Altered cerebral gray matter volume and functional connectivity in patients with residual dizziness of benign paroxysmal positional vertigo.

AIM: To provide a theoretical basis for the study of the pathogenesis of residual dizziness (RD) from the perspective of imaging. MATERIALS AND METHODS: The general clinical data of the RD group and healthy control (HC) group were statistically analysed by two independent sample t tests, rank sum tests or chi-square tests. The imaging data of the two groups of people were preprocessed and statistically analysed by using the data processing and analysis for brain imaging (DPABI) software package. RESULTS: Compared with the HC group, the grey matter volume (GMV) in the left medial superior fro

Analysis of Bone Mineral Density and Bithermal Caloric Test Results in Unilateral Idiopathic Benign Paroxysmal Positional Vertigo Patients.

OBJECTIVE: Benign paroxysmal positional vertigo (BPPV) is the most frequently diagnosed type of peripheral vestibular vertigo. Studies have reported a possible association between BPPV and osteoporosis; however, there is still a lack of evidence. This study aimed to investigate the characteristics of bone mineral density (BMD) and bithermal caloric test results in patients with unilateral posterior canal idiopathic benign paroxysmal positional vertigo (pc-iBPPV). METHODS: BMD was measured in 65pc-iBPPV menopausal female patients and 65 healthy controls using dual-energy xray absorptiometry an…

Vestibular Rehabilitation of the Persons affected by Benign Paroxysmal Positional Vertigo (BPPV) by Physical Therapy and Repositioning Maneuvers.

Vestibular rehabilitation therapy (VRT) mainly comprises physical therapies that encourage head, eye, and truncal movements, accelerating the recovery of patients with acute peripheral labyrinthine dysfunction. VRT aims to improve vestibular hypofunction by reinforcing vestibulo-ocular, vestibulospinal, and vestibulocollic reflexes. An asymmetry in peripheral vestibular inputs from the pair of membranous labyrinths to the central nervous system frequently results from vestibular lithiasis, causing benign paroxysmal positional vertigo (BPPV). The article discusses the pathophysiology, subtypes,…

Comparison of the Efficacy of Appiani and Zuma Maneuvers in Patients with Apogeotropic Horizontal Canal Benign Paroxysmal Positional Vertigo.

BACKGROUND: Apogeotropic horizontal canal benign paroxysmal positional vertigo (HC-BPPV) is a challenging variant of BPPV with lower treatment success rates compared to the geotropic form. The Appiani maneuver (ApM) and the Zuma maneuver (ZmM) represent 2 repositioning strategies targeting this condition, but head-to-head comparative data remain limited. METHODS: A prospective randomized comparative study was conducted between December 2024 and November 2025. Patients with unilateral apogeotropic HC-BPPV confirmed by videonystagmography (VNG)-assisted supine roll testing were assigned to eith…

Shigeno correction technique is effective in reducing postural crises in the treatment of posterior canal benign paroxysmal positional vertigo.

PURPOSE: Benign paroxysmal positional vertigo (BPPV) is the most common peripheral vestibular disorder, often affecting the posterior semicircular canal (PC). Treatment for PC-BPPV typically involves repositioning or liberatory maneuvers, such as the Epley and Semont maneuvers. A rare but concerning event may occur at the end of these maneuvers: the patient may experience an intense sensation of falling with pronounced retropulsion, resembling a "Tumarkin-like" crisis. This phenomenon is entirely distinct from the so-called "residual dizziness," a long-lasting, chronic condition unrelated to t…

Towards Providing an Automated Approach to Differentiating the Nystagmus of Ménière's Disease, Vestibular Migraine, and Benign Paroxysmal Positional Vertigo.

OBJECTIVE: The diagnosis of vertigo is challenging, particularly as patients usually present while asymptomatic. We have developed an ambulatory medical device that allows vestibular telemetry to record eye movements over a 30-day period to aid the diagnosis of vertigo. We have undertaken proof-of-concept work to identify unique properties of nystagmus that could be used to differentiate between three of the most common causes of vertigo: Ménière's disease, vestibular migraine, and Benign Paroxysmal Positional Vertigo. PATIENTS: We analyze the nystagmus from patients with a diagnosis of Méniè…

Benign paroxysmal positional vertigo in a child: an infrequent complication following a fairground ride and post-cochlear implant surgery.

OBJECTIVE AND IMPORTANCE: Benign paroxysmal positional vertigo (BPPV) is an uncommon complication that can develop after cochlear implantation. This condition has been documented in adults as a complication of cochlear implant surgery. However, there is no reported literature on BPPV in children who have undergone cochlear implant surgery. CLINICAL PRESENTATION: A 13-year-old girl underwent the insertion of a cochlear implant and developed BPPV as a result of visiting fairground rides approximately 2 years after surgery. INTERVENTION: We performed a right Epley's maneuver. She was advised to…

Pseudo-spontaneous nystagmus: a new sign to diagnose the affected side in lateral semicircular canal benign paroxysmal positional vertigo.

Early diagnosis of the affected side in Lateral Semicircular Canal Benign Paroxysmal Positional Vertigo is important in effectively applying treatment manoeuvres. This study was performed to examine the frequency of a new clinical sign, pseudo-spontaneous nystagmus, in a large cohort of patients with Lateral Semicircular Canal Benign Paroxysmal Positional Vertigo, comparing its efficacy in the identification of the involved side with that of other diagnostic signs, seated supine positioning nystagmus, and the intensity of the nystagmus evoked by the head yaw test in the supine position. Overal…

Walk-rotate-walk test identifies patients responding to Lempert's maneuver, with benign paroxysmal positional vertigo of the horizontal canal.

Two hundred and fifteen patients were diagnosed and treated for benign paroxysmal positional vertigo of the horizontal canal (BPPV-HC). All patients were tested with conventional positional nystagmus tests lying supine and rotating head for geotropic nystagmus, registered with Frenzels glasses, and in 109 cases with ENG. The walk-rotate-walk (WRW) test, developed by one of us (T.R.) and described in the text, was applied to all patients. The immediate good treatment results with Lempert's maneuver verify the correct diagnosis of BPPV-HC. The WRW test is a more sensitive test for BPPV-HC than e…

Atypical posterior canal benign paroxysmal positional vertigo mimicking anterior canal involvement: a case series and review of the literature.

BACKGROUND: Benign paroxysmal positional vertigo most commonly affects the posterior semicircular canal, producing upbeat-torsional nystagmus during positional testing. An apogeotropic variant of posterior semicircular canal benign paroxysmal positional vertigo may present with downbeat-torsional nystagmus, thereby mimicking anterior canal benign paroxysmal positional vertigo and potentially leading to misdiagnosis. CASE PRESENTATION: We report a case series of 14 patients (mean age 56.2 years; 10 females, 4 males; all of Kosovar ethnicity) presenting with positional downbeat nystagmus sugges…
