Apheresis to Mitigate Atherosclerotic Vascular Disease.

BACKGROUND: Therapeutic apheresis is a term used to describe a group of treatments where blood components are separated in real time, and one component is removed, exchanged, and/or treated to remove pathogenic substances from the circulation. Plasma exchange, which removed all plasma components, and lipid apheresis which selectively removes lipoproteins from circulation, have both been used to treat atherosclerotic vascular diseases. METHODS: To review the literature regarding the application of therapeutic apheresis for atherosclerotic vascular diseases. RESULTS: Primarily lipid apheresis

Therapeutic plasma exchange: Experience in a third level hospital, 2013-2016, Lima (Peru).

BACKGROUND: Therapeutic plasma exchange (TPE) is an extracorporeal procedure which consists of removing the patient's plasma and replacing it with an appropriate replacement fluid. Plasma and blood cells are separated by a centrifugation process. Our department has used TPE for several years, and in 2013 we introduced an institutional apheresis protocol. The main objective of this report is to describe the TPE procedures performed between 2013 and 2016 in the Peruvian population. METHODS: We analyzed the technical and clinical aspects of 864 centrifugal TPE procedures as well as the associate

[The use of therapeutic apheresis in neurological diseases and comparison between plasma exchange and immunoadsorption].

The use of plasmapheresis in neurological diseases with immune-mediated pathogenesis is widely certified. In recent years, the technological evolution of the dialysis membranes allowed to accompany the classical plasma exchange (PEX) treatment of apheresis by means of selective adsorption (IA). It has proved to be of equal therapeutic efficacy and, at the same time, devoid of most of the PEX side effects. The recent guidelines of the American Society for Apheresis (ASFA) and, later, the American Academy of Neurology, outlined directions and diagrams for the application of the method that has f

Recent advances in therapeutic apheresis.

Recent advances in therapeutic apheresis include technical improvements, new indications, and pathophysiological insights. A new device that adsorbs endotoxins onto immobilized human albumin from human whole blood was recently developed. In a prospective randomized controlled trial (endotoxin adsorber study EASY), apheresis-treated patients had more improved APACHE II scores than controls. In a prospective randomized trial, the Prosorba column containing immobilized staphylococcal protein A was tested against sham apheresis in patients with end-stage rheumatoid arthritis. A significant improve

Successful twenty-four-hour lung preservation with donor core cooling and leukocyte depletion in an orthotopic double lung transplantation model.

Donor core cooling with cardiopulmonary bypass is a valid method of clinical lung preservation. However, organ ischemia with this method is still limited to short-term intervals. Since circulating leukocytes participate in postischemic injury through the release of oxygen-derived free radicals, we examined whether leukocyte depletion by mechanical filtration could extend ischemic tolerance of the lung during preservation and subsequent double lung transplantation. Bovine donor lungs were preserved by donor core cooling (10 degrees to 15 degrees C) with cardiopulmonary bypass. Donor lungs were

ABO-incompatible kidney transplantation: impact of apheresis on graft and patient survival in recipients with low isoagglutinin titer.

ABO-incompatible (ABOi) kidney transplantation carries a high risk of acute antibody-mediated rejection due to the presence of isoagglutinins. To mitigate this risk, current protocols recommend performing apheresis before transplantation. Our objective was to evaluate outcomes in ABOi recipients with low isoagglutinin titers, comparing those who did and did not undergo pre-transplant apheresis. We conducted a multicenter study including recipients of ABOi kidney transplants between 2012 and 2022. A total of 78 patients with baseline isoagglutinin titers ≤1:8 were included; 41 received pre-tran

[Collection of peripheral blood stem cells from ABO incompatible allogeneic donors by using blood cell separator].

To evaluate the yield of the blood cell separator for collection of peripheral blood stem cells (PBSC) from ABO major and (or) minor incompatible allogeneic donors and the feasibility of PBSC component infusion to the recipients without removal of erythrocytes or plasma, the Cobe Spectra (Version 6.1) blood cell separator was utilized to collect PBSC component from 9 allogeneic donors. Of all the donors, 4 were ABO major incompatible, 2 were minor incompatible and the other 3 were both major and minor incompatible to corresponding recipients. In each cycle, different amount of PBSC component w

Lp(a) and cardiovascular risk: Investigating the hidden side of the moon.

AIMS: This article reports current evidence on the association between Lp(a) and cardiovascular (CV) disease and on pathophysiological mechanisms. The available information on therapy for reduction of lipoprotein(a) is also discussed. DATA SYNTHESIS: Although some evidence is conflicting, Lp(a) seems to increase CV risk through stimulation of platelet aggregation, inhibition of tissue factor pathway inhibitor, alteration of fibrin clot structure and promotion of endothelial dysfunction and phospholipid oxidation. Lp(a) 3.5-fold higher than normal increases the risk of coronary heart disease a

[H.E.L.P. therapy (heparin-induced extracorporeal LDL precipitation) in sudden deafness].

BACKGROUND: The H.E.L.P. (Heparin-induced extracorporal low-density lipoprotein precipitation) system selectively eliminates fibrinogen, LDL (low-density lipoprotein) cholesterol, triglycerides, and LP (a) (lipoprotein a) from the blood plasma by means of extracorporal circulation. The reduction of these macromolecules immediately improves the hemorheological situation and leads to a significant decrease of plasma viscosity and red cell transmission time (RCTT), which correlates to the deformation of red blood cells. There are no known contraindications for this therapy. PATIENTS: Sudden hear

Hematopoietic progenitor cell count as a potential quantitative marker in apheresis products during allogeneic stem cell transplantation.

