A 30-year-old AIDS-patient with cryptococcal meningitis developed subacute bilateral visual loss associated with high cerebrospinal fluid (CSF) pressure. With immediate CSF drainage the blindness was reversible. The importance of prompt CSF drainage in AIDS-related cryptococcal meningitis with visual failure is stressed.

The Sofia Eye Survey was a population-based blindness and visual impairment survey of 6,275 randomly selected men and women from the Sofia district and Sofia city in western Bulgaria. A complete house-to-house census on all adults over age 40 years was performed, and visual acuity performed on 98%. Two cases of transient cortical blindness during coronary and graft angiography are described. In both cases, the onset of blindness was immediately preceded by manipulation of a guidewire in the left subclavian artery during attempts to selectively engage the internal mammary artery. The ERGs of 9 cases (18 eyes) of congenital stationary night blindness with normal fundi or myopia were tested. All eyes showed nonrecordable rod ERG and cone ERG with normal a-wave. Scotopic mixed ERGs were of the negative type in 7 eyes and of the subnormal type in 11 eyes. The b/a ratio was reduced in all eyes. The ERG characteristics are useful for the classification, estimation of the probable location of the lesion, and differential diagnosis of the disease.

In a series of 11 patients with classic pituitary apoplexy, two patients had the acute onset of bilateral blindness. After transsphenoidal decompressive surgery both patients had useful recovery of vision. Although impairment of neurologic function appears irreversible when central nervous tissue has been compressed to the point of total loss of function it would appear that the optic chiasm may be an exception to this experience.
