Cavitary choroidal melanoma.

CASE REPORT: We report an unusual case of cavitary choroidal melanoma. The results of ultrasonography, magnetic resonance imaging, computed tomography, and immunohistochemical studies are presented for a 38-year-old woman who developed an amelanotic tumor in the posterior choroid. B-scan ultrasonography disclosed intratumoral cavitations. Systemic and extraocular extension studies were negative. Enucleation was performed and histopathologic examination showed a choroidal melanoma of spindle cell type, with intratumoral cavitations lined by flattened tumor cells. COMMENTS: The majority of prev…

Polypoidal choroidal vasculopathy.

BACKGROUND: Polypoidal choroidal vasculopathy (PCV) is a unique form of occult choroidal neovascular membrane. The clinical presentation and angiographic findings distinguish idiopathic PCV from other known neovascular and choroidal degenerative disorders. The characteristic morphology of PCV includes the presence of a branching network of inner choroidal vessels with terminal aneurysmal dilations. Other key findings include subtle nodular protrusions, which may precede multiple serosanguineous retinal pigment epithelial detachments. If Bruch's membrane is compromised, there is an additional r…

Polypoidal choroidal vasculopathy.

Polypoidal choroidal vasculopathy was first described as a peculiar hemorrhagic disorder of the macula, characterized by recurrent sub-retinal and sub-retinal pigment epithelium bleeding in middle aged black women. The use of indocyanine green angiography and subsequently of optical coherent tomography has widened our ability to study and understand the pathophysiology of this disorder. The primary abnormality involves the choroidal circulation, and the characteristic lesion is an inner choroidal vascular network of vessels ending in an aneurysmal bulge or outward projection, visible clinicall…

[Brachytherapy in choroidal melanoma].

BACKGROUND AND PURPOSE: Brachyterapy in the management of intraocular tumors was introduced in the Department of Ophthalmology in Cracow in the late sixties, soon after it was applied by Stallard. For many years, similar to other oncological centers, we used 60Co and in 1994 we started with 106Ru and at the end of 1997 with 125J. The aim of this paper is to present our experiences with 106Ru and 125J brachytheray in posterior uveal melanoma. MATERIAL AND METHODS: Our studies comprised 164 patients with choroidal melanoma, treated in the years 1995-1997 with 106Ru and 19 patients treated betwe…

[Pseudotumoral tuberculous choroiditis].

It is presented a case of a secondary tumoral retinal detachment that clinical examination and all paraclinical investigation go for a choroidal malignant melanoma. The anatomopathological examination of the enucleated eyeball has surprisingly put in evidence some choroidal consumptive lesions.

Circumscribed choroidal hemangioma.

BACKGROUND: Circumscribed or solitary choroidal hemangioma is a relatively rare, benign vascular tumor. Because its appearance may mimic more serious intraocular lesions, eye care practitioners should be familiar with differential diagnosis and treatment options. METHODS: A patient presented with an asymptomatic, elevated, red-orange lesion above the macula in the right eye and a history of longstanding unexplained visual acuity loss in the eye. Intraocular lesions such as amelanotic melanoma, metastatic tumor, choroidal osteoma, disciform scar, serous detachment, and central serous chorioret…

Multifocal inner choroiditis.

Young, moderately myopic women presenting with typical signs of ocular histoplasmosis (OHS) but negative serology or skin tests were delimited in 1984 as a new entity of "punctate inner choroidopathy". On the basis of observations in 17 patients combined with similar cases reported in the literature, the following hypothesis is proposed herein. Myopia, young age and female sex are general high-risk factors for the development of focal choroidopathy. A hypothesis is presented to explain this accumulation. Women have a significantly higher risk of developing bacteremia before the age of 50 years…

Expulsive choroidal haemorrhage.

Expulsive choroidal haemorrhage is a dramatic and serious complication of cataract surgery that occurred in five patients out of ten thousand consecutive cataract surgeries performed by the author during the year 1989 and 1990. Report about this dreaded complication after cataract surgery are scanty and as far as I can remember I have not seen any report in Indian ophthalmic literature recently. Since cataract surgery forms the major part of intra ocular surgeries performed in our country, I thought it would be appropriate to report about this rare complication which may occur to all of us. Ou…

Nocardia choroidal abscess.

Nocardia is a Gram positive, aerobic, filamentous branching micro-organism that rarely causes human infection. When infection does occur it usually takes the form of a subcutaneous abscess or a pneumonia-like illness. We describe a case of a patient with chronic lymphocytic leukaemia who developed painless loss of vision in the right eye secondary to a choroidal abscess after a prolonged course of treatment on several immunosuppressive agents. The patient also complained of right shoulder pain that was unresponsive to conventional therapy, and had been admitted and treated for several episodes…

Radiotherapy of choroidal metastases.

With binocular indirect ophthalmoscopy, fluorescin angiography and ultrasonography 68 choroidal metastases in 52 eyes of 39 patients were diagnosed. The primary tumors were mainly breast cancer (81%) and lung cancer (10%). After radiation treatment the visual acuity improved in 17 eyes (38%), stabilized in 15 eyes (33%), whereas in 13 eyes (29%) deterioration could not be prevented (seven eyes unknown). Regression of the lesions or its accompanying secondary retinal detachment was seen in 78% of the eyes treated. Acute transient side effects of radiation therapy were kerato-conjunctivitis (nin…

Circinate choroidal degeneration.

We present a patient whose fundus lesions were bilateral, symmetrical grayish zones with pigments surrounding the macular region. She showed good visual acuity, normal color vision, mild deterioration of scotopic electroretinographic response, normal photopic electroretinographic response, subnormal electrooculogram and paracentral ring scotoma. Fluorescein angiography revealed regional atrophy of the choriocapillaris. The diagnosis of circinate choroidal degeneration was made. We believe that pericentral pigmentary retinal degeneration and circinate choroidal degeneration are distinct entitie…

Choroidal ganglioneuroma in neurofibromatosis.

A twenty-one year old female with previously unsuspected neurofibromatosis presented for evaluation of a blind painful eye. Histopathologic examination of the enucleated specimen revealed choroidal thickening with ovoid bodies and proliferation of connective tissue with pigment-containing cells and ganglion-like cells. Electron microscopic study of the latter cell population revealed typical morphologic features of ganglion cells, including numerous electron-dense intracellular granules and an abundance of mitochondria and rough endoplasmic reticulum. These pathologic findings were interpreted…

Amelanotic choroidal nevi.

A review of 373 patients with presumed choroidal nevi revealed 19 (5.1%) to have largely or totally amelanotic nevi. All patients but one with amelanotic nevi were white, and all were 48 years of age or older. Each lesion was located posterior to the equator, had a mean diameter of 3.2 mm, and was less than or equal to 1 mm in elevation. Intravenous fluorescein angiography most often disclosed early and late hyperfluorescence, corresponding to the amelanotic areas of the nevus. No lesions were observed to grow during a mean 14-month follow-up period. The differential diagnosis of amelanotic ne…

Cryopexy of a choroidal melanoma.

Transconjunctival cryopexy was applied to a malignant melanoma of the ciliary body and choroid of a 46-year-old white man. The tumor immediately became gray-white and pigment cells were liberated in the anterior chamber. Subsequently, an enucleation was performed which showed a localized area of cryodestruction of the melanoma which was lined by macrophages. The significance of this finding in implications for the use of cryopexy in the management of melanomas are discussed.
