Efficacy and safety of sibutramine in obese white and African American patients with hypertension: a 1-year, double-blind, placebo-controlled, multicenter trial.

BACKGROUND: Obesity is a highly prevalent medical condition and is commonly accompanied by hypertension. This study assessed the efficacy and safety of treatment with sibutramine hydrochloride for promoting and maintaining weight loss in obese patients with controlled hypertension, including a subset analysis of African American patients. PATIENTS AND METHODS: Obese patients with a body mass index (BMI, calculated as weight in kilograms divided by the square of height in meters) between 27 and 40 and a history of hypertension controlled with a calcium channel blocker (with or without concomit

Normal bone mineral density after treatment for childhood acute lymphoblastic leukemia diagnosed between 1991 and 1998.

OBJECTIVE: We investigated whether previous reports of reduced bone mineral density after management for childhood acute lymphoblastic leukemia (chALL) were confirmed in a more recently treated cohort. STUDY DESIGN: In a cross-sectional study 75 subjects who were given the diagnosis of chALL between January 1, 1991, and December 31, 1997 (69% standard, 31% high risk), at Denver Children's Hospital and who were 11 to 82 months post-diagnosis with no history of relapse, secondary malignancy, or transplant underwent whole body areal bone mineral densitometry (BMD(A) expressed as age- and sex-sta

The effect of hypothetical interventions to limit weight gain on incident type 2 diabetes mellitus among people with HIV initiating antiretroviral therapy in the United States, 2012-2021.

Among people with HIV (PWH), initiating antiretroviral therapy (ART) is associated with weight gain that might increase the risk of cardiometabolic outcomes. We estimated the effect of hypothetical interventions to restrict increases in body mass index (BMI) in the two years after ART initiation on the 7-year risk of incident type-2 diabetes mellitus (T2DM) among PWH in the North American AIDS Cohort Collaboration on Research and Design (NA-ACCORD) clinical cohorts using the parametric extended g-formula. Among 13,822 normal weight or overweight PWH (BMI ≥18.5 and ≤29.9 kg/m2) who initiated AR

Prognostic implications of weight gain and weight loss in adults with congenital heart disease.

BACKGROUND: There are conflicting data about the association between obesity and clinical outcomes in adults with congenital heart disease (CHD), and the effects of weight gain or weight loss remain unclear. The purpose of this study was to determine whether a temporal change in body mass index (BMI) was associated with clinical outcomes independent of baseline BMI in adults with CHD. METHODS: Retrospective cohort study of adults with CHD that had clinical assessments at baseline and 5 years afterwards. Weight gain and weight loss were defined as ≥5% change from baseline BMI. Atherosclerotic

A pilot evaluation of the efficacy and acceptability of a novel imaginal exposure prevention (I-ERP) group programme to treat core weight, shape and social fears or phobias in adolescents with anorexia nervosa in an inpatient setting.

BACKGROUND: Anxiety due to a phobia of normal body weight is a core feature and maintenance factor of anorexia nervosa (AN). This is the first study to explore the efficacy and acceptability of using a novel imaginal exposure response prevention group to target fears associated with being a normal body weight to reduce anxiety in adolescents with AN. METHODS: The lead author adapted an I-ERP manual used to treat AN in adults in 1-1 therapy. Content was adapted for an adolescent population, sessions reduced from 10 to 4 and was delivered in a group format with audio recordings to be more acces

Inclusion of butterfly pea hay in corn silage (with or without cobs) increases nutrient intake and carcass weight in lambs.

This study evaluated the effects of mixed corn silages, with and without commercially viable cobs, combined with butterfly pea (Clitoria ternatea L.) hay on intake, digestibility, water balance, nitrogen balance, and carcass characteristics of lambs. Twenty crossbred intact male lambs (18.0 ± 4.8 kg body weight) were distributed in a randomized complete block design with a 2 × 2 factorial arrangement and five replicates per treatment. Treatments consisted of corn silage with cobs (CSC), CSC plus butterfly pea hay (BH), corn silage without cobs (CSWC), and CSWC plus BH. Butterfly pea hay inclus

Biphasic pattern of thymus regeneration after whole-body irradiation.

Whole-body irradiation of mice with 300 or 400 R causes a precipitous fall in thymus weight, followed by an increase in the mitotic index and an almost complete restoration of thymus mass. This phase is followed by a secondary fall in thymus weight and gradual recovery. This secondary fall can be prevented by intravenous injection of bone marrow or shielding of the hind limbs during irradiation. The hypothesis is proposed that the thymus depends on the migration of cells from the bone marrow to the thymus for the maintenance of its cell population. Bone marrow cells with chromosome markers inj

Exploring individualized crovalimab dosing in PNH through in silico modelling: Potential for improved convenience and cost efficiency.

AIM: Paroxysmal nocturnal haemoglobinuria (PNH) is a rare, acquired haematopoietic stem cell disorder. Crovalimab, a complement C5-inhibitor, is approved for PNH and can be self-administered subcutaneously every 4 weeks, offering a more convenient route than intravenous C5-inhibitors. Since exposure often exceeds pharmacokinetic targets and varies with body weight, we aimed to explored individualized dosing of crovalimab using clinical trial simulations to optimize therapy. METHODS: We compared standard, weight-based and therapeutic drug monitoring (TDM)-guided regimens in treatment-naïve PNH

Weight suppression predicts time to remission from bulimia nervosa.

OBJECTIVE: To investigate whether, at study entry, (a) weight suppression (WS), the difference between highest past adult weight and current weight, prospectively predicts time to first full remission from bulimia nervosa (BN) over a follow-up period of 8 years, and (b) weight change over time mediates the relationship between WS and time to first full remission. METHOD: A well-characterized sample of women with BN (N = 110; M age = 25.58 years, SD = 6.48) from the Massachusetts General Hospital Longitudinal Study of Eating Disorders was interviewed at 6-12 month intervals over 8 years. Behavioral Responses to Uncertainty in Weight-Restored Anorexia Nervosa - Preliminary Results.

