Prescription diuretic abuse in patients with bulimia nervosa.

Bulimia nervosa, a common eating disorder usually characterized by binge eating and self-induced vomiting, may also involve abuse of prescription diuretics. This article describes four patients who abused prescription diuretics in large quantities (up to 2 g/d of furosemide) for extended periods of time. Physical examination and laboratory values provided few clues to the diagnosis of bulimia nervosa. Other eating-related behaviors previously linked to bulimia nervosa--including abuse of diet pills, illicit amphetamines, and laxatives, as well as withholding of insulin in one diabetic patient-…

Empirical assessment of identity in anorexia and bulimia nervosa.

Researchers have neglected the study of the self concept of individuals who develop anorexia and bulimia nervosa. Towards rectifying this situation, Identity Structure Analysis (ISA) is introduced as an appropriate framework allowing the specification and empirical testing of postulates regarding disturbed identity development in anorexic disorders. The data presented distinguish a common constellation of identity characteristics in those suffering from anorexia and bulimia nervosa that differentiate them from normal and psychiatric control groups. Their identity pattern tends to be anti-devel…

Suppression of eating by fenfluramine in patients with bulimia nervosa.

Fifteen patients with bulimia nervosa received fenfluramine (60 mg po) or placebo under double-blind, randomly ordered conditions. Two hours later food was presented. Significantly less food was eaten after fenfluramine and the quantity eaten was inversely correlated with serum fenfluramine levels. Significantly fewer patients reported bulimic symptoms during the test after fenfluramine, but no significant effect was demonstrated after leaving the ward. Fenfluramine caused drowsiness but did not reduce hunger ratings. Similarly, eating failed to reduce hunger ratings normally in the patients. …

Electrolyte and other physiological abnormalities in patients with bulimia.

The frequencies of various forms of eating-related behaviour (such as vomiting and laxative abuse) are reported for a series of non-anorectic bulimia patients seen for evaluation in an eating disorders clinic. The results of serum electrolyte, glucose and other screening tests in these patients are presented. Electrolyte abnormalities were found in 82 of the 168 patients (48.8%) who were diagnosed as having either bulimia or atypical eating disorder. The most common abnormality was metabolic alkalosis (27.4%); hypochloremia (23.8%) and hypokalemia (13.7%) were also commonly seen. No significan…

Bulimia nervosa: an ominous variant of anorexia nervosa.

Thirty patients were selected for a prospective study according to two criteria: (i) an irresistible urge to overeat (bulimia nervosa), followed by self-induced vomiting or purging; (ii) a morbid fear of becoming fat. The majority of the patients had a previous history of true or cryptic anorexia nervosa. Self-induced vomiting and purging are secondary devices used by the patients to counteract the effects of overeating and prevent a gain in weight. These devices are dangerous for they are habit-forming and lead to potassium loss and other physical complications. In common with true anorexia n…

Salivary and gustatory alterations among bulimia nervosa patients.

OBJECTIVE AND DESIGN: This study systematically examined salivary composition and taste perception and monitored related subjective complaints in a group of bulimia nervosa (BN) patients. SUBJECTS: Fifty-two consenting female individuals participated in the current cross-sectional study, 26 patients diagnosed with BN according to the Diagnostic and Statistical Manual of Mental Disorders-IV (DSM-IV) criteria (mean age: 24+/-7 years) and 26 healthy controls matching in age and body mass index (BMI). METHODS: All participants were given diagnostic BN-related questionnaires, had taste and saliva…

Diagnosing bulimia nervosa with parotid swelling. Case report.

Patients with bulimia nervosa (BN) may show dental erosion, resulting from the effect of acid regurgitation. Asymptomatic bilateral parotid swellings may also be present. Other signs include serum electrolyte imbalance and Russell's sign. The authors describe the case of a 26-year-old woman with BN, whose only clinical manifestation of BN was her bilateral parotid swellings. Because patients with BN tend to be secretive about their purging, it is important that the clinician consider BN as part of a differential diagnosis when faced with such painless parotid gland swellings.

Integrative time-limited group therapy for bulimia nervosa.

