A case of a rare anomaly of the common bile duct associated with an abnormal arrangement of the pancreaticobiliary ductal union.

A 39 year-old Japanese female patient with a duplication of the distal portion of the common bile duct is presented herein. Moreover, an abnormal arrangement of the pancreaticobiliary ductal union, congenital biliary dilatation and carcinoma of the gallbladder were all demonstrated by cholangiographic and endoscopical studies. The patient underwent radical surgery for advanced adenosquamous carcinoma of the gallbladder, and her postoperative course was satisfactory. A reflux of pancreatic juice into the bile duct was demonstrated, but it was eliminated and considered to be a contributory etiol

DNA damage in the stomach after vagotomy measured by 32P-postlabelling.

This study analysed gastric mucosal DNA by 32P-postlabelling in a series of patients who have had previous vagotomy for benign peptic ulcer disease. DNA adduct levels were found to be significantly higher in patients who had had previous truncal vagotomy than in those who had had previous highly selective vagotomy (p < 0.001). Intragastric bile concentrations were also considerably higher in patients after truncal vagotomy but there was no correlation between intragastric bile concentrations and DNA adduct levels. These results suggest that, although duodenogastric reflux may be a cause of gas

Synchronous gallbladder and pancreatic cancer associated with pancreaticobiliary maljunction.

We report the case of a 46-year-old woman who presented with chronic intermittent abdominal pain without jaundice; abdominal ultrasonography showed thickening of the gallbladder wall and dilatation of the bile duct. Endoscopic retrograde cholangiopancreaticography showed pancreatobiliary maljunction with proximal common bile duct dilatation. Pancreatobiliary maljunction was diagnosed. A computed tomography scan of the abdomen showed suspected gallbladder cancer and distal common bile duct obstruction. A pancreatic head mass was incidentally found intraoperative. Radical cholecystectomy with pa

The Impact of Laryngopharyngeal Reflux on Occurrence and Clinical Course of Recurrent Respiratory Papillomatosis.

OBJECTIVES/HYPOTHESIS: Laryngopharyngeal reflux (LPR) has been proposed both as a trigger for recurrent respiratory papillomatosis (RRP) onset and as a factor favoring an aggressive clinical course. STUDY DESIGN: In this prospective study, 106 participants were recruited within a period of 24 months at the Department of Otorhinolaryngology and Cervicofacial Surgery, University Medical Centre Ljubljana. METHODS: This study compared a group of RRP patients (N = 36) with a group of LPR patients (N = 28) and a group of healthy participants (N = 42) based on Reflux Symptom Index (RSI), Reflux Fin

Comparison Between Billroth-II with Braun and Roux-en-Y Reconstruction After Laparoscopic Distal Gastrectomy.

PURPOSE: This study aims to compare the effectiveness of Billroth-II with Braun and Roux-en-Y reconstruction after laparoscopic distal gastr … c residue and reflux esophagitis were not significantly different between the groups. Gastritis and bile reflux were more frequently observed in the Billroth-II with Braun group (p = 0.004 and p < 0.001, respectively). At 2 years postoperatively, gastric residue was not significantly different, but gastritis, bile reflux, and esophagitis were more frequent in …

The effect of lethal endotoxaemia on the liver glycogen metabolism in guineapigs with common bile duct obstruction.

The effect of hepatocellular trauma due to bile reflux and endotoxaemia on liver glycogen metabolism was studied in guineapigs with common bile duct obstruction. Considerable drops in basal glycogen levels of hepatocytes from bile duct ligated (BDL) animals were recorded in comparison with the sham operated (SHAM) ones. However, the regurgitation of bile did not affect the basal blood glucose concentrations of BDL guineapigs. The circulating glucose was consumed, mainly reflecting the level of energy requirement of the peripheral tissues in the endotoxaemic SHAM pair-fed animals and the BDL gr…

A Role for High Mobility Group Box 1 (HMGB1) Release in the Pathogenesis of Gastroesophageal Reflux Disease.

BACKGROUND: Many gastroesophageal reflux disease (GORD) patients have heartburn symptoms despite PPI treatment, potentially via esophageal mucosa exposure to weakly acidic bile salts, leading to oxidative stress. High mobility group box 1 (HMGB1) is a nuclear protein secreted during oxidative stress that binds to receptors including TLR2, TLR4, and RAGE, inducing inflammatory signaling. We describe the release and potential downstream effects of HMGB1 in GORD. METHODS: Esophageal biopsies were obtained from healthy controls (HC), functional heartburn (FH), non-erosive reflux disease (NERD), a

One-Anastomosis Gastric Bypass Revision for Gastroesophageal Reflux Disease: Long Versus Short Biliopancreatic Limb Roux-en-Y Gastric Bypass.

PURPOSE: One-anastomosis gastric bypass (OAGB) may be associated with refractory gastroesophageal reflux disease (GERD). The nature of this GERD remains unclear. This complication can be treated either with an additional enteroenterostomy without shortening of gastric pouch (long biliopancreatic limb Roux-en-Y gastric bypass (L-BPL-RYGB)) or revision to conventional short biliopancreatic limb Roux-en-Y gastric bypass (S-BPL-RYGB). The objective of this study is to compare the aforementioned procedures in terms of efficacy on GERD symptoms. MATERIALS AND METHODS: Retrospective analysis between

Importance of duodenogastric reflux in the surgical outpatient practice.

