BACKGROUND: The incidence of closed proximal humerus fractures (CPHF) is steadily increasing worldwide. Open reduction internal fixation (ORIF) remains the most common surgical technique while reverse shoulder arthroplasty (RSA) is becoming a noteworthy approach. The optimal management of CPHF (ORIF vs. RSA) remains complex and continually evolving.
METHODS: This retrospective cohort study utilized data from the National Inpatient Sample (2016-2019). Patients with proximal humerus fractures (PHF) treated with ORIF or RSA were included. Demographics, fracture characteristics, comorbidities, an

Volar proximal interphalangeal (PIP) joint fracture-dislocations presenting more than two weeks after injury pose significant management challenges, as progressive soft tissue fibrosis and capsular contracture frequently necessitate open reduction. Reports of successful delayed closed reduction with long-term follow-up in this setting remain limited in the literature. We report an 18-year-old man who presented 16 days after injury with a neglected volar PIP fracture-dislocation of the left ring finger, managed with delayed closed reduction under sedation and fluoroscopic guidance the following

PURPOSE: Nasal bone fracture reduction surgery is normally followed by nasal packing to control bleeding. Yet, patients suffer from pain and require further analgesic treatments following nasal packing and removal. This study aimed to evaluate the effect of fentanyl-soaked packing as a method of controlling pain after nasal surgeries in a prospective, randomized, double-blind controlled trial.
METHODS: Sixty-five patients that have undergone closed nasal bone fracture reduction surgery were included in this study. Thirty-two patients were treated postoperatively with 50 mcg fentanyl-soaked Me

Infectious complications of closed reduction and percutaneous pinning of supracondylar humerus fractures are exceedingly rare. Although postoperative Pseudomonas infection is a feared complication associated with noncompliance and a wet cast, there are no reports in the literature of this occurring. We present the devastating complication of a pediatric patient who developed Pseudomonas aeruginosa subperiosteal abscess, osteomyelitis, and elbow septic arthritis after presenting to the clinic multiple times with a wet cast after closed reduction and percutaneous pinning of a supracondylar humer

Pediatric humerus lateral condyle fractures (PHLCF) are one of the most common fractures among children. PHLCF is approximately 20% of all pediatric elbow fractures. Among the treatment modalities are: nonoperative treatment with splinting, closed reduction with percutaneous K-wire fixation (CRPP), and open reduction with K-wire fixation (ORIF). We aimed to evaluate radiological, functional, and clinical outcomes of Jakob type 2 PHLCF treated either with CRPP or ORIF. We hypothesized that CRPP may be as safe and effective as ORIF for Jakob type 2 PHLCF. In this retrospective study, we included

BACKGROUND: Closed reduction and percutaneous pinning (CRPP) is the standard treatment for pediatric supracondylar humerus fractures (SCHF). However, closed reduction can be challenging in a subset of patients and may fail, requiring conversion to open reduction. A practical preoperative tool to predict failed closed reduction remains lacking.
METHODS: This multicenter retrospective cohort study included pediatric patients with SCHF who underwent intended CRPP at Xiangtan Central Hospital and Zhuzhou Central Hospital between 2020 and 2025. The primary outcome was failed closed reduction, defi

BACKGROUND: Displaced U- or H-shaped sacral fractures (Roy-Camille Grade II or III) are treated at our institution by early transcondylar traction and manual countertraction, hyperlordosis induced by a pad positioned under the lumbo-sacral junction, and percutaneous ilio-sacral screw fixation. The objective of this study was to evaluate the outcome of this technique used in a level 1 trauma centre. Hypothesis Our early reduction technique provides anatomical reduction of U- or H-shaped sacral fractures by correcting the sagittal malalignment due to the intra-sacral kyphosis, thereby obviating

BACKGROUND: Anatomic reduction of some displaced pediatric supracondylar humerus fractures is not attainable via closed manipulation, thus necessitating open reduction. Open reduction has been associated with increased complications, including elbow stiffness, scarring, iatrogenic neurovascular injury, and longer hospital stays. Using a Schanz pin to aid in closed reduction may decrease the need for conversion to an open procedure, possibly reducing morbidity.
DESCRIPTION OF TECHNIQUE: A percutaneously placed 2.5-mm Schanz pin was drilled into the posterior humeral diaphysis and used as a joy

INTRODUCTION: Both fractures of the lumbar spine and at the thoracolumbar junction are quite common. The treatment of these fracture types is discussed controversially. Some authors advocate surgical treatment even in fractures without neurologic compromise while other series report good results after non-operative treatment.
MATERIALS AND METHODS: Between January 1997 and April 2004, 324 patients with spinal fractures were admitted to our institution. Hundred and thirty-six patients with compression and burst type fractures treated by closed reduction and casting were available for follow-up

BACKGROUND: Femoral neck fractures, pose significant risks of AVN and non-union. Recent studies suggest that adding capsulotomy to CRIF could reduce complications by alleviating intracapsular pressure and enhancing femoral head vascularity. This study aims to assess the efficacy of capsulotomy combined with CRIF in reducing the incidence of AVN and non-union in Garden III femoral neck fractures compared to CRIF alone.
METHODS: Participants aged 18-55 with Garden III femoral neck fractures were randomly assigned to either the CRIF-only group or the CRIF with capsulotomy group. Outcomes include

The neck of femur fracture (FNF) in children is a rare injury with a high incidence of complications such as avascular necrosis (AVN), coxa-vara and nonunion. The aim of this review is to compare the incidence of complications between open reduction with internal fixation (ORIF) and closed reduction with internal fixation (CRIF) of FNF in children. Two independent reviewers searched EMBASE, MEDLINE, COCHRANE and PUBMED databases from inception until April 2022 according to the preferred reporting items for systematic reviews and meta-analyses guidelines. Studies included comparison of complica

BACKGROUND: Interventions using ultrasound-guided closed reduction and percutaneous pinning (UG-CRPP) of humeral lateral condylar fractures (HLCFs) have been increasingly applied; however, their effectiveness for unstable HLCFs and the criteria for ultrasound outcomes remain unclear. This study assessed the outcomes of UG-CRPP for HLCFs and evaluated the success criteria in children.
METHODS: Data were retrospectively collected from 106 patients with unstable HLCFs admitted to three hospitals between January 2021 and August 2022. Fifty-five cases were left-sided and 51 cases were right-sided:

BACKGROUND: Percutaneous pinning fixation (PCP) has been used for the treatment of distal radius fractures for decades, especially in the elderly with fragile soft tissue. However, achieving and maintaining a sound anatomic reduction before PCP is difficult if we use the manipulative reduction method alone. Our study innovatively applied the Steinmann pin retractor for closed reduction combined with PCP, to provide a new protocol for the treatment of distal radius fractures.
METHODS: From March 2017 to July 2018, 49 patients out of 57 that met the inclusion criteria but not the exclusion crit