Impaired decision-making under conditions of uncertainty seems to contribute to the expression and maintenance of anorexia nervosa (AN), but it is not clear whether this impairment is a disease state that would remit with treatment, or a persisting trait in patients with AN. To examine this question, a longitudinal study was conducted in 12 female inpatients with AN (age M = 22.2, SE = 1.36), before (Time-1) and after reaching a body mass index of >17.5 kg/m2 (Time-2). Intolerance of uncertainty (IU) was assessed via a decision-making task, the wheel of fortune (WOF). Weight gain at Time-2 was

Central and peripheral peptides regulating eating behaviour and energy homeostasis in anorexia nervosa and bulimia nervosa: a literature review.

A large body of literature suggests the occurrence of a dysregulation in both central and peripheral modulators of appetite in patients with anorexia nervosa (AN) and bulimia nervosa (BN), but at the moment, the state or trait-dependent nature of those changes is far from being clear. It has been proposed, although not definitively proved, that peptide alterations, even when secondary to malnutrition and/or to aberrant eating behaviours, might contribute to the genesis and the maintenance of some symptomatic aspects of AN and BN, thus affecting the course and the prognosis of these disorders.

Weight gain after diagnosis of gestational diabetes mellitus and its association with adverse pregnancy outcomes: a cohort study.

BACKGROUND: Gestational diabetes mellitus (GDM) and excessive body weight are two key risk factors for adverse perinatal outcomes. However, it is not clear whether restricted gestational weight gain (GWG) is favorable to reduce the risk for adverse pregnancy and neonatal outcomes in women with GDM. Therefore, this study aimed to assess the association of GWG after an oral glucose tolerance test with maternal and neonatal outcomes. METHODS: This prospective cohort study assessed the association of GWG after an oral glucose tolerance test (OGTT) with pregnancy and neonatal outcomes in 3126 wome

Ultrafiltration Rate Thresholds in Maintenance Hemodialysis: An NKF-KDOQI Controversies Report.

High hemodialysis ultrafiltration rate (UFR) is increasingly recognized as an important and modifiable risk factor for mortality among patients receiving maintenance hemodialysis. Recently, the Kidney Care Quality Alliance (KCQA) developed a UFR measure to assess dialysis unit care quality. The UFR measure was defined as UFR≥13mL/kg/h for patients with dialysis session length less than 240 minutes and was endorsed by the National Quality Forum as a quality measure in December 2015. Despite this, implementation of a UFR threshold remains controversial. In this NKF-KDOQI (National Kidney Foundat

Protein-calorie malnutrition in squirrel monkeys: adaptive response to calorie deficiency.

Control, low-protein, low-calorie, and low-protein-plus-low-calorie diets were fed to 37, 14, 8, and 16 infant squirrel monkeys, respectively, from ages 2-8 wk. The deficient diets were planned to prevent growth and were designed to examine the short- and long-term effects of defined nutritional restrictions on body weight, food intake, hematocrit, plasma albumin, growth hormone, and cortisol concentrations. During the restriction period, plasma albumin levels were significantly decreased in both groups with a protein restriction component: plasma cortisol levels were significantly increased o

Biliopancreatic diversion. Clinical experience.

AIM: Biliopancreatic diversion (BPD) has been accepted as an effective surgical treatment of morbid obesity. The paper presents the results of a personal clinical experience with this procedure, with special focus on the quality of life. METHODS: From January 1, 1992 to June 30, 2002, 122 patients (F/M: 108/14, mean age: 40.2 years, range 21-61) underwent BPD. Mean preoperative body weight was 122.4 kg (range 91-200), with a mean Body Mass Index of 49.1 kg/m2 (range 38-78). Three of these patients were converted from a previous vertical banded gastroplasty to BPD (1 patients with stomach pres

Association of serum butyrylcholinesterase with concurrent dry-weight change in maintenance hemodialysis: a retrospective longitudinal study.

BACKGROUND: Assessing nutrition-related change in maintenance hemodialysis is challenging because commonly used biomarkers such as serum albumin are influenced by inflammation and volume status. Prescribed dry weight (DW) is a clinically important treatment target, but longitudinal DW change may reflect both volume-related and nutrition-related body-mass change. Serum butyrylcholinesterase (BChE), a liver-synthesized protein, may better capture nutrition-related change in this setting. We examined whether longitudinal change in BChE was associated with concurrent change in prescribed DW and co

Childhood Eating and Feeding Disturbances.

Eating and feeding disturbances are prevalent yet understudied health conditions in youth. They are characterized by aberrant eating behaviors, cognitive and emotional dysfunctions, and dysregulated body weight. The Diagnostic and Statistical Manual of Mental Disorders Fifth Edition defines several feeding and eating disorders with a common onset in youth; however, data on their clinical validity at young ages are lacking. Further non-normative eating behaviors exist, but their clinical relevance needs elucidation. This Special Issue compiles state-of-the-art reviews and empirical research on

Juvenile generalized circinate pustular psoriasis treated with oral cyclosporin A.

We report on a 17-year-old boy presenting with relapsing generalized circinate pustular psoriasis exacerbated by streptococcal angina. Because of the severe course in his case, we started systemic treatment with cyclosporin A and corticosteroids. Corticosteroids could easily be tapered down. Cyclosporin A maintenance therapy was realized with 100 mg/day (1.6 mg per kg body weight and day). Side effects were a temporary increase in blood pressure during initiation with 200 mg cyclosporin A/day. After dose reduction no side effects were seen. The pustular lesions disappeared and the PASI score d
