Arthroscopic Pectoralis Minor Release After Reverse Shoulder Arthroplasty: A Novel Consideration for Postarthroplasty Pain.

CASE: A 74-year-old male patient presenting with chronic radiating shoulder pain, paresthesias, and weakness had previously undergone reverse shoulder arthroplasty and anterior cervical discectomy and fusion for an irreparable cuff tear and cervical radiculopathy, respectively. After being diagnosed with neurogenic thoracic outlet syndrome and undergoing physiotherapy, the patient's recalcitrant condition was surgically managed with arthroscopic pectoralis minor tenotomy, suprascapular nerve release, and brachial plexus neurolysis. CONCLUSION: This ultimately led to complete pain relief and i

[Primary reconstructive operation in surgical treatment of perinatal brachial plexus palsy in children].

Perinatal brachial plexus palsy (PBPP) constitutes a serious medical problem. PBPP treatment consists in conservative and operative procedur … n root avulsion from the spinal medulla. In such cases the avulsed roots are connected to nerves of cervical plexus, applying the method of neurotization. The reason of numerous controversies around the choice of treatment method for PBPP patients is the desire to balance procedures aiming at the proper functioning of the patient in future.

True neurogenic thoracic outlet syndrome: electrophysiological diagnosis in six cases.

We report 6 patients with true neurogenic thoracic outlet syndrome. All were female and presented unilateral severe atrophy of the thenar muscles. Pain in the affected upper limb was frequently reported, but sensory deficit was slight or absent. Reduced amplitude of ulnar and median compound muscle action potential associated with a normal amplitude of median sensory nerve action potential (SNAP) and a reduced amplitude of ulnar SNAP was indicative of a chronic axon loss in the lower trunk of the brachial plexus. The absence of the medial antebrachial cutaneous SNAP in 5 patients and a reducti

New Diagnosis of Obstructive Sleep Apnea Following Anterior Cervical Discectomy Complicated by Rheumatoid Arthritis: A Case Report.

Obstructive sleep apnea (OSA) is the most common variant of sleep-disordered breathing that often goes undiagnosed. OSA is characterized mainly by anatomical obstruction or partial collapse of upper airways during sleep. The obstruction is multifactorial, and a lesser-known fact is that damage to the pharyngeal plexus during head and neck procedures or placement of hardware in the cervical area can lead to narrowing or collapse of the upper airway. We present such a case of a 59-year-old female who developed new-onset OSA after undergoing anterior cervical discectomy and fusion (ACDF). The sev

Thoracic outlet syndrome: a rare case with bilateral cervical ribs and bilateral anterior scalene hypertrophy.

Thoracic outlet syndrome (TOS) is a rare neurovascular disorder generally caused by the presence of a cervical rib or hypertrophic scalene anterior muscle that can compress the brachial plexus and/or subclavian vessels. In the vascular form, the symptoms are caused by the compression of the artery and/or the subclavian vein. In the first case, the compression is caused by the cervical rib and leads to hypo-perfusion with cooling and cyanosis of the upper limb, while in the second case, the compression is caused by the anterior scalene muscle and leads to congestion, cyanosis, swelling and pain

Spinal and paraspinal giant cervical cavernous malformation with postpartum presentation.

Cavernous malformations (CMs) are found throughout the CNS but are relatively uncommon in the spine. In this report, the authors describe a giant CM with the imaging appearance of an aggressive, invasive, expansive tumor in the cervical spine. The intradural extramedullary portion of the tumor originated from a cervical nerve root; histologically, the lesion was identified as an intraneural CM. Most of the tumor extended into the paraspinal tissues. The tumor was also epidural, intraosseous, and osteolytic and had completely encased cervical nerve roots, peripheral nerves, branches of the brac

Cervicovertebral phlebography: pathological results.

Cervical phlebography via the femoral vein approach was done in patients with spinal tumors, cervical radiculopathies, and myelopathies. Tumors originating from or extending into the spinal canal readily compress and obstruct the epidural venous plexus. Because of the lateral position of the epidural veins, the foraminal veins are deformed when cervical roots are compressed in the intervertebral foramina by spondylosis of the uncinate processes or lateral disk herniation. In congenitally narrow canals without significant spondylotic bars, the epidural veins were not opacified. Nonfilling of ve

Neuroendoscopy-Assisted Modified Anterior Approach for Contralateral Cervical 7 Nerve Transfer: A Cadaveric Study.

