A novel vaccine for cervical cancer: quadrivalent human papillomavirus (types 6, 11, 16 and 18) recombinant vaccine (Gardasil).

Human papillomaviruses (HPVs) are one of the most common sexually transmitted infections and remains a public health problem worldwide. There is strong evidence that HPV causes cervical, vulva and vaginal cancers, genital warts and recurrent respiratory papillomatosis. The current treatments for HPV-induced infections are ineffective and recurrence is common-place. Therefore, to reduce the burden of HPV-induced infections, several studies have investigated the effi cacy of different prophylactic vaccines in clinical human trials directed against HPV types 6, 11, 16, or 18. Notably, these HPV t

Vaccination in patients with cancer: Gaps in coverage, barriers, and the role of clinician recommendations.

Cancer patients require a tailored vaccination strategy. This study aims to assess the vaccination status of cancer patients, evaluate disease incidence and severity in vaccinated individuals, identify reasons for noncompliance with guideline-recommended vaccines, and explore vaccine hesitancy. A cross-sectional survey, including 36 multiple-choice and 6 open-ended questions, was conducted in across Turkey. A total of 506 patients participated between September 1, 2024, and December 31, 2024. Among the participants, 59% (n = 302) were receiving active treatment. At the time of diagnosis, 90.5%

Innovations in HPV vaccination and their role in global cervical cancer control: a review.

Cervical cancer remains a primary cause of gynecologic oncology-related mortality globally, with a disproportionate burden in sub-Saharan Africa. The pathogenesis of the disease is fundamentally driven by persistent infection with high-risk human papillomavirus (HPV) genotypes, most notably HPV-16 and HPV-18. The evolution of prophylactic HPV vaccination has transformed preventive oncology, progressing from first-generation bivalent and quadrivalent formulations to the nonavalent vaccine, which extended protection to five additional oncogenic types. These vaccines, composed of L1 virus-like pa

Vaccination Against Respiratory Infections in Adults with Cancer: A Concise Guide for Clinicians.

Global cancer incidence reached 20 million new cases across 185 countries in 2022, with approximately 10 million cancer-related deaths annually. Among adults with solid tumors and hematological malignancies, infections are a major contributor to morbidity and mortality, with respiratory infections playing a particularly significant role. These infections not only reduce life expectancy but can also delay cancer therapy, negatively affect treatment outcomes, and increase healthcare costs. In recent years, the burden of respiratory infections in this population has been driven by influenza virus

Primary Cancer Prevention Through Vaccination: Advances, Challenges, and Global Perspectives.

Oncogenic viral and bacterial infections constitute a significant portion of the global cancer burden, accounting for up to 15% of cases, particularly in low- and middle-income countries. Prophylactic vaccination thus appears to be the most effective tool for preventing these preventable cancers, especially those linked to human papillomavirus (HPV) and hepatitis B virus (HBV). This narrative review synthesizes recent data concerning the impact, advances, limitations, and prospects of available and developing cancer vaccines. Research shows that HPV vaccination significantly reduces the incide…

Human papillomavirus vaccination among childhood cancer survivors: A scoping review.

Childhood cancer survivors are at increased risk for human papillomavirus (HPV) cancers yet are less likely to be vaccinated against HPV than the general population. This scoping review identified studies describing barriers, facilitators, and strategies to improve HPV vaccination in this vulnerable population. We searched PubMed, Cumulative Index to Nursing and Allied Health Literature (CINAHL), and Web of Science to identify full-length studies, published in English, from July 2006 to June 2024. We excluded survivorship studies unrelated to HPV vaccination and non-empirical studies (e.g. com

Flagellins as Vaccine Adjuvants and Cancer Immunotherapy: Recent Advances and Future Prospects.

Flagellin, an essential structural protein of bacterial flagella, has emerged as a potent modulator of both specific and nonspecific immunity, demonstrating significant potential as a vaccine adjuvant and carrier. By inducing the release of pro-inflammatory cytokines like IL-1β, TNF-α, IL-6, IL-8, and IL-12, flagellin activates the innate immune system, enhancing antigen-specific adaptive immune responses mediated by tumour-specific type 1 helper T cells and cytotoxic T cells, thus positioning it as a valuable adjuvant or complementary therapy for various cancers and infectious diseases. This

Neoantigen-Displaying Protein Nanoparticles as a Therapeutic Cancer Vaccine Against Melanoma.

Although interest in peptide-based cancer vaccines has surged in the era of personalized immunotherapy enabled by the discovery of neoantigens, the effective generation of neoantigen-specific T cell responses has been limited. Here, a Brucella BP26 protein-based nanoparticle displaying the MHC class II-restricted melanoma neoantigen, M30, is reported for use as a therapeutic cancer vaccine. Genetic engineering of 10 tandem repeats of the M30 neoepitope to a BP26 monomer results in the self-assembled, neoantigen-displaying protein nanoparticles (BP26-M30 NPs). Subcutaneous immunization of mice …

Neoantigen landscape supports feasibility of personalized cancer vaccine for follicular lymphoma.

Personalized cancer vaccines designed to target neoantigens represent a promising new treatment paradigm in oncology. In contrast to classical idiotype vaccines, we hypothesized that "polyvalent" vaccines could be engineered for the personalized treatment of follicular lymphoma (FL) using neoantigen discovery by combined whole-exome sequencing (WES) and RNA sequencing (RNA-seq). Fifty-eight tumor samples from 57 patients with FL underwent WES and RNA-seq. Somatic and B-cell clonotype neoantigens were predicted and filtered to identify high-quality neoantigens. B-cell clonality was determined b…

[Combined Efficacy of Dendritic Cell Vaccine and Oncolytic Adenovirus in Colorectal Cancer].

