[Aplasia of the posterior tibial artery in a child with idiopathic clubfoot].

A case of a completely absent posterior tibial artery discovered during a procedure for congenital clubfoot correction in a 6 month old male infant is reported. Anomalies of the anterior tibial artery associated with the clubfoot-deformity are common, the absence of the posterior tibial artery is, however, very rare.

Evaluation of the treatment of idiopathic clubfoot by using the Ponseti method.

The initial treatment of congenital idiopathic talipes equinovarus (clubfoot) is most often nonsurgical. However, surgical treatment in the form of posteromedial release is often undertaken after failure of conservative measures. The prevalence of both immediate and long-term complications in surgically treated clubfeet has cultivated a renewed interest in nonsurgical treatment. The Ponseti method for treating clubfoot has seen a revived interest among those caring for infantile clubfeet. We report on our first 34 infants (57 clubfeet) treated by using the techniques and principles described b…

Percutaneous fixation in clubfoot surgery: a radiographic and gait study.

Twenty patients with Type II clubfeet were evaluated an average of 3.1 years after complete surgical soft tissue release. A pinned group had … t significantly different for the two treatment groups, but was significantly different between all clubfoot patients (3.6 degrees of in-toeing) and the control group (5.0 degrees of out-toeing). Tourniquet time was significantly greater for the pinned than for the non-pinned group, but the duration of surgery was similar. Similar radiographic and gait mea …

Significance of talar distortion for ankle mobility in idiopathic clubfoot.

The abnormal bony feature found most consistently in clubfeet is talar distortion. The significance of the talar distortion for mobility of the tibiotalar joint was investigated. Twenty-seven congenital clubfeet in 19 patients were examined at a minimal followup of 20 years. In all patients Turco's posteromedial release was done because of idiopathic clubfoot. Radiographic assessment of the feet included measurement of the talocalcaneal angle and index, and the tibiocalcaneal angle. The degree of talar flattening was estimated by the ratio of the curvature of the talar dome to the length of th…

[Results of treatment of congenital clubfoot with modified Goldner's technique].

The authors present the results of surgical treatment of resistant idiopathic and recurrent clubfoot (type IV according to Dimeglio classification). In the group who underwent a primary procedure, results were noted according to the Magone classification: 35.7% very good results, 11.9% good results, 9.52% satisfactory and 42.5% bad results (recurrent clubfeet accounts for 37.5%). The authors noted that the Goldner technique yields results comparable to the peri-talar release technique.

[Clubfoot orthosis with integrated dynamic cuff for retention and redressing].

Conservative treatment of clubfoot deformities with braces and bandages is traditionally several hundred years old. The clubfoot braces of Venel, Scarpa, and Schulthess, as well as the redressing bandage of Cheselden, are reviewed as typical examples. The treatment of clubfoot deformities was initiated in the Balgrist Hospital for Orthopedic Surgery by W. Schulthess. Recently, in Balgrist, we developed a brace that retains and redresses the foot. This brace is described, and early clinical results are reported.

[The possibilities of operative treatment of residual and recurrent congenital clubfoot].

The paper presents 14 different surgical procedures recommended in literature for the correction of singular deformed elements of congenital clubfeet. Basing on clinical material collected from patients treated during the last five years, the authors introduce the application of the presented procedures for the correction of residual and recurrent deformities occurring in congenital clubfeet. Basing on their own experience, knowledge and available publications the authors attempt to assess the indications for each particular procedure and their mutual connections, which depend on: the kind and

[Evaluation of treatment outcome in congenital clubfoot by subtalar release].

This paper presents an analysis of the results of congenital clubfoot treatment by partial or complete subtalar release performed through the Cincinnati approach. Of 116 patients (142 feet) who underwent surgery in the years 1995-1996, 33 (47 feet) came in for final follow-up. The type of primary deformity was defined in only in 36 feet. There were 25 type II deformity and 1 type III deformity. Total subtalar release was performed in 39 feet. Partial subtalar release was performed in 8 feet. During the release procedure, the calcaneo-cuboid joint was not opened, but stabilized with a K-wire. C…

Evaluation of pathologic abnormalities of clubfoot by magnetic resonance imaging.

Three-dimensional analyses of clubfoot in infants younger than 1 year of age were done using magnetic resonance imaging in an in vivo study. Twenty-one patients (31 feet) with congenital clubfoot were examined. The average age at examination was 8.6 months (range, 4-12 months). All patients originally were treated using corrective casts. Seventeen feet required complete subtalar release operations and the remaining 14 feet were treated conservatively with various orthoses. Four measurements using magnetic resonance imaging were performed in the transverse and coronal planes as follows: the cal…

The natural history and longitudinal study of the surgically corrected clubfoot.