To the surgeon, the importance of duodeno-gastro-esophageal reflux is related firstly to the injurious effects it has on the mucosa of the stomach and the esophagus, and secondly to the underlying pathophysiology causing the regulation of duodenal contents into the stomach and subsequently into the esophagus. Normally, the reflux of duodenal contents into the stomach rarely causes symptoms and consequently is usually not a primary disease. The symptoms develop after operations that distort or remove the pylorus. Consequently, in most situations the disease is iatrogenic in etiology. In contras

Cell proliferation in the gastric corpus in Helicobacter pylori associated gastritis and after gastric resection.

Patients who have undergone gastric resection are at higher risk of developing gastric carcinoma than normal subjects, and bile reflux is believed to play a role in carcinogenesis. An increase in mucosal cell proliferation increases the likelihood of a neoplastic clone of epithelial cells emerging, particularly where there is chronic epithelial injury associated with bile reflux. Helicobacter pylori is considered a major risk factor for gastric cancer in the intact stomach. It has been shown previously that antral cell proliferation is increased in H pylori gastritis and falls to normal levels…

The production of anomalous pancreaticobiliary ductal union in canine models.

For the purpose of resolving the mechanism of the ill-effect of refluxed pancreatic juice, due to the anomalous pancreaticobiliary ductal union, canine models were produced. Mongrel adult dogs and puppies were used. Pancreatico-cholecystostomy was performed on 5 adult dogs and 5 puppies. Pancreatico-choledochostomy was performed on 10 adult dogs and 5 puppies. In both procedures, the cylindrical dilatation of the choledochus resulted. The pathological changes in the choledochus were very similar to the cylindrical choledochal dilatation in the human. Elastic fibers diminished universally and i

The postcholecystectomy syndrome. A role for duodenogastric reflux.

We have assessed the relationship between dyspepsia and gallstones and evaluated the effects of cholecystectomy on symptoms, endoscopic findings, and degree of duodenogastric reflux. Thirty patients with gallstones were enrolled in our study. Their symptoms, gastroscopic findings, and bile salt concentrations in fasting gastric juice were evaluated before and after surgery. Before cholecystectomy, biliary colic was present in 26 patients and dyspepsia in 20 patients; 16 patients also had biliary colic. After surgery, biliary colic disappeared in all patients. Dyspeptic symptoms improved in 12

The Role of Laryngopharyngeal Reflux Disease in Laryngeal and Hypopharyngeal Squamous Cell Carcinoma: A Systematic Review.

BACKGROUND: Laryngeal and hypopharyngeal squamous cell carcinomas (L/HSCC) are common head and neck cancers with rising incidence globally. Laryngopharyngeal reflux disease (LPRD) is suspected to play a role in carcinogenesis through unknown mechanisms. The aim of this review was to summarize the literature findings about the potential carcinogenesis mechanisms associated with LPRD in L/HSCC. METHODS: A PubMed, Embase, and Web of Science database search was carried out by two independent investigators for studies investigating laryngeal and hypopharyngeal mucosa injuries and premalignancy mod

Targeting LIM kinase in cancer and neurofibromatosis.

OBJECTIVE AND IMPORTANCE: The gallbladder is used to divert cerebrospinal fluid (CSF) in patients with hydrocephalus when all other sites have been exhausted. This is seen in hydrocephalic patients who have reached teenage years but have undergone multiple shunt revisions, abdominal operations and repeated neck vein cannulations during childhood. One complication of the lumbar-gallbladder shunt is discussed as well as its pathophysiologic theory and management. CLINICAL PRESENTATION: A case of a patient with a lumbar-gallbladder shunt who developed chemical meningitis from reflux of bile into

Combined antrectomy and Roux-en-Y anastomosis in the surgical treatment of recurrent peptic ulceration.

The results of antrectomy performed for recurrent peptic ulceration after vagotomy and drainage are frequently marred by bilious vomiting. Consequently, there has been interest in combining antrectomy with Roux-en-Y drainage to prevent this complication. The experience of one surgeon in revisional gastric surgery has been studied retrospectively. Thirty-six patients have been reviewed in detail, 24 of whom had a combined antrectomy and Roux-en-Y anastomosis: 16 for recurrent peptic ulceration, 2 for non-ulcer dyspepsia and 6 for bile reflux gastritis. None of these has required reoperation, an…

Benign stenosis of the papilla of Vater. Diagnosis of 119 cases with conventional and selective low radiomanometry.

Intra-operative radiomanometry with serial conventional films (panoramic cholangiography) and intra-abdominally placed large dental films (selective low cholangiography) produce a dynamic and static, millimetric study of the lower part of the common duct. It revealed in 2,095 patients with benign biliary pathology 119 cases of stenosis of the papilla of which 34 are detected during secondary operations of 85 patients. With the procedure of the authors a clear definition of the benign stenosis of the papilla is obtained radiomanometrically: 1. moderate hypertony; 2. disappearance of the normal
