Cor triatriatum sinister with an intact interatrial septum and a decompressing vein in a toddler.

Cor triatriatum sinister is a very rare cardiac anomaly that may lead to pulmonary hypertension, right ventricular dilation, and eventually right heart failure. We report a case of a toddler who presented with respiratory distress and cardiomegaly and was found to have cor triatriatum sinister with a restrictive communication, decompressing vertical vein, pulmonary hypertension, severe tricuspid regurgitation, and severe right ventricular dysfunction. She underwent a successful surgical repair, with normalisation of right ventricular function and pulmonary artery pressure.

Pompe's disease or type IIa glycogenosis.

This is the report of a five-month-old child presenting clinical evidence of Pompe's disease: severe hypotonicity, hyporeflexia and congestive heart failure. The ECG showed a short PR interval, the chest radiography disclosed marked cardiomegaly, and the echocardiogram revealed marked left ventricular hypertrophy - the most typical finding of this disease. A skeletal muscle biopsy led to final diagnosis, because in the histopathologic study marked increased glycogen accumulation was evident. Death occurred two months after symptom onset.

[Selenium deficiency and cardiomyopathy in a patient on long-term parenteral nutrition].

A 28 year-old Japanese male with Crohn's disease maintained on total parenteral nutrition for 5 consecutive years showed congestive cardiomyopathy. His symptoms (palpitation and precordial pain), arrhythmia on ECG and cardiomegaly on chest X-ray film were progressive. By intravenous selenium supplementation of 100 micrograms/day (selenite) his symptoms disappeared dramatically and plasma selenium level, erythrocyte selenium level and glutathione peroxidase activity improved to normal ranges. It is suggested that this myocardial damage was caused by selenium deficiency.

The radiological aspects of chest complication in childhood measles.

Chest radiographs of 335 Nigerian children presenting with pulmonary and cardiac complications of measles were studied systematically. Perihilar (88%) and diffuse (69%) pulmonary infiltrates were the commonest findings. Other abnormalities included segmental consolidation (34%), emphysema (20%), cardiomegaly (12%) and atelectasis (10%). Pneumothorax and pleural effusion were seen in 24 (7%) and 14 (4%) children respectively. Eight children (2.5%) had subcutaneous and mediastinal emphysema. In patients with atelectasis and segmental consolidation there was a striking preponderance of right side…

Lysosomal glycogen storage disease with normal acid maltase.

Two unrelated 16-year-old boys had mental retardation, cardiomegaly, and proximal myopathy. One also had hepatomegaly. Histochemistry and electronmicroscopy of muscle biopsies showed lysosomal glycogen storage resembling acid maltase deficiency. Biochemical studies of skeletal muscle showed increased content of glycogen of normal structure; acid alpha-glucosidase activity in both urine and muscle was normal. Other enzymes of glycogen metabolism were also normal. The cause of this apparently generalized glycogenosis with no demonstrable enzyme defect is unknown.

Heart failure in the neonate due to extreme abnormalities of heart rate: clinical and radiographic features.

Extreme variations of heart rate caused clinical and radiographic findings of heart failure in six neonates in the absence of structural heart disease. Two infants had heart block with heart rates below 50/min and four had paroxysmal atrial tachycardia with rates approaching 300/min. Each infant had interstitial or alveolar pulmonary edema and most had cardiomegaly. After restoration of a normal heart rate, there was rapid and dramatic resolution of pulmonary edema and reduction in cardiac size.

Congenital Aneurysm of the Right Atrium: Two Cases Report.

Congenital aneurysm or enlargement of the right atrium is a rare condition. Two children operated on at the age of 14 months and 11 years old for congenital aneurysm of the right atrium are reported. Both presented cardiomegaly and symptoms of paroxysmal supraventricular tachycardia. Diagnosis was established by echocardiography. Surgical resection was successful. Both patients are free of symptoms and their chest X-ray and echocardiogram are normal. The first patient is now in her 17th postoperative year. The patients' evolution suggests that the surgery is a curative procedure.

Premature closure of the foramen ovale secondary to congenital aortic valvular stenosis in a stillborn.

Premature closure of the foramen ovale is a rare and deleterious condition that can occur as an isolated defect or in association with other congenital and cardiovascular anomalies. We report on the pathologic findings in a 22-week stillborn male fetus with premature closure of the foramen ovale, severe aortic valve stenosis, cardiomegaly, intact atrial and ventricular septa, hypoplasia of the ascending aorta, and hypoplastic aortic arch with a preductal coarctation ridge. To the best of our knowledge, this is only the second report on this rare constellation of complex congenital cardiac defe…

Heart failure due to coronary fistulas from the right and left coronary circulation into the right atrium.

