Familial adenomatous polyposis is characterized by the development of multiple (>100) colorectal adenomas throughout the colorectum. This disorder can be caused by a germline mutation in the adenomatous polyposis coli gene and can be diagnosed either clinically or genetically. After diagnosis with the condition, patients should undergo prophylactic proctocolectomy with a neoreservoir, usually an ileoanal pouch, at an appropriate time. Individuals with a family history of this disease who have not been diagnosed should be advised to attend genetic counseling and to enroll in appropriate clinica

Endoscopic haemostasis should be attempted as the initial approach in most cases of gastrointestinal (GI) bleeding, although cross-disciplinary collaboration is a prerequisite. For variceal bleeding, band ligation is the method of choice in the elective setting, although injection therapy still has a role in acute bleeding. Histoacryl remains preferable for fundic varices in most parts of the world. For peptic ulcer bleeds, injection therapy should be combined with at least one 'mechanical' modality, thermal treatment or clipping. In rebleeding, a single endoscopic retreatment can be attempted

OBJECTIVES: Protein-losing enteropathy (PLE) is often difficult to diagnose. We evaluated the diagnostic yields of underlying diseases of PLE among esophagogastroduodenoscopy, colonoscopy, fluoroscopic conventional enteroclysis (FCE), videocapsule endoscopy (VCE), and double-balloon enteroscopy (DBE) and prognosis after treatment.
METHODS: Between June 2003 and August 2010, 25 consecutive patients with PLE confirmed by fecal α1-antitrypsin clearance (n=18) and technetium 99m human serum albumin scintigraphy (n=19) were enrolled, investigated, and treated.
RESULTS: Of 25 patients, 4 (16%) wit

A 76-year-old woman on anticoagulation for atrial fibrillation presented with melaena. Her medical history included previous jejunal resection, aortic aneurysm repair, and cervical cancer in remission. Initial upper gastrointestinal endoscopy and complete colonoscopy showed no source of bleeding. Despite discontinuation of anticoagulation, she developed recurrent gastrointestinal bleeding with severe transfusion-dependent anaemia. Capsule endoscopy demonstrated bluish submucosal vascular lesions in the jejunum, suggestive of Blue Rubber Bleb Nevus Syndrome (BRBNS), a diagnosis further supporte

BACKGROUND: The early diagnosis of small-bowel tumors (SBT) was a difficult task until the advent of capsule endoscopy (CE) and double-balloon endoscopy (DBE) allowed access to the disease site. However, although CE and DBE have greatly simplified the task, DBE studies have yet to provide sufficient data on the diagnosis and outcome of patients with SBTs.
OBJECTIVE: To determine the efficacy of DBE examination in the detection and diagnosis of SBT.
DESIGN: A retrospective analysis of cross-sectional case series.
SETTING: Seven major medical centers in Japan.
PATIENTS: The first 1035 consec

BACKGROUND: Schistosomiasis is a chronic parasitic infection endemic in many countries. Colonic schistosomiasis is a rare entity with no specific clinical manifestations or endoscopic aspects, which delays the diagnosis. Diagnosis is primarily dependent on histopathological analysis, and treatment with antihelminthics typically resolves the infection.
CASE SUMMARY: We present the case of a 21-year-old male who suffered from chronic diarrhea and abdominal pain. Physical examination found no abnormalities, blood tests were normal, and stool examination was negative. A colonoscopy revealed a nod

Despite aggressive medical therapy, many patients with Crohn's disease require surgical intervention over time. After surgical resection, disease recurrence is common. Ileo-colonoscopy and the Rutgeerts score are commonly used for diagnosis and monitoring of post-operative endoscopic recurrence. The latter is the precursor of clinical recurrence and therefore it impacts prognosis and patient management. However, due to the limited length of bowel assessed by ileo-colonoscopy, this procedure can miss out-of-reach, more proximal lesions in the small bowel. This limitation introduces an important

BACKGROUND: To study the performance and efficacy of small bowel capsule endoscopy (SBCE) in a large cohort of patients with surgically altered gastrointestinal anatomy (SAGA).
METHODS: We retrospectively reviewed all SBCE studies between January 2000 and August 2025. Patients with SAGA (prior gastrectomy, gastric bypass, small bowel resections, esophagectomy, or Whipple procedures) were matched 1:2 to patients with native gastrointestinal (GI) anatomy. Primary outcome was SBCE completion rate. Secondary outcomes included diagnostic yield, capsule retention, and GI transit times.
RESULTS: A

INTRODUCTION: Current guidelines suggest swallowable capsule-sponge devices with biomarkers can be used as alternatives to endoscopy for detecting Barrett's Esophagus (BE) in at-risk patients. We analyzed the screening performance of capsule-sponge with Trefoil-Factor-3 (TFF3) in patients with symptomatic acid reflux and assessed test performance across individual risk factors.
METHODS: The study protocol was registered in PROSPERO (CRD420250652091). Eligible studies reported results for both capsule-sponge-TFF3 and endoscopic assessment in adults with reflux symptoms and no history of BE. A