This article presents an integrative group therapy model for the treatment of bulimia nervosa (BN) and describes the 12-session format, incorporating components of cognitive-behavioral therapy (CBT), psychoeducation, interpersonal therapy (IPT), and relational therapy (RT), in detail. Previous reports have found CBT, IPT, and RT to be effective approaches for BN when used separately. The integrative approach may have the advantage of achieving symptom reduction by two different mediating mechanisms, those that directly affect eating behaviors and those that address the interpersonal and relati…

Bulimia nervosa: a 5-year follow-up study.

Fifty patients with bulimia nervosa were followed up for a minimum length of 5 years. Standardized interviews, observer-rated instruments and self-rated questionnaires were employed both at entry and at completion of the follow-up period. After tracing, the positive response rate was 72%; one patient had died. At the time of follow-up 31% of those traced had made a complete recovery and 25% still fulfilled diagnostic criteria for bulimia nervosa. Judged on behavioural symptoms 47.2% of responders had a 'good' outcome. Good outcome was associated with fewer social problems, higher social class,…

Sleep Health in Bulimia Nervosa: A Systematic Review.

OBJECTIVE: Research on sleep in bulimia nervosa (BN) is methodologically varied and yields inconsistent findings. This review synthesised BN-sleep studies and examined whether comorbid mood disorders contribute to sleep disturbance. METHOD: PsycINFO, Medline (Ovid), and Scopus were searched for English or French quantitative studies comparing sleep health variables between individuals with diagnosed BN and healthy controls. Extracted data included study design, sample characteristics, sleep and BN measures, and key findings. Methodological quality was evaluated using a validated risk-of-bias …

Bulimia symptoms and anger and aggression among adolescents.

BACKGROUND: Previous research has indicated that anger and aggression may be elevated in adolescents with a bulimia nervosa (BN) diagnosis. However, as yet, little is known about whether bulimia symptoms are linked to anger and aggression in adolescents in the general population. To address this deficit this study aimed to explore the associations between a clinical level of bulimia symptoms (CLBS) and anger, anger rumination and aggression in community-based adolescents, and determine whether gender is important in this context. METHODS: This study was conducted on a representative sample of…

Food insecurity and bulimia nervosa in the United States.

BACKGROUND: Food insecurity occurs when access to food is limited by financial hardship. Yet, paradoxically, food insecurity is associated with overeating, with emerging evidence that it may be related to disordered eating. A recent report found that food insecurity was associated with binge-eating disorder (BED), but it is not yet known whether food insecurity is also associated with bulimia nervosa (BN). METHODS: Participants were 873 respondents recruited online who completed a battery of established measures and were categorized into three study groups: healthy-weight (HW), BED, and BN. H…

Bulimia nervosa in adolescents: prevalence and treatment challenges.

Bulimia nervosa (BN) is a serious psychiatric illness that typically develops during adolescence or young adulthood, rendering adolescents a target for early intervention. Despite the increasing research devoted to the treatment of youth with anorexia nervosa (AN) and adults with BN, there remains a dearth of evidence for treating younger individuals with BN. To date, there have been four published randomized controlled trials comparing psychosocial treatments, leaving significant room to improve treatment outcomes. Family-based treatment is the leading treatment for youth with AN, while cogni…

Achalasia as a complication of bulimia nervosa: A case report.

OBJECTIVE: Oesophageal achalasia is a medical condition characterised by oesophageal aperistalsis, an increased resting pressure with partial or incomplete relaxation of the lower oesophageal sphincter. Bulimia nervosa (BN) is an eating disorder manifested by binge eating attacks followed by recurrent inappropriate compensatory behaviours, such as self-induced vomiting and excessive exercise. Dysphagia, regurgitation, vomiting, retrosternal pain, heartburn, weight loss, avoidance of eating, consumption of large amount of liquids and aberrant eating behaviours are symptoms of both achalasia and…

Oral and Physical Manifestations of Anorexia and Bulimia Nervosa.

Food avoidance and disordered eating behavior are hallmarks of the psychopathologic disorders known as anorexia nervosa and bulimia nervosa. Anorexia nervosa is characterized by severe restriction of food intake, which leads to weight loss and the medical consequences of starvation. Bulimia nervosa is characterized by attempts to curtail food intake, interspersed with binge eating, followed by self- induced vomiting to rid the body of food. Tooth erosion and heightened sensitivity, caused by tooth contact with gastric acid during vomiting, are among several intraoral signs and symptoms of anor…
