The purpose of the present study was to evaluate the impact of frozen red cell transfusion on the transmission of hepatitis C virus (HCV) before the introduction of blood donor screening. Anti-HCV antibodies were detected in 59 patients with sickle-cell disease who required chronic transfusions and had exclusively received frozen red blood cells (RBC). The files were reviewed for clinical signs of chronic hepatitis C. Anti-HCV antibodies were detected in 2 adult patients; both also had clinical evidence of HCV infection. No other patient showed signs of acute or chronic HCV hepatitis. In a con

It is prudent to avoid blood transfusion whenever possible. Recent reports question the standards previously considered acceptable for blood transfusion. Acute hemorrhage followed by hypovolemia, tachycardia and orthostatic hypotension with dizziness are still considered acceptable criteria for automatic blood transfusion in most patients. We report two cases in which hypovolemia, tachycardia and orthostatic hypotension with dizziness were present. Transfusion was avoided. Hypovolemia, tachycardia and orthostatic hypotension with dizziness do not require transfusion in a few carefully selected…

An assessment of the benefits of prophylactic blood transfusion during 51 pregnancies in women with haemoglobin SS, haemoglobin SC and haemoglobin S-beta-thalassaemia in a retrospective study from 1978 to 1984 has failed to demonstrate any clear conclusions as to their efficacy. Of the transfused patients, 22% developed atypical red cell antibodies and 14% had immediate minor transfusion reactions. Our data show no significant difference in fetal or maternal outcome between those patients who were transfused prophylactically and those who were not. A prospective study to clarify the appropriat…

Ten homozygous sickle cell patients, aged from 4 to 20 years, underwent a periodic red blood cells transfusion program. Indications were = one stroke or more in 4 cases, repeated severe painful crisis in 2 cases, and for others = chronic anaemia induced by hypersplenism, priapism, chronic leg ulceration, dwarfism with delayed sexual maturation. Transfusion program lasted from 6 months to 6 years. In 7 cases, a phlebotomy preceded the transfusion by the same venipuncture in order to reduce the inevitable iron overload progression. Clinical benefit was clear in the following cases: lack of painf

A total of 130 cordocenteses, including 96 intravascular fetal blood transfusions, were performed in 21 pregnancies complicated by red cell isoimmunization. Transfusions were commended at 18-34 weeks' gestation and repeated up to 7 times, at 1- to 4-week intervals. The volumes of transfused blood were 5-150 ml, the haematocrits 62-88% and the rate of transfusions 1-15 ml/min. The pretransfusion fetal haematocrits were 5-34% and the posttransfusion ones 28-60%. Final transfusions were given at 32-36 weeks' gestation. Of the 21 pregnancies, 18 delivered at 31-39 weeks and 3 are continuing; 1 bab…

Transfusion of 500 ml of blood, contributed by a donor three days before the onset of Rocky Mountain spotted fever and refrigerated for nine days, caused this disease in the recipient. The blood donor died of Rocky Mountain spotted fever after six days; rickettsia were identified in various tissues by immunofluorescence techniques. The recipient of the blood became mildly ill and recovered fully; specific antibiotic treatment was initiated on the fourth day of illness. Diagnosis of Rocky Mountain spotted fever was confirmed in the recipient by positive serologic reactions and isolation of Rick

Forty-four patients undergoing coronary artery bypass surgery participated in an autologous blood donor program. Blood was collected at weekly intervals up to three days prior to the scheduled date of surgery. Sixteen patients donated a total of two autologous units each, and 28 patients donated one autologous unit each. No donor morbidity or mortality was encountered during or following blood donation. Autologous blood accounted for more than one-third of all blood required by these patients throughout their hospital courses. Autologous transfusion appears to be a safe procedure for patients

Seven White American male blood donors with Italian surnames were found to have red cell glucose-6-phosphate dehydrogenase (G-6-PD) deficiency among 1,285 with Greek or Italian surnames screened. Five different genetic variants were found; G-6-PDs Mediterranean (2), "Athens-like" (2), San Juan, Columbus and "Canton-like". Clinical evaluation of 23 patients who received 24 units of G-6-PD-deficient blood (G-6-PD A-) failed to reveal any deleterious effects. Screening of Black donors for G-6-PD deficiency is believed unnecessary; further data are needed before a recommendation can be made concer