Surgical treatment for clubfoot has been largely directed at finding the best one-stage operation for the resistant clubfoot. Eighteen patients with 27 clubfeet (average follow-up 11 years since first surgery; range, 3.5-24 years) were reviewed. More than one clubfoot operation was required in 56% of cases. Forty-six percent were corrected after one surgery; 33% required a second surgery and 14% required a third operation. One patient with particularly severe feet required a fourth operation on each foot. The mean age at the time of surgery was 1.26 years, 5.12 years, and 8 years for the first…

Surgical treatment of clubfoot: the significance of talocalcaneonavicular malposition correction.

In a retrospective study on the surgical management of clubfoot based on clinical and radiographic assessments, 171 feet in 137 patients were reviewed. The surgical procedure was selected according to the degree of the deformity. The more severe cases (group A, 75 feet) were surgically treated according to Turco's one-stage posteromedial release, whereas the milder degrees of deformity (group B, 96 feet) were corrected by elongation of the Achilles tendon with posterior capsulotomy. The mean age of the patients at surgery was 12.5 months in group A and 5.2 months in group B. The mean follow-up…

Computed tomography for femoral and tibial torsion in children with clubfoot.

Forty-seven children with 70 clubfeet had computed tomography studies performed to determine the degree of femoral, tibial, and total limb torsion in both lower limbs. The total limb torsion angle (angle between the axis of the femoral neck and the axis of the ankle), which describes the relationship between femoral and tibial torsion, was used to evaluate the whole rotational deformity of the lower limb. The children were between the ages of 2 and 10 years (mean, 5 years) at the time of the computed tomography study. The mean femoral torsion was 25 degrees in the limbs with a clubfoot and 23 …

The significance of prenatally identified isolated clubfoot: is amniocentesis indicated?

OBJECTIVE: Our purpose was to determine the significance of finding an isolated clubfoot on a prenatal sonogram. STUDY DESIGN: All fetuses found to have an isolated congenital clubfoot over a 9-year period were retrospectively identified. Fetuses with associated anomalies were excluded. Review of medical records for obstetric and neonatal outcome and pathologic and cytogenic results were tabulated. RESULTS: Eighty-seven fetuses were identified from our database as having isolated clubfoot on prenatal ultrasonography, with complete follow-up available for 68 fetuses. Sixty of the 68 fetuses w…

Prevalence of unsuspected myopathy in infants presenting for clubfoot surgery.

The objective of this study was an evaluation of the prevalence of myopathies in paediatric patients scheduled for orthopaedic surgery (clubfoot) performed under regional anaesthesia. Seventeen infants scheduled for lower limb orthopaedic surgery were studied to verify coexisting neuromuscular disorders with electromyography and muscle biopsy during surgery. All surgical procedures were performed under caudal block or spinal anaesthesia, associated with light general anaesthesia. No major cardiorespiratory, neurological or malignant hyperthermic complications (muscle rigidity, arrhythmias, hyp…

[Comparison of evaluation methods of the results of congenital clubfoot treatment].

INTRODUCTION: An ideal method for the assessment of the results of treatment in congenital clubfoot should take into account the morphology of the foot and its function, the personal satisfaction of the patient of his parents, interexaminer variability, and be applied for any type of treatment. MATERIAL AND METHODS: We evaluated at- end of growth the results of treatment of 35 patients with unilateral clubfoot, and analyzed them according to 13 different rating scores already published in the literature. The material was divided in 3 groups; 15 feet having undergone multiple surgeries, 10 fee…

[Current aspects of treatment for clubfoot in infants and young children].

The congenital club-foot is one of the frequent malformations (incidence around 2%). The treatment during early childhood implies conservative measures with corrective manipulations and casting, surgical procedures with extensive soft tissue releases. The technically demanding medial release is a well established procedure; however it is often not performed because the peritalar malpositions are often underestimated in the young child. After surgical correction stabilization of the foot for prolonged time is important and follow-up is mandatory until adulthood is reached.

Our experiences with the early operative treatment of congenital clubfoot.

In operative clubfoot therapy, early operation plays an important role. Between 1975 and 1982, we operated on 55 children with 72 clubfeet: 56 posteroplantar releases in patients with an average age of 5.8 months, and 16 posteromedial releases in patients with an average age of 8.1 months. The average follow-up was 3.5 years. The results, based on an investigation scheme according to Henkel, were good and very good in 94%. Operative treatment is indicated after 3 to 4 months of intensive plaster treatment if there has been no further correction.
