[The thoracic outlet: true syndromes, disputed syndrome (TOS, thoracic outlet syndrome). Current status 1991].

There are 5 syndromes involving the thoracic outlet. The first four, although not well known, especially the first two, are authentic; they are: 1) arterial, due to a well formed cervical rib or to an incompletely formed first rib; 2) neurological, related to the fibrous band associated with a rudimentary cervical rib or a giant transverse process of C7; 3) venous, namely "effort thrombosis"; 4) late post-traumatic, secondary to a fracture of the clavicle. The study of these four syndromes prepares the reader to that of the controversial fifth syndrome, which is entirely subjective, made only …

Radiologic signs of the 4p- (Wolf) syndrome.

The improved Giemsa-trypsin banding technique was used to differentiate the 4p- (Wolf) from the 5p- (cri du chat) syndrome. New radiographic findings of polydactyly, cervical ribs, and fusion defects of the elbows, ribs, and spine are presented in a girl with 4p- syndrome with 45 chromosomes and a B4/G22 translocation [45, XX, -4, -22, +t(4p22q)].

Klippel-Feil syndrome in congenital scoliosis.

STUDY DESIGN: Retrospective review. OBJECTIVE: To investigate the clinical manifestations and radiological characteristics of Klippel-Feil syndrome (KFS) in congenital scoliosis (CS). To identify the incidence of spinal or extraspinal abnormities in KFS. SUMMARY OF BACKGROUND DATA: KFS is an uncommon condition, characterized as improper segmentation of one or more cervical spine segments. "Scoliosis" is potentially the most common manifestation associated with KFS. However, the clinical manifestations and radiological characteristics of KFS in patients with CS are less reported. METHODS: A

Injuries significantly associated with thoracic spine fractures: a case-control study.

OBJECTIVE: To determine injuries significantly associated with traumatic thoracic spine (T-spine) fractures METHODS: This was a case-control study undertaken in an adult trauma centre. Cases were patients admitted with a traumatic T-spine fracture between January 1999 and August 2007. Each case had two controls matched for sex, age (+/-5 years) and injury severity classification (major/minor). Data were collected from patient medical records and the trauma service database. Multivariate logistic regression was used to determine injuries significantly associated with T-spine fracture. RESULTS

Recurrent thoracic outlet syndrome after first rib resection.

After initial relief of severe thoracic outlet syndrome by transaxillary resection of the first rib, a few patients may gradually develop recurrent neurologic symptoms in the neck, shoulder, arm and hand caused by postoperative scar tissue entrapment of segments of the brachial plexus. Once they begin, these symptoms may relentlessly progress to the extent they become unresponsive to all forms of conservative management. Two distinct patterns of recurrent TOS, which may occur separately or simultaneously, have been identified. Clinical measures that have proved effective in establishing the di

Critical upper limb ischemia caused by initially undiagnosed thoracic outlet syndrome--case report.

A case of a progressive ischemia of the right upper extremity in a 57-year-old male caused by primarily undiagnosed compression of a subclavian artery by an accessory cervical rib is presented. Critical limb ischemia persisting for 7 years despite conservative treatment and a thrombectomy of upper extremity arteries and thoracic sympathectomy, led to a loss of three fingers and development of a non-healing ulceration of right upper extremity. Eventually a cervical rib resection together with a subclavio-brachial venous bypass graft permanently reversed the critical limb ischemia and allowed th…

[Rare disorders in neck surgery].

The diagnosis and treatment of unusual surgical conditions of the neck in the newborn, infancy, and paediatric age-group are discussed. The median cervical cleft, the scalenus cervical rib syndrome, the cervical teratoma, changes of the thymus, tumour-like abnormalities of the large veins, calculus formation of the sub-mandibular salivary gland, severe traumatic and iatrogenic injury may all present difficult diagnostic and therapeutic problems.

[Paroxysmal hemicrania. Post-stenotic dilatation of the subclavian artery].

A 50 year-old women presented with a history of increasingly frequent bouts of cervicofacial pain over the last 18 months, and a pulsatile tumor in the supraclavicular region. Angiography showed a stenosis of the left subclavian artery with post-stenotic dilatation related to a cervical rib. No further pain was experienced after removal of the rib. The probable rate of the autonomic nervous system in the physiopathology of this painful syndrome is discussed.

Spinal fractures and/or spinal cord injuries are associated with orthopedic and internal organ injuries in proximity to the spinal injury.

BACKGROUND: the demographics, mechanisms of injury, and concurrent injuries associated with cervical, thoracic and lumbar spinal fracture and/or spinal cord injury remain poorly characterized. METHODS: Patients aged 18 and older with spinal injury between 2011 and 2015 in the National Trauma Data Bank (NTDB) were identified. Patient demographics, comorbidity burden, mechanism of injury, and associated injuries were analyzed. RESULTS: in total, 520,183 patients with acute spinal injury were identified including 216,522 cervical, 191,218 thoracic, and 220,294 lumbar. The age distributions were

Severe kyphoscoliosis observed in a human skeleton from the Edo period.

