity between October 2001 and June 2005, with regular follow-up, were retro

Medication and nutrient administration considerations after bariatric surgery.

PURPOSE: Medication and nutrient administration considerations after bariatric surgery are discussed. SUMMARY: Bariatric surgery is categorized by surgical technique (i.e., restrictive procedure or a combination of restrictive and malabsorptive procedures). Roux-en-Y gastric bypass is the most frequently performed bariatric surgery in the United States. Patients who have undergone this surgery are at risk for nutrient deficiencies. Several factors, such as pH and absorption sites, should be considered when providing these patients with appropriate supplementation. Drug solubility and surface