BACKGROUND: This case reports a rare disease in which the clinical symptoms are completely inconsistent with the primary site. As the occurrence of this disease is partly rare, the challenges in the diagnosis of phosphaturic mesenchymal tumors are discussed. It also provides a new clinical treatment option, that is, the combined treatment with anlotinib capsules after surgery and the long-term follow-up to observe the effect of the combined treatment.
CASE PRESENTATION: A 27-year-old Chinese woman presented with generalized pain and multiple fractures for 2 years. The present case was initial

The barium meal with plain X-ray films of the small intestine has for decades been the undisputed gold standard in imaging of the small intestine. More recently, X-rays and fluoroscopy with an overall accuracy of 73% have been replaced by multislice computed tomography (MSCT) or modern magnetic resonance imaging (MRI). Ultrasound is suitable for the orienting investigation of the small intestine in the context of general abdominal sonography as well as for dedicated examinations with a sensitivity of 67-96% and a specificity up to 97%. The endoscopic examinations of the small bowel, such as vi

Lipomas of the gastrointestinal tract are extremely rare, slow-growing and most often asymptomatic tumours. They are composed of highly differentiated fat cells surrounded by a fibrous capsule. Tumours larger than 2 cm can produce symptoms of abdominal pain and discomfort or cause anaemia due to ulceration and bleeding, or intestinal obstruction due to intussusception. This is a case report of a 73-year-old man with a bleeding duodenal lipoma situated at the papilla duodeni major which caused difficulty in the diagnostic process, and the endoscopic visualization raised suspicion of cancer of t

BACKGROUND: Small-bowel capsule endoscopy (CE) is a prime modality for evaluation of the small bowel. The Lewis score (LS) and the Capsule Endoscopy Crohn's Disease Activity Index (CECDAI) are validated endoscopic indices for quantification of small-bowel inflammation on CE. It is unclear whether these indexes are interchangeable for the evaluation of mucosal inflammation in established Crohn's disease (CD). The aim of this study was to compare the quantitative evaluation of small- bowel inflammation by LS and CECDAI.
METHODS: Patients with known quiescent small-bowel CD for at least 3 months

Obscure GI bleeding is a relatively common problem facing internists, gastroenterologists, and surgeons in a typical clinical practice. The etiology is occasionally suggested by the patient's age, history, and medications. Management is complicated and typically requires a team-oriented approach, with input from the internist, gastroenterologist, radiologist, and surgeon alike. SBFT and enteroclysis seem to have a limited role, unless there is a high suspicion of a small bowel mass lesion or Crohn's disease. Scintigraphy may be performed in patients with active bleeding in whom endoscopy has f

Introduction. Gastrointestinal stromal tumor (GIST) in the ileum is an extremely rare cause of recurrent lower gastrointestinal bleeding (GIB). Case Report. An 89-year-old man was admitted with melana. He had extensive PMH of CAD post-CABG/AICD, AAA repair, chronic anemia, myelodysplastic syndrome, lung cancer after resection, and recurrent GIB. Prior EGDs, colonoscopies, and upper device-assisted enteroscopy showed duodenal ulcer, A-V malformation s/p cauterization, and angioectasia. On admission, Hb was 6.0 g/dL. An endoscopic capsule study showed an ulcerated tumor in the ileum. CT showed n

Currently, oesophago-gastroduodenoscopy is the standard method to diagnose the presence of oesophago-gastric varices and to estimate the risk of bleeding. It is recommended that all patients undergo endoscopic screening for varices at the time when cirrhosis is diagnosed. After screening endoscopy, patients with medium or large varices should be treated to prevent bleeding, while all other patients should undergo periodic surveillance endoscopy. However, at a given point in time a variable proportion of patients will not have varices, since the prevalence of varices is variable. Thus, screenin

Angioleiomyoma is a benign smooth muscle and vessel tumor; laryngeal localization is extremely rare with only 24 cases described in the literature; moreover, it should be considered in the differential diagnosis of laryngeal mass. Endoscopic complete surgical excision with dissection along capsule is now considered the gold-standard treatment for small and well-circumscribed laryngeal angioleiomyoma. We present a case of laryngeal angioleiomyoma successfully treated with carbon dioxide laser technology which resulted in a bleeding reduction and adequate hemostasis with less tissue damage and g

Hemosuccus pancreaticus is a rare etiology of obscure gastrointestinal bleeding characterized by bleeding into the pancreatic duct. The diagnosis may be delayed for months to years, due to the episodic nature of bleeding and failure to consider the diagnosis. Patients often undergo multiple endoscopies and radiologic evaluations prior to diagnosis. Incidental gastrointestinal findings may lead to unnecessary endoscopic and surgical interventions. This report describes a patient with hemosuccus pancreaticus diagnosed in the era of video capsule endoscopy and double balloon enteroscopy, whose ma
