Spinal canal capacity in simulated displacements of the atlantoaxial segment: a skeletal study.

In normal, physiological circumstances there is ample room in the spinal canal to accommodate the spinal cord. Our study aimed to identify the degree of compromise of the spinal canal which could be anticipated in various atlantoaxial pathological states. We examined paired atlas and axis vertebrae using high-definition radiography and simultaneous photography in both normal and simulated pathological orientations in order to measure the resultant dimension of the spinal canal and its percentage occlusion. At the extreme of physiological axial rotation (47 degrees) the spinal canal is reduced

Nonrheumatoid cranial settling.

Cranial settling is a condition in which erosive changes of the atlantal lateral masses result in downward telescoping of the atlas onto the axis body, anterior displacement of the atlantal posterior arch, and subsequent ventral and dorsal cervicomedullary compression. Cranial settling is described in conjunction with rheumatoid basilar invagination and atlanto-axial instability, possibly representing the most life-threatening abnormality associated with rheumatoid arthritis. The authors describe a case of symptomatic cranial settling in a nonrheumatoid patient with chronic, severe, spasmodic

Factors influencing the outcome of cervical spine injuries.

Factors such as cause of the trauma, concomitant injuries, and cervical sagittal spinal canal size were compared between survivors and nonsurvivors of cervical spine injuries. Traffic accidents were more commonly the cause of the trauma among nonsurvivors (76%) than among survivors (50%). Concomitant injuries were of a more serious degree among nonsurvivors than in survivors. Cervical sagittal spinal canal size was measured at the atlas level. Patients without any neurologic deficits had significantly (p less than 0.001) wider canals (24.1 mm) than fatalities (20.7 mm). The energy level at the

[Surgical stabilization of C1 and C2 fractures].

We present operative procedures for the stabilization of fractures of C1 and C2. As is the case for fractures of the extremities, a direct approach to the injury should be attempted in order to avoid the inclusion of intact segments in the spondylodesis. The aim of the three presented techniques - direct screw fixation of fracture of the dens axis and traumatic lysis of C2 - is to active anatomical reconstruction by optimal stabilization. The new technique of transarticular screw fixation of C1-2 developed by Magerl allows unisegmental three-dimensional stabilization in cases in which at least

Lateral subluxation of the atlanto-axial joint in rheumatoid arthritis.

Involvement of the cervical spine by rheumatoid disease is common, but lateral subluxation at the atlanto-axial level has not been recorded previously. The condition is due to asymmetrical erosion of the lateral atlanto-axial facet joint, and may be complicated by collapse of the lateral mass of the axis. The condition should be suspected in patients with rheumatoid arthritis (RA) who present with occipital, auricular, and/or facial pain.

Mandible and tongue-splitting approach for giant cell tumor of axis.

In two cases with a giant cell tumor of the axis, the mandible and tongue-splitting approach permitted excision of the tumor and anterior fusion of the spine from the atlas to the third cervical vertebra. With this approach, there is no need to extend or rotate the cervical spine during the operation. In addition, the operative field extends from the clivus to the fourth cervical vertebra, and safe and sufficient anterior decompression is possible. Although this approach has some disadvantages, contemporary techniques of intravenous hyperalimentation and postoperative respiratory management pr

Upper spine morphology in hypophosphatemic rickets and healthy controls: a radiographic study.

BACKGROUND/OBJECTIVES: The aim of this study was to describe upper spine morphology in adult patients with hypophosphatemic rickets (HR) compared with controls to assess differences in spine morphology in terms of severity of skeletal impact and to study associations between spine morphology and craniofacial morphology. MATERIAL/METHODS: The study population comprised 36 HR patients and 49 controls. The atlas and axis dimensions were measured on cephalograms, and the differences between the groups were estimated by regression analysis. The upper spine morphology was visually assessed to estim

Cervical spine synostosis: an anatomical study with emphasis on embryological and clinical aspects.

PURPOSE: The objectives were to study the anatomical features of fused vertebra in the cervical region. MATERIALS AND METHODS: The study included 363 cervical vertebras which were macroscopically observed for the fusion and their morphology was studied. RESULTS: Among our specimens, one fusion was observed between the C1 and C2 and the other was between C2 and C3. The mean anteroposterior diameter of normal atlas and axis were 42.1 +/- 0.9 mm and 46 +/- 0.5 mm respectively. The fused C1-C2 had 39 +/- 0.6 mm anteroposterior diameter. The mean lengths of anterior arch of normal atlas and body

MRI-based Quantification of Ossification Centers in the Human Fetal Atlas and Axis Vertebrae.

BACKGROUND: Although fetal MRI has been increasingly widely used in clinical and research settings, quantitative studies specifically targeting the ossification centers of the atlas and axis remain scarce. This study aims to quantitatively assess the ossification centers of the atlas and axis in fetuses using Magnetic Resonance Imaging (MRI) and establish standardized reference data for prenatal evaluation. MATERIALS AND METHODS: This study included 41 human fetuses (24 males and 17 females) at 17 to 42 weeks of gestation, collected after spontaneous abortion or preterm birth that met ethical

Atlas fractures.

Fractures of the atlas account for 1-2 % of all vertebral fractures. We divide atlas fractures into 5 groups: isolated fractures of the anterior arch of the atlas, isolated fractures of the posterior arch, combined fractures of the anterior and posterior arch (so-called Jefferson fractures), isolated fractures of the lateral mass and fractures of the transverse process. Isolated fractures of the anterior or posterior arch are benign and are treated conservatively with a soft collar until the neck pain has disappeared. Jefferson fractures are divided into stable and unstable fracture depending

Unrecognized c1 lateral mass fracture without instability; the origin of posterior neck pain.

Posterior neck pain is a common complaint of patients in the pain clinic. The atlas (C1) burst fracture is known to be a cause of posterior neck pain and instability. Although the atlas burst fracture and instability can be discovered by plain X-rays which show lateral mass displacement or widening of the atlantodental interval, assessment of an atlas burst fracture can be difficult if there is no instability in the imaging study. Here we report a case of a 46-year-old female patient who had complained of sustained posterior neck pain for 6 months. Plain X-rays showed only disc space narrowing

Unstable C1 Fracture Managed with Internal Fixation Using Lateral Mass Screws: A Case Report.

CASE: A 47-year-old woman presented with an unstable C1 fracture after falling down several stairs. She was found to have a sagittal split fracture of the right C1 lateral mass extending into the posterior arch. The fracture was treated with a direct posterior osteosynthesis of C1 using lateral mass screws. CONCLUSION: Surgical management of unstable C1 fractures has traditionally involved posterior fusion of C1 to C2 or fusion from the occiput to C2. These fusion procedures can be quite functionally limiting. Recently, direct osteosynthesis of C1 has been shown to be an effective, motion-pre