BACKGROUND: This study aims to investigate a safer and more minimally invasive method for transferring the contralateral C7 nerve in the treatment of central spastic paralysis of the upper limb, while also providing anatomic data to support this approach. METHODS: Eight anatomic specimens from the head and neck were utilized: 4 dry specimens were used to measure anatomic data, whereas the other 4 fresh specimens were used to simulate the transposition of the contralateral C7 nerve for observing bilateral nerve anastomosis. Relevant anatomic landmarks and their surrounding relationships were e

Anastomosis between the transverse cervical nerve and marginal mandibular nerve: how often does it occur?

Anastomoses in the neck between the sensory transverse cervical nerve (C2,3) and the cervical branch of the facial nerve are common, but communications with more superior branches of the facial nerve are rare. After we had identified a case where the transverse cervical nerve joined the marginal mandibular branch of the facial nerve in the submandibular triangle during a selective neck dissection, we searched for this variant in 86 neck dissections to see if it occurs more often than expected. We found it in one further patient. As this anastomosis of the nerve could easily be confused with th

Neuroparalytic keratitis and the effect of cervical sympathectomy following operative procedures for trigeminal neuralgia.

A study of 248 patients who underwent operative procedures for trigeminal neuralgia over a nineteen year period is presented, with particular reference to those who developed neuroparalytic keratitis to see if cervical sympathectomy had any protective effect on the cornea. 51 patients underwent cervical sympathectomy, 46 as a prophylactic procedure, and 5 as a treatment for neuroparalytic keratitis. Of these patients 3 (5.9%) developed neuroparalytic keratitis, but 2 of them had had an inadequate sympathectomy. Forty seven patients who did not undergo cervical sympathectomy had either an anaes

Compromising abnormalities of the brachial plexus as displayed by magnetic resonance imaging.

Magnetic resonance images (MRI) of brachial plexus anatomy bilaterally, not possible by plain radiographs or CT, were presented to the Vascular Surgery, Neurology, and the Neurosurgery departments. Patients were requested for MRI of their brachial plexus. They were referred for imaging and the imaging results were presented to the faculty and housestaff. Our technique was accepted and adopted to begin referrals for MRI evaluation of brachial plexopathy. Over 175 patients have been studied. Eighty-five patients were imaged with the 1.5 Tesla magnet (Signa; General Electric Medical Systems, Milw

Severe Transient Pharyngeal Paralysis Following C2 Fracture Repair.

OBJECTIVES: We report a case of isolated pharyngeal plexus injury following posterior plating of a C2 fracture in an 84-year-old white male. METHODS: Methods include a case report with literature review. RESULTS: The patient presented with dense pharyngeal paralysis and inability to initiate swallowing but maintained true vocal cord movement. The patient required gastrostomy tube feeding for 3 months but eventually recovered his swallowing function. Prior literature on acute pharyngeal paralysis following upper cervical spine fracture repairs is reviewed with recommendations for care of this

The supraclavicular nerve.

INTRODUCTION/AIMS: The aim of this study was to describe a new method for studying the supraclavicular nerve (SCN) conduction and to report four cases with SCN lesions. METHODS: The SCN was antidromically recorded with a pair of self-adhesive electrodes located in the middle of the clavicle. Stimulation (<5 mA) was delivered 7 cm proximally with a bar electrode. To facilitate recording, it was explained to the participant that they would feel a very faint electrical sensation locally and an electrical tingle upward (ear) or downward (shoulder/clavicle). Each participant was asked to say when

An Update on the Diagnosis, Treatment, and Management of Occipital Neuralgia.

This report intends to summarize the underlying pathophysiology, relevant symptoms, appropriate diagnostic workup, necessary imaging, and medical and surgical treatments of occipital neuralgia (ON). This was done through a comprehensive literature review of peer-reviewed literature throughout the most relevant databases. The current understanding of ON is that it causes neuropathic pain in the distribution of the greater occipital nerve, the lesser occipital nerve, the third occipital nerve or a combination of the 3. It is currently a subset of headaches although there is some debate if ON sho

A comprehensive review of the great auricular nerve graft.

The great auricular nerve (GAN) is a superficial branch of the cervical plexus that innervates parts of the mandible, auricle, and earlobe. Over the past 30 years, the GAN has become the nerve graft donor of choice for many surgeons for reconstructing injured facial nerves. In this review, we discuss the anatomy and function of the GAN, while focusing on surgical landmarks and the characteristics that make it a suitable nerve graft donor. In addition, we present and summarize published case reports on use of the GAN for grafting. We hope that this review will provide surgeons with an up-to-dat…
