PURPOSE: The purpose of this study was to evaluate whether the use of a longitudinal corticotomy (S-Z osteotomy) results in more rapid consolidation following distraction osteogenesis of short tibiae.
METHODS: Sixty-seven lengthening procedures were performed in 51 patients ranging in age from nine to 38 (mean 25) years. Diagnoses included short stature (32 tibiae), postpolio limb deformity (22 tibiae), osteomyelitis (three tibiae), trauma (two tibiae) and other diagnoses (eight tibiae). BACKGROUND: Recently, limb lengthening devices have shifted from external fixators to telescoping magnetic intramedullary lengthening nails (MILNs), which lengthen strictly along the bone's anatomic axis. A case of Ollier's disease with deformity and shortening of the humerus is presented. Lengthening of 9 cm and deformity correction of 50 degrees were accomplished with excellent functional and cosmetic results. Unique features of this case were the use of a multiaxial correction monolateral frame and the formation of normal bone within the region of diseased Ollier's bone.

A modified innominate osteotomy was employed to treat postural imbalance by a hemipelvic lengthening in twenty patients, five to twenty years old. Transiliac limb lengthening osteotomy is a modified Salter innominate osteotomy which uses a trapezoidal interposition bone graft instead of the usual triangular graft to achieve up to 3 cm of intrapelvic lengthening. Between June 1987 and June 1992, 62 tibiae in 52 patients underwent lengthening by using the Ilizarov technique. Follow-up was from 18 months to 5 years. The mean age at surgery was 12.9 years (range, 5-19). The etiology of limb shortening was congenital in 53 and acquired in nine tibiae. Thirty-five tibiae had bifocal and 27 had unifocal treatment. OBJECTIVE: To relate morphology of new bone formation to outcome after tibial lengthening performed in patients with achondroplasia.
MATERIAL AND METHODS: A retrospective analysis of 60 tibial segments in 30 achondroplasia patients was performed. There were 22 female patients and eight male patients, with a mean age of 9.8 years. New bone formation was classified by shape, homogeneity and density. Pixel values in relation to original bone were measured using a picture-archiving communication system (PACS). Clinical outcome was described by the external fixator and maturation indices.