A severe case of kyphoscoliosis was observed in the skeletal remains of a male from the late Edo period in Japan. This individual exhibited left curvature thoracic scoliosis and kyphotic deformity, with ankylosis of the fifth to twelfth thoracic vertebrae at the intervertebral joints. The Cobb angle was 77° and the kyphotic angle was 76°. The superior articular facet of the cervical vertebrae was lower on the left side than the right side. Additionally, the sacral base was inclined to the right, resulting in the superior articular process and facet being lower on the right than on the left. Th

[Neurovascular compression syndrome of the upper thoracic outlet].

UNLABELLED: Neurovascular compression syndrome of the upper thoracic outlet is a collective term encompassing various entities: cervical ribs, structural variations of the first rib, costoclavicular syndrome, fibromuscular anomalies of anterior and middle scalene muscles, pectoralis minor syndrome and various pathologies of subclavian vessels. Their combination is based on the common substrate subject to compression (neurovascular bundle passing through narrow cervicoaxillary canal). OBJECTIVE: To analyze research of etiology, pathogenesis and treatment of neurovascular compression syndrome o…

Incidental imaging findings of congenital rib abnormalities - a case series and review of developmental concepts.

BACKGROUND: Congenital rib abnormalities are found in approximately 2% of the general population. Usually, they occur in isolation and are rarely symptomatic, but they can also be associated with other malformations. MATERIALS AND METHODS: We reviewed imaging examinations performed over a period of 2 years (2014-2015), enabling us to identify isolated rib abnormalities in 6 adult patients. RESULTS: The case series consisted in 3 cases with bilateral cervical ribs and 1 case each with bifid rib, costal fusion and rib pseudarthrosis. In all patients, the costal anomalies were discovered incide…

Prenatal ossification as an indicator of exposure to toxic agents.

Following exposure to bromodeoxyuridine (BUDR), acetazolamide (ACZM), trypan blue (TRBL), cortisone (CORT), or diphenylhydantoin (DPH), alizarin-stained, cleared fetuses were examined at 18 days postcoitus for unossified cervical vertebral centra; number of ossified caudal vertebrae; number of ribs; and ossification of sternebrae, metatarsals, metacarpals, and phalangeal rows. At all teratogenic doses, in no vehicle-treated groups, and rarely in lower-dose groups, there were significant increases in frequency of unossified cervical centra, the first vertebra (C1) being most often affected, and

Surgical management of a floating shoulder with multiple ribs, cervico-dorsal spine fracture and lung injury: A case report.

Floating shoulder associated with multiple rib fractures with cervical dorsal spine fractures and lung injury has not been reported in the literature. Patient underwent single stage elective clavicle and scapula fracture stabilization. Good functional outcome at 6 months postoperative with near normal range of movements. Surgery allows early mobilization and better functional outcome. A CT scan of thorax and spine is highly recommended in such high velocity shoulder injuries to detect associated rib, spinal fractures and lung injury.

Type III Klippel-Feil syndrome: case report and review of associated craniofacial anomalies.

Klippel-Feil syndrome (KFS) is a complex syndrome of osseous and visceral anomalies that include the classical clinical triad of short neck, limitation of head and neck movements and low posterior hairline. It may also be associated with anomalies of the genitourinary, musculoskeletal, neurologic and cardiac systems. We report a case of type III KFS with associated rib anomalies such as cervical rib, fusion and bifid ribs, scoliosis and fused crossed renal ectopia. The aim of this paper was to summarize all craniofacial anomalies that occur in association with KFS, so that clinicians would be …

[The costoclavicular neurovascular compression syndrome].

This is a description of a great variety of abnormal compressions of nerve, arterial, and rather seldom venous structures at the base of the neck or thoracic outlet, such as supernumerary cervical rib. abnormal ligaments, musculus scalenus anterior hypertrophy and positional alterations, interfering with the normal inter-reactions between first rib and structures overlying it. With a special reference to the hypothetical mechanism of the symptomatology a wide range of denominations of this particular syndrome have been adopted, namely: scalenus anticus syndrome, cervical rib syndrome, costocla…

Axillary-subclavian venous thrombosis.

From 1974 through 1984 six patients with axillary-subclavian venous thrombosis were studied and treated. This disorder represents 1% of the various types of venous thrombosis reported in the literature. Symptoms appeared following effort in four patients, while a cervical rib and a scar bridle after radical mastectomy were identified as the underlying cause in two patients. Ages ranged from 17 to 59 years, with a mean age of 33 and equal sex distribution. None of the six patients had blood clotting defects. The diagnosis was based on clinical evaluation and was confirmed by phlebography. There…