BACKGROUND: The quality and quantity of hematopoietic stem cells in apheresis products are essential to the success of peripheral blood hematopoietic stem cell transplantation (PB-HSCT). While the flow cytometry measurement of CD34+ cells as a golden standard for stem cell count is labor and cost-intensive, hematopoietic progenitor cell number evaluated by XN Sysmex series automated hematology analyzers (XN-HPC) is suggested as a surrogate marker. MATERIALS AND METHODS: We evaluated the correlation and consistency of XN-HPC and CD34+ cell count in apheresis samples from both allogeneic donors

State of the art of low-density lipoprotein apheresis in the year 2003.

Low-density lipoprotein (LDL) apheresis is a last-resort treatment for hypercholesterolemic patients resistant to conservative lipid-lowering therapy. In the extracorporeal circuit, LDL, Lp(a) and coagulation factors are selectively eliminated, while the beneficial proteins like high-density lipoprotein, albumin and immunoglobulins are returned to the patient. Clinical effects of LDL apheresis comprise improvement of symptoms like angina and exercise tolerance, reduction of clinical coronary events like unstable angina, need for angioplasty or bypass operation, myocardial infarction and ultima

Measurable residual disease assessment in apheresis product from multiple myeloma patients: Correlation with biology, depth of response and survival.

While measurable residual disease (MRD) in bone marrow (bmMRD) is a validated surrogate biomarker in multiple myeloma (MM), the clinical relevance of MRD in the apheresis product (aMRD) remains unclear. We retrospectively analysed 422 MM patients who underwent autologous stem cell transplantation (ASCT) between 2005 and 2023 at the University Hospital of Salamanca, all evaluated for aMRD by 8-colour flow cytometry. Overall, 45 patients (10%) were aMRD+ve and 377 (90%) were aMRD-ve, with similar baseline characteristics except for a higher frequency of t(11;14) in aMRD+ve cases. Patients with r

Therapeutic Apheresis for Neurological Diseases: A Five-Year Retrospective Study at a Large Academic Medical Center.

Therapeutic plasma exchange (TPE) represents a crucial treatment modality for a wide range of neurological disorders. However, data regarding institutional variation in practice and clinical trends over time is limited, especially concerning diagnostic indications, treatment setting, line access, safety, and the impact of novel drug therapies on TPE utilization. We analyzed all TPE procedures performed at our institution between 2019 and 2023 for neurological indications. A total of 109 patients and 1310 procedures were retrospectively reviewed, focusing on procedure volumes, clinical paramete

Evinacumab as an adjunct to lipid apheresis in an infant with homozygous familial hypercholesterolemia.

OBJECTIVES: Investigation of the efficacy of the angiopoietin-like protein 3 (ANGPTL3) antibody evinacumab in a four-year-old infant with homozygous familial hypercholesterolemia (HoFH) as an adjunct to lipid apheresis. CASE PRESENTATION: A two-year-old boy was found to have xanthomas of the Achilles tendon and LDL cholesterol levels around 900 mg/dL. HoHF was subsequently confirmed by molecular genetics. At the age of three, lipid apheresis was started twice a week. At the age of four, a four-weekly infusion of evinacumab was started. This resulted in a 67 % reduction in LDL cholesterol befo

Efficacy of granulocyte and monocyte adsorptive apheresis on skin and joint manifestations of palmoplantar pustulosis with pustulotic arthro-osteitis: A multicentric, prospective, observational study.

Granulocyte and monocyte adsorptive apheresis (GMA) selectively removes activated granulocytes and monocytes from the peripheral blood. In 2012, GMA was approved in Japan as a treatment for generalized pustular psoriasis and localized pustular psoriasis or palmoplantar pustulosis (PPP). Limited evidence from case reports and monocentric studies suggested that GMA is an effective treatment for skin and joint symptoms of PPP with pustulotic arthro-osteitis (PAO). The present, prospective, observational study was performed at three dermatology departments in Japan to evaluate the efficacy and saf

Comparison of cytokine changes in three different lipoprotein apheresis systems in an ex vivo whole blood model.

INTRODUCTION: Even if proprotein convertase subtilisin/kexin type 9 inhibitors have replaced lipoprotein apheresis in many patients, lipoprotein apheresis still is an important option in homozygous familial hypercholesterolemia, progressive atherosclerosis or when removal of lipoprotein(a) is indicated. Additional possible favorable effects beyond lipid lowering could include changes in the concentration of cytokines and improvement of hemorheology. METHODS: We evaluated how whole blood adsorption, dextran sulfate plasma adsorption, and double filtration plasmapheresis lipoprotein apheresis s

Low-density lipoprotein apheresis therapy for steroid- and cyclosporine-resistant idiopathic membranous nephropathy.

We herein report three cases of Nephrotic Syndrome (NS) with idiopathic membranous nephropathy (IMN) in which the patients were resistant to steroid (PSL) and cyclosporine (CyA) therapy. All three patients had a high risk of renal failure because of persistently high levels of proteinuria. We performed low-density lipoprotein apheresis (LDL-A) and achieved complete remission in two cases. The third patients developed NS relapse and a deteriorating renal function; however, withdrawal from dialysis therapy was achieved. There are no proven therapies for treating patients with PSL- and CyA-resist