Dendritic cell(DC)vaccine therapy has been widely studied as cancer immunotherapy that potently induces cytotoxic T lymphocytes. However, their efficacy in clinical practice has not yet been established. We have developed an oncolytic adenovirus OBP-702 carrying the tumor suppressor gene p53 and have demonstrated its therapeutic potential to induce cytopathic effect and activate antitumor immunity via p53 induction. In this study, we investigated the combined effect of p53-transduced DC vaccine and OBP-702 in colorectal cancer.

The Future Role of Cytology in Cervical Cancer Screening in the Era of HPV Vaccination.

BACKGROUND: Up until recently, cervical cytology was the mainstay for cervical cancer screening. However, the established association between human papillomavirus (HPV) infection and cervical cancer has led to changes in preventive strategies, with cytology being replaced by the use of high-risk HPV (hrHPV) testing and primary prevention being achieved by HPV vaccination. In this context, the role of cervical cytology is shifting to secondary triage of HPV-positive women. As vaccination is leading to decreased HPV infections and significant cervical abnormalities (CIN2+), data on the impact of

Implications of mRNA-based SARS-CoV-2 vaccination for cancer patients.

SARS-CoV-2 infection and the resulting COVID-19 have afflicted millions of people in an ongoing worldwide pandemic. Safe and effective vaccination is needed urgently to protect not only the general population but also vulnerable subjects such as patients with cancer. Currently approved mRNA-based SARS-CoV-2 vaccines seem suitable for patients with cancer based on their mode of action, efficacy, and favorable safety profile reported in the general population. Here, we provide an overview of mRNA-based vaccines including their safety and efficacy. Extrapolating from insights gained from a differ

Novel personalized cancer vaccine platform based on Bacillus Calmette-Guèrin.

BACKGROUND: Intratumoral BCG therapy, one of the earliest immunotherapies, can lead to infiltration of immune cells into a treated tumor. However, an increase in the number of BCG-induced tumor-specific T cells in the tumor microenvironment could lead to enhanced therapeutic effects. METHODS: Here, we have developed a novel cancer vaccine platform based on BCG that can broaden BCG-induced immune responses to include tumor antigens. By physically attaching tumor-specific peptides onto the mycobacterial outer membrane, we were able to induce strong systemic and intratumoral T cell-specific immu

BCG vaccine and COVID-19: implications for infection prophylaxis and cancer immunotherapy.

The COVID-19 pandemic has killed over 400 000 people globally. Ecological evidence indicates that countries with national universal BCG vaccination programs for tuberculosis (TB) prevention have a much lower incidence of severe COVID-19 and mortality compared with those that do not have such programs. BCG is a century old vaccine used for TB prevention via infant/childhood vaccination in lowto middle-income countries with high infection prevalence rate and is known to reduce all-cause neonatal mortality. BCG remains the standard immunotherapy treatment for patients with high-risk non-muscle in

In silico Evaluation of PLAC1-fliC As a Chimeric Vaccine against Breast Cancer.

BACKGROUND: Breast cancer is one of the most prevalent cancers among women. Common cancer treatment methods are not effective enough, and there is a need for a more efficient treatment procedure. Cancer vaccine is a novel immunotherapy method that stimulates humoral and/or cellular immunity against cancer. Placenta-specific protein 1 (PLAC1) is a cancer/testis antigen, prevalent in breast cancer and rarely found in normal tissues. FliC, as a bacterial adjuvant, when fused to PLAC1 can elicit humoral and cellular responses. Therefore, PLAC1-fliC is a chimeric protein, which can be considered a

Vaccine and immune cell therapy in non-small cell lung cancer.

Despite new advances in therapeutics, lung cancer remains the first cause of mortality among different types of malignancies. To improve survival, different strategies have been developed such as chemotherapy combinations, targeted therapies and more recently immunotherapy. Immunotherapy is based on the capability of the immune system to differentiate cancer cells from normal cells to fight against the tumor. The two main checkpoint inhibitors that have been widely studied in non-small cell lung cancer (NSCLC) are PD-1/PD-L1 and CTLA-4. However, interactions between tumor and immune system are

Human Papillomavirus and Its Testing Assays, Cervical Cancer Screening, and Vaccination.

Human papillomavirus (HPV) was found to be the causative agent for cervical cancer in the 1980s with almost 100% of cervical cancer cases testing positive for HPV. Since then, many studies have been conducted to elucidate the molecular basis of HPV, the mechanisms of carcinogenesis of the virus, and the risk factors for HPV infection. Traditionally, the Papanicolaou test was the primary screening method for cervical cancer. Because of the discovery and evolving understanding of the role of HPV in cervical dysplasia, HPV testing has been recommended as a new method for cervical cancer screening

Optimal Cervical Cancer Screening in Women Vaccinated Against Human Papillomavirus.

BACKGROUND: Current US cervical cancer screening guidelines do not differentiate recommendations based on a woman's human papillomavirus (HPV) vaccination status. Changes to cervical cancer screening policies in HPV-vaccinated women should be evaluated. METHODS: We utilized an individual-based mathematical model of HPV and cervical cancer in US women to project the health benefits, costs, and harms associated with screening strategies in women vaccinated with the bivalent, quadrivalent, or nonavalent vaccine. Strategies varied by the primary screening test, including cytology, HPV, and combin
