National guidelines and the legal standard of care for the management of cauda equina syndrome (CES) patients in primary care have recently changed. Any patient with a history of symptoms of CES must be referred to secondary care for urgent MR imaging (MRI) even in the absence of any objective signs of CES. This article reviews the prevalence and clinical diagnosis of CES, the new national guidelines and management in primary care. The standard of care for doctors, the legal basis of national guidelines and an illustrative judgement are discussed. This new standard of care for GPs will make it…

BACKGROUND: Schwannomatosis is a rare disorder characterized by multiple schwannomas without vestibular schwannomas or other features of neurofibromatosis type 2 (NF2). It commonly presents with neuropathic pain, neurological deficits, and soft tissue tumors but rarely leads to cauda equina syndrome, a serious condition requiring urgent intervention.
MATERIALS AND METHODS: We report a 28-year-old Pakistani female with progressive back pain, lower limb weakness, sensory deficits, bladder and bowel incontinence, and multiple tender swellings, consistent with cauda equina syndrome. Neurological …

INTRODUCTION: Cauda Equina Syndrome (CES) is a surgical emergency and an intimidating medico-legal challenge. Poor clinical specificity leads to a high rate of negative (normal/No-CES) MRI scans, placing additional burden on emergency services. A proportion of patients re-present with CES-like symptoms. The aim of this study was to assess this cohort of patients and identify the incidence, clinical risk factors, comparative MRI-findings and predictive factors for subsequent True-CES.
METHODS: Retrospective review of prospective data of patients aged 18-65 years, who first attended with suspec…

INTRODUCTION: Cauda equina syndrome (CES) following lumbar disc herniation is exceedingly rare in pregnancy and there is limited literature outlining management of CES in pregnancy. There is further limited data addressing the management of periviable pregnancies complicated by CES.
CASE PRESENTATION: A 38-year-old female at 22 weeks gestation presented with worsening lower back pain radiating to the right posterior lower extremity. She was initially managed with conservative therapy, but re-presented with worsening neurologic symptoms, including fasciculations and perineal numbness. Magnetic…

Cauda equina during pregnancy represents a rare entity, with data regarding optimal treatment being very scarce in the pertinent literature. Given the scarcity of current evidence on the topic, this study conducts a systematic review and analysis of existing literature concerning cauda equina syndrome (CES) management in pregnant women. A comprehensive search was performed across multiple databases, yielding 26 level IV peer-reviewed articles that met the inclusion criteria. These studies collectively encompassed 30 pregnant patients with CES, with a mean age of 31.2 years and an average gesta…

INTRODUCTION: Low back pain is common during pregnancy, affecting up to 76% of women. However, symptomatic lumbar disc herniation (LDH) is rare, occurring in about 1 in 10,000 pregnancies, with less than 2% progressing to cauda equina syndrome (CES). The overlap in symptoms between LDH and typical pregnancy-related conditions complicates both diagnosis and management. This review aims to enhance understanding of LDH and CES during pregnancy and provide a comprehensive overview of treatment options.
METHODOLOGY: A systematic review of the PubMed database was conducted following PRISMA guidelin…

Cauda equina syndrome (CES) is a rare and severe type of spinal stenosis, where all the nerves in the lower back suddenly become severely compressed. It is a serious medical emergency, and compression of the nerves in the lower portion of the spinal canal can lead to permanent loss of bowel and bladder control, paraesthesia, and paralysis of the legs if left untreated. Causes of CES include: trauma, spinal stenosis, herniated discs, spinal tumour, cancerous tumour, inflammatory and infectious conditions or due to an accidental medical intervention. CES patients typically present with symptoms …

Achondroplasia is the most common cause of congenital dwarfism and is associated with multiple complications due to impaired skeletal development. Herein, we report a 21-year-old woman with achondroplasia experiencing lower back pain which progressed to lower limb weakness and urinary retention. Magnetic resonance imaging revealed decreased interpedicular distance and severe stenosis of the lumbosacral spinal canal. Cauda equina syndrome was diagnosed and she underwent laminectomy. After decompression, pain, muscle strength and bladder function improved significantly. This case highlights the …

Spondyloptosis at a lumbar level is usually encountered in an isthmic or a dysplastic types of spondylolisthesis. Post-traumatic lumbar spondyloptosis is comparatively a rare entity. As the injury involves a complete failure of all the osteo-ligamentous structures, it is highly unstable and has a high probability of a complete neurological deficit. On the contrary, the injury also has a lot of chances of meaningful neurological recovery if realignment and stabilization are done on an urgent basis. Here we report a case of L4-L5 post-traumatic spondyloptosis with complete motor weakness below t

BACKGROUND: Classical Baastrup's disease is a degenerative disorder of the lumbar spine characterized by the approximation of adjacent spinous processes due to excessive lordosis. This results in edema, sclerosis, cyst, bursitis, and midline epidural fibrosis and is often overlooked as a cause of low back pain. Here, we report a patient with atypical Baastrup's disease and lumbar spinal stenosis who presented with a cauda equina syndrome.
CASE DESCRIPTION: A 67-year-old male presented with low back pain of 1 year's duration. This exacerbated over the past 3 weeks, becoming associated with the…

The patient is a 42-year-old woman with metastatic primary peritoneal carcinoma and known brain metastases, who subsequently developed cauda equina syndrome after presenting with ataxia, lower extremity weakness, and bowel and bladder incontinence secondary to leptomeningeal metastasis after treatment with neoadjuvant chemotherapy, surgical debulking, and adjuvant chemotherapy. Metastases to the central nervous system (CNS) and leptomeninges are rare events in epithelial ovarian and primary peritoneal carcinomas as these tumours do not have a predilection for the CNS. Cauda equina syndrome is …

This group of articles looks at the BASS guidelines for CES. TG and AC gave us the background on the long journey taken in publishing this, SA summarized the forum discussion on the BASS Web site, and NT gave us a medicolegal comment. The guidelines are concise, highlighting the need for prompt MRI scanning and as a consequence emergency surgery in appropriate cases. This has resource implication in terms of MRI availability and a comprehensive spinal on-call system. The question of whether operating "in the small hours" carries increased risk or whether we are using this as an excuse not to g

Cauda equina syndrome (CES) is a rare but serious neurosurgical emergency that can have devastating long-lasting neurologic consequences. Compression of the cauda equina can result in paralysis of bowel and bladder function. Such compression has been considered the only absolute indication for surgery in cases of lumbar disk disease. Therefore, it is extremely important that physicians be aware of the condition so that a surgeon is consulted before neurological damage becomes permanent. This article reports the results of delayed surgical decompression in cases of lumbar disk herniation with C…