A patient with a history of hypertension presented with worsening shortness of breath, fatigue, and productive cough with blood-tinged sputum. A continuous murmur was heard at the lower sternum and chest X-ray showed mild cardiomegaly. A dilated right atrium with moderately elevated pulmonary artery pressures was detected by transthoracic echocardiogram. Stress ECG did not reveal any inducible ischemia. Cardiac catheterization demonstrated an RCA aneurysm with fistula to the right atrium and a circumflex artery fistula to the right atrium, which were corrected by surgery.

[Left atrial mass observed in a patient in Abidjan, Ivory Coast].

The purpose of this report is to present a case involving a tumor-like mass in the left atrium of a 27-year-old woman in Abidjan, Ivory Coast. The mass was discovered by echocardiography carried out after cardiomegaly was detected by roentgenography performed in the context of debilitated general condition, fever and positive serology for HIV1. The patient responded favorably to anti-tuberculosis treatment with complete regression of the mass and improvement of general condition. Based on this outcome, the most likely diagnosis was cardiac tuberculoma, a rare complication of tuberculosis.

Giant coronary artery aneurysm causing superior vena cava syndrome and congestive heart failure.

Coronary artery aneurysms are defined as localized coronary artery dilations >1.5 times the diameter of the adjacent segments. In adults, they are commonly of atherosclerotic origin and are usually asymptomatic. This is the first case report of a coronary artery aneurysm causing superior vena cava syndrome and heart failure. Chest x-ray showed cardiomegaly, and transthoracic echocardiography showed a mass that was confirmed by transesophageal echocardiography, computed tomography, and coronary angiography. In conclusion, giant coronary artery aneurysms can cause superior vena cava syndrome.

Ultrasound-guided interstitial laser therapy for the treatment of placental chorioangioma.

BACKGROUND: Placental chorioangioma is a relatively rare malformation with potential to cause fetal hydrops and even death. We describe ultrasound-guided interstitial laser as a modality for treating chorioangiomas of the placenta. CASE: The patient presented at midgestation with placental chorioangioma resulting in fetal cardiomegaly. Interstitial laser was performed under local anesthesia on two occasions and resulted in arrest of flow through the tumor feeder vessel. CONCLUSION: Interstitial laser therapy in a patient with placental chorioangioma resulted in a successful outcome.

[Mediastinal teratoma in a newborn infant].

A newborn with mediastinal teratoma developed tachypnea and cyanosis within the first hours of life. Chest x-ray revealed a subtotal opacification of the left hemithorax, which at first was suspected to be caused by a cardiomegaly. Ultrasound examination demonstrated a large mass in the left hemithorax with multiple echo-free areas. This finding was suspicious for a mediastinal teratoma. The tumor which was originating from the mediastinum was apple-sized and could be resected totally by thoracotomy. Histology revealed a cystic teratoma covered with normal thymus tissue.

Tuberous sclerosis: a neuropathological and immunohistochemical (PAP) study.

Clinical, neuropathological, and immunohistochemical (PAP) findings are reported in a 7 month-old girl with Tuberous sclerosis (TS). Polycystic kidneys and massive cardiomegaly constituted the prominent pathological features. The neuropathology of the syndrome was characterized by Megalencephaly, Subependymal giant cell astrocytoma, Very large protoplasmic astrocytes in Golgi-rapid method preparations of cortical tubers and Glial fibrillary acidic protein (GFAP)-negative giant cells in the subependymal nodules as well as in the cortical tubers. The identity of these giant cells is discussed.

Diffuse meningocerebral angiodysplasia and renal agenesis: a case report.

A third case of meningocerebral angiodysplasia involving the cerebral cortex, with necrosis of both cortex and white matter is described in a neonate with Potter's syndrome. It is suggested that cortical vessel anomalies act as a local intracerebral shunt to produce periventricular infarction. This disorder differs from pure meningeal angiectasis without cerebral infarction and from the classic arteriovenous malformation of the central nervous system with massive systemic shunt, cardiomegaly, and neonatal cardiac failure. Meningocerebral angiodysplasia and renal agenesis appear to form a rare …

Which is a real valuable screening tool for lung cancer and measure thoracic diseases, chest radiography or low-dose computed tomography?: A review on the current status of Japan and other countries.

Chest radiography (CR) has been used as a screening tool for lung cancer and the use of low-dose computed tomography (LDCT) is not recommend … tions. In the case of cardiovascular disease (CVD), CR may be a better screening tool for detecting cardiomegaly, whereas LDCT may be a more useful tool for detecting vascular changes. Therefore, the current status of thoracic disease screening is that LDCT may be a better screening tool for detecting lung cancer, COPD, and vascular changes. CR may be a suitab …
