Atlantoaxial subluxation-induced myelopathy in cleidocranial dysplasia. Case report.

The authors describe the clinical course and treatment of a patient with cleidocranial dysplasia in whom spastic myelopathy developed due to atlantoaxial subluxation. This 27-year-old woman with cleidocranial dysplasia and a history of atlantoaxial subluxation presented with spastic myelopathy. Surgery was performed twice for cervical myelopathy and atlantoaxial subluxation, including laminectomy at the atlas and cervicooccipital fusion in which the Luque rod system was used, as well as C1-2 fusion via the transpharyngeal route. Solid bone fusion was achieved by 7 months postsurgery. Postopera

[Bilateral polyradiculitis after an epidural block: a rare, serious complication].

Permanent neurological complications related to neuraxial techniques are rare. We report a case in which performance of the regional block and the intervention were free of noteworthy incidents. Postoperative pain was controlled by use of multimodal analgesia. The epidural catheter was removed on the ward after 24 hours. However, 72 hours after the intervention, the patient developed lower limb weakness and associated paresthesia mainly on the right side. Examination disclosed flaccid paralysis from the second lumbar vertebra affecting mainly the right lower extremity with no involvement of th

A pilot study of physiotherapy education using videoconferencing.

We investigated the possibility of instructing people in how to assist patients with activities of daily living via videoconferencing. A videoconference system was installed in two rooms to simulate a connection between a home and an institution. The subjects were five first-year university students. First, they saw a video-tape on several general assistance procedures to help a patient with sitting up, standing up and walking with a cane. Then they assisted a simulated patient who reproduced the effects of right hemiplegia due to a stroke. They also received instructions from a physiotherapis

Total hip arthroplasty in juvenile rheumatoid arthritis.

A series of 17 primary total hip arthroplasties in patients with juvenile rheumatoid arthritis (JRA) were performed at a mean age of 18 years with an average follow-up period of 9.3 years. There were 13 cemented prostheses and four noncemented. All patients were satisfied with their hip surgery and reported either slight or no hip pain according to the Harris hip rating system. Ambulation improved postoperatively and all but one patient was at least a limited community ambulator (able to ambulate short distances outside the home with crutches, walker, or cane). Roentgenogram evaluation showed

[Total hip replacement for congenital dislocation of the hip in elderly patients].

From 1986 to 1989, total hip replacement was performed on ten patients (twelve hips) with congenital dislocation of the hip. The average age of the patients was fifty-three years (ranging from forty-eight to sixty years), and the mean follow-up period was twenty-eight months (ranging from six to forty-eight months). Of the ten patients, eight had unilateral replacement and two had bilateral replacement, for a total of twelve arthroplasties. Eight arthroplasties were performed on the right and four on the left. Most patients left the hospital two to three weeks after operation. All the patients

Comparisons of the neurotoxic activity of the venom of several species of funnel web spiders (Atrax).

A bioassay has been developed to allow comparison of the neurotoxic activity of the venom of individuals of the four most common species of funnel web spiders (Atrax species). In the absence of purified Atrax neurotoxin, the stimulatory action of the venom on skeletal muscle from the cane toad, Bufo marinus, was compared with that of known concentrations of acetylcholine. Several experiments to validate the assay were performed, then venom samples from both male and female spiders were assayed. It was found that individual female spiders of all Atrax species normally secreted several times as

Pluripotent Stem Cells as a Preclinical Cellular Model for Studying Hereditary Spastic Paraplegias.

Hereditary spastic paraplegias (HSPs) comprise a family of degenerative diseases mostly hitting descending axons of corticospinal neurons. Depending on the gene and mutation involved, the disease could present as a pure form with limb spasticity, or a complex form associated with cerebellar and/or cortical signs such as ataxia, dysarthria, epilepsy, and intellectual disability. The progressive nature of HSPs invariably leads patients to require walking canes or wheelchairs over time. Despite several attempts to ameliorate the life quality of patients that have been tested, current therapeutica

Amputation versus conservative treatment in severe open lower-limb fracture: A functional and quality-of-life study.

INTRODUCTION: Severe lower-limb trauma is a major event in a patient's life, and treatment is a challenge that has not been sufficiently studied. The main objective of the present study was to assess the difference in disability between amputees and patients who kept their leg after severe open lower-limb fracture. HYPOTHESIS: The study hypothesis was that amputation allows better functional recovery and quality of life, in the same time-frame. MATERIALS AND METHODS: All male and female patients aged over 18 years admitted to one of the trauma centers of Marseille (France) for major lower-li

Enhanced pedestrian walkway object detection using deep learning and pelican optimization algorithm for assisting disabled persons.

Walking is a significant transportation method, but the convenience of pedestrian surroundings for individuals with blindness is highly challenging. Pedestrians with blindness familiarize themselves with guidelines in their surroundings, which might be artificial or natural. To overcome these troubles, it is highly significant for them to perceive the features of an environment. Currently, numerous methods like long white canes and GPS are deployed to improve pedestrian walkways for sightless people. So, they can utilize it as the primary assistive device for recognition and also the vital eco

Gait variability and disability in multiple sclerosis.

Gait variability is clinically relevant in some populations, but there is limited documentation of gait variability in persons with multiple sclerosis (MS). This investigation examined average and variability of spatiotemporal gait parameters in persons with MS and healthy controls and subsequent associations with disability status. 88 individuals with MS (age 52.4±11.1) and 20 healthy controls (age 50.9±8.7) performed two self-paced walking trials on a 7.9-m electronic walkway to determine gait parameters. Disability was indexed by the Expanded Disability Status Scale (EDSS) and ranged betwee

The association between cognition and gait in a representative sample of very old people - the influence of dementia and walking aid use.

BACKGROUND: Cognition has been related with gait speed in older adults; however, studies involving the oldest age group, where many have mobility disability and cognitive impairment, are few. The aim was to investigate the association between global cognitive function and gait speed in a representative sample of very old people, and whether the association was affected by dementia, and walking aid use. METHOD: This cross-sectional study included 1317 participants, mean age 89.4 years, and 68% women, from the Umeå85+/Gerontological Regional Database. Self-paced gait speed was measured over 2.4

Nanocircularity in Agriculture with BioC-Dots: Vinasse-Derived Carbon-Dots and Their Usage as Potential Metal Ion Sensors.

The sugar cane refining industry is crucial in producing sugar and biofuel ethanol. However, as demand for these products has grown, so has the generation of waste, with vinasse emerging as the primary pollutant. This study investigates the potential of utilizing vinasse as a raw material to produce carbon dots (BioC-dots), a high-value product. Application of the synthesized BioC-dots in metal ion detection is yet to be explored. Luminescent nanocarbons (BioC-dots) were successfully synthesized from vinasse using a green synthesis method and characterized through transmission electron microsc

[Involvement of autonomic nervous system since middle age in elderly patient with giant axonal neuropathy caused by novel genetic mutation].

A 69-year-old man began to experience difficulty with walking at the age of 5 years and started use of a cane at around 13 years, then finally started using a wheelchair at 17 years old. A diagnosis of Charcot-Marie-Tooth disease was previously determined at another hospital, though neither peripheral nerve biopsy nor gene analysis was conducted. He visited our institution at the age of 54 years and irregular outpatient examinations were started, which indicated slowly progressive muscle weakness and sensory disturbance of the limbs, leading to a decline in activities of daily living.
