Bile duct injury (BDI) is a well-recognised complication of laparoscopic cholecystectomy (LC). Following a BDI, bile usually leaks into the peritoneal space and causes biliary peritonitis. This manifests as non-specific abdominal pain and fever occurring several days after the surgery. It can be managed by laparoscopic washout with or without bile duct repair. Congenital bile duct cysts are rare in adulthood. The most frequently used classification was proposed by Todani in 1977. However, in rare cases, not all the bile duct cysts are suitable to this classification. Hereby, we describe the case of an unclassified and very rare form of congenital bile duct cyst--isolated cystic duct cyst. En-block resection of the cyst, along with gallbladder, is the treatment of choice. Although exceptional, cystic duct cysts should be included in Todani classification so that the surgeons to be aware for this variation.

Congenital cystic dilatation of bile ducts is a rare condition. We report a retrospective study about 18 patients having congenital bile duct cysts. According to Todani's classification, 11 cases were type I and 7 were type V. Six patients from the first group had a pancreatobiliary maljunction. Perioperative cholangiography and laparoscopic ultrasound are two equally effective methods of diagnosing common bile duct stones. Ultrasound, however, can diagnose other pathological changes and is not associated with radiation risk. Intraoperative diagnostics should be performed in cases where the risk of stones or other pathology is considered to be high. Standard intraoperative diagnostics probably do not reduce the risk of accidential lesions of the common bile duct. Choledochoscopy and perioperative ERCP include a possibility for therapy and may be chosen in selected cases.

Endoscopic sphincterotomy (ES) has become the gold standard nonoperative modality for the removal of common bile duct (CBD) stones. Morbidity is 2%-10%, and mortality less than 2%.
