Stomatitis in the bone marrow transplant patient. An overview and proposed oral care protocol.

Bone marrow transplantation (BMT) is a treatment modality associated with a wide spectrum of gastrointestinal complications. One effect is painful stomatitis. Stomatitis is defined as an inflammatory and ulcerative reaction of the oral cavity. Stomatitis in the transplant patient is attributable to one, several, or all of the following: effects of high-dose chemotherapy and/or radiotherapy delivered pretransplant; infection; acute or chronic graft-versus-host disease (in the allogeneic transplant recipient); the condition of the oral cavity pretransplant; and side effects of other medications

Up-front combination therapy in pulmonary arterial hypertension: from clinical trials to 'real world' observational studies.

Combination therapy in pulmonary arterial hypertension is widely used in daily clinical practice. There is a wide variation among reference centers, with respect to not only when and how combination therapy should be initiated, but also what constitutes the most effective multidrug regimen. Presently, no combination therapy has proven to be more effective than the other. However, add-on therapy, in case of unsatisfactory improvements after initiation of specific monotherapy, seems to have more evidence of effectiveness than up-front therapy at the diagnosis. Data from national pulmonary arteri

Current opinion in germ cell cancer 2000.

Despite its relative rarity compared with the common adult cancers, scientific and clinical interest in germ cell cancer is increasing. From the point of view of epidemiology, the controversy about the relative importance of intrauterine versus postpubertal risk factors has continued. Evidence to support the importance of intrauterine factors comes from reports from Norway, Canada, and the US, confirming the Danish observation that the rising incidence of germ cell cancer is linked to a birth cohort effect; evidence in support of the importance of postpubertal risk comes from three case/contro

Efficacy of, and quality of life after antireflux surgery.

A complete evaluation of reflux operations depends on measurements of preoperative and postoperative clinical symptoms such as dysphagia and heartburn, assessment of quality of life and objective measurements of 24-hour oesophageal pH monitoring and manometry. Quality of life can be measured with questionnaires such as the Gastrointestinal Symptom Rating Scale (GSRS), the Psychological General Wellbeing Index (PGWB) and the Reflux-related Visual Analogue Scale (RVAS). Failure of treatment is indicated by persistent symptoms such as heartburn, need for antisecretory medication, and new symptoms

[Rosacea. Clinical features and treatment].

The paper describes clinical features, concepts in the pathogenesis, and therapeutical aspects of rosacea. Particular attention is paid to recent experiences on topical therapy with metronidazole. A Norwegian double-blind, multi-centre clinical trial compares metronidazole 1% cream applied twice daily with placebo cream in the treatment of 97 patients with rosacea. After eight weeks of treatment there was a statistically significant difference between the two regimens in favour of metronidazole cream. No important side effects were registered. It is concluded that 1% metronidazole cream is an

American Burn Association Clinical Practice Guidelines on Burn Shock Resuscitation.

This Clinical Practice Guideline (CPG) addresses the topic of acute fluid resuscitation during the first 48 hours following a burn injury for adults with burns ≥20% of the total body surface area (%TBSA). The listed authors formed an investigation panel and developed clinically relevant PICO (Population, Intervention, Comparator, Outcome) questions. A systematic literature search returned 5978 titles related to this topic and after 3 levels of screening, 24 studies met criteria to address the PICO questions and were critically reviewed. We recommend that clinicians consider the use of human al

Intraoperative Hyperspectral Imaging for Perfusion Assessment and Emerging Decision Support in Abdominal Surgery: A Systematic Review of Clinical Studies.

Background and Objectives: Intraoperative assessment of tissue perfusion remains a decisive but imperfect step in abdominal surgery. Surgeons still rely heavily on visual judgement when choosing bowel transection lines, constructing anastomoses, judging intestinal viability, or assessing graft reperfusion, even though these decisions are directly linked to anastomotic leak, conduit ischemia, postoperative liver dysfunction, and graft failure. Hyperspectral imaging (HSI) is an emerging contrast-free optical technology that generates quantitative maps of tissue oxygenation, hemoglobin distributi

Sensorineural hearing loss after select procedures.

Hearing is one of the most significant senses; therefore, any loss can be frightening. Previous studies have found that certain surgical and medical procedures can result in hearing loss. Cardiac surgical procedures are most commonly associated with hearing loss owing to the use of the cardiopulmonary bypass pump. Other noncardiac, nonotologic surgical procedures also may result in hearing loss. Additionally, medical procedures such as myelogram and lumbar puncture have been associated with hearing loss. Studies suggest that hearing loss can result any time cerebrospinal fluid is leaked. This

Multiple comorbid neuropathologies in the setting of Alzheimer's disease neuropathology and implications for drug development.

Dementia is often characterized as being caused by one of several major diseases, such as Alzheimer's disease (AD), cerebrovascular disease, Lewy body disease, or a frontotemporal degeneration. Failure to acknowledge that more than one entity may be present precludes attempts to understand interactive relationships. The clinicopathological studies of dementia demonstrate that multiple pathologic processes often coexist. How overlapping pathologic findings affect the diagnosis and treatment of clinical AD and other dementia phenotypes was the topic taken up by the Alzheimer's Association's Rese

Depression in multiple system atrophy: Views on pathological, clinical and imaging aspects.

Multiple system atrophy (MSA) is a common atypical parkinsonism, characterized by a varying combination of autonomic, cerebellar, and pyramidal systems. It has been noticed that the patients with MSA can be accompanied by some neuropsychiatric disorders, in particular depression. However, there is limited understanding of MSA-related depression. To bridge existing gaps, we summarized research progress on this topic and provided a new perspective regarding pathological, clinical, and imaging aspects. Firstly, we synthesized corresponding studies in order to investigate the relationship between

STRA-MI-VT (STereotactic RadioAblation by Multimodal Imaging for Ventricular Tachycardia): rationale and design of an Italian experimental prospective study.

BACKGROUND: Ventricular tachycardia (VT) is a life-threatening condition, which usually implies the need of an implantable cardioverter defibrillator in combination with antiarrhythmic drugs and catheter ablation. Stereotactic body radiotherapy (SBRT) represents a common form of therapy in oncology, which has emerged as a well-tolerated and promising alternative option for the treatment of refractory VT in patients with structural heart disease. OBJECTIVE: In the STRA-MI-VT trial, we will investigate as primary endpoints safety and efficacy of SBRT for the treatment of recurrent VT in patient

Characteristics of VA hospitals by participation status in a large pragmatic embedded clinical trial.

BACKGROUND: The Diuretic Comparison Project (DCP) was a multi-center, embedded pragmatic trial conducted within the VA healthcare system comparing chlorthalidone and hydrochlorothiazide in preventing major adverse cardiovascular events and non-cancer deaths in hypertensive patients. Study procedures were decentralized, and facility leadership first had to agree and accept study procedures before staff approached participants. Recent evidence suggests facilities that choose and choose not to participate in trials may differ and this study considered such differences within DCP. METHODS: A cros

On the value of doing nothing: anger and cardiovascular disease in clinical practice.

Type A behavior, anger, and hostility have all been described as psychosocial risk factors or potential risk factors for cardiovascular disease (CVD). However, in the populations evaluated, the methods used to measure these traits and the endpoints used to determine outcomes have varied widely. The results of these studies have also been quite disparate. The present article reviews data from all the existing quantitative (meta-analytic) and qualitative systematic review articles that cover this topic, using the Database of Abstracts of Reviews of Effects (DARE) criteria. We conclude that the e
