Potential hypothalamic mechanisms in trigeminal neuropathic pain: a comparative analysis with migraine and cluster headache.

Trigeminal neuropathic pain (TNP), migraine, and cluster headache (CH) profoundly impact the quality of life and present significant clinical challenges due to their complex neurobiological underpinnings. This review delves into the pivotal role of the hypothalamus in the pathophysiology of these facial pain syndromes, highlighting its distinctive functions and potential as a primary target for research, diagnosis, and therapy. While the involvement of the hypothalamus in migraine and CH has been increasingly supported by imaging and clinical studies, the precise mechanisms of its role remain …

Circadian Features of Cluster Headache and Migraine: A Systematic Review, Meta-analysis, and Genetic Analysis.

BACKGROUND AND OBJECTIVES: Cluster headache and migraine have circadian features at multiple levels (cellular, systems, and behavioral). A thorough understanding of their circadian features informs their pathophysiologies. METHODS: A librarian created search criteria in MEDLINE Ovid, Embase, PsycINFO, Web of Science, and Cochrane Library. Two physicians independently performed the remainder of the systematic review/meta-analysis using Preferred Reporting Items for Systematic Review and Meta-Analyses guidelines. Separate from the systematic review/meta-analysis, we performed a genetic analysis…

Regional cerebral blood flow as predictor of response to occipital nerve block in cluster headache.

BACKGROUND: Cluster headache is an excruciating disorder with no cure. Greater occipital nerve blockades can transiently suppress attacks in approximately 50% of patients, however, its mechanism of action remains uncertain, and there are no reliable predictors of treatment response. To address this, we investigated the effect of occipital nerve blockade on regional cerebral blood flow (rCBF), an index of brain activity, and differences between treatment responders and non-responders. Finally, we compared baseline perfusion maps from patients to a matched group of healthy controls. METHODS: 21…

Use of cluster analysis to validate IHS diagnostic criteria for migraine and tension-type headache.

Cluster analysis was used to validate headache diagnostic criteria of the International Headache Society (IHS). Structured diagnostic interviews were conducted on 443 headache sufferers from a community sample, which was randomly split to allow replication. Hierarchical cluster analysis of symptoms in both subsamples revealed two distinct (P<.001) clusters: (1) unilateral pulsating pain, pain aggravated by activity, and photophobia and phonophobia, and (2) bilateral pressing/tightening pain, mild to moderate intensity, and absence of nausea/vomiting. These clusters were consistent with IHS mig

[3-year follow-up after endonasal microscopic paranasal sinus surgery in migraine and cluster headache].

BACKGROUND: Migraine and cluster headache can both be triggered by sensitive intranasal areas. METHODS: Endoscopic nasal surgery was performed in 20 patients with chronic migraine without aura or cluster headaches that were refractory to other forms of treatment for a mean period of 18 years (range of 1-45 years). The selected patients showed clinical and radiographic evidence of contact between the middle turbinate and the nasal septum. All patients experienced immediate relief of pain following topical application of cocaine to the presumable triggering area. Five patients with cluster head…

Imbalance of heart rate regulation in cluster headache as based on continuous 24-h recordings.

An altered autonomic balance is considered to be a pathogenetic factor in cluster headache syndrome, although there is varying data on sympathetic and/or parasympathetic activation during attacks and/or attack-free intervals. The aim of the present study was to evaluate the day/night pattern of heart rate during the active and remission phases of cluster headache. In addition, the relationship between heart rate changes and the site of pain was investigated to determine if an autonomic imbalance was related to the lateralization of pain. Thirty-nine patients (34 with primary episodic cluster h…

The impact of climatic factors on headache patterns: a 14-year time series analysis of cluster and tension-type headaches in primary care.

BACKGROUND: Weather-related influences on pain and neurological symptoms are widely discussed in biometeorology. However, the long-term impact of climatic variables on primary care headache consultations remains unclear. This study aimed to examine whether climatic variables are associated with consultations for cluster headaches, tension-type headaches (TTH), and unspecified headaches over 14 years. METHODS: Data (2010-2023) were extracted from medical records using International Classification of Primary Care, Second Edition (ICPC-2) codes. Meteorological variables (temperature, rainfall, w…

Genome-wide association susceptibility loci for cluster headache support a role for inflammation in the pathophysiology.

BACKGROUND: Recent genetic findings have broadened the perspectives of cluster headache pathophysiology. Genes identified by genome wide association studies are likely to be involved in the pathophysiological mechanisms underlying the disease. In this study, we performed validation and characterization of eight loci corresponding to nine genes identified in previous genome wide association studies. METHODS: Genetic loci were validated by means of a case-control study using TaqMan genotyping in 671 individuals, and a meta-analysis using previously genotyped data. Pyrosequencing methylation ana…

Stereotactic radiosurgery in the management of cluster headache: evidence from a systematic review and meta-analysis.

Cluster headache (CH) is a highly disabling primary headache disorder, and a subset of patients remain refractory to medical or neuromodulatory therapies. Stereotactic radiosurgery (SRS) has been explored as a minimally invasive alternative, but reported outcomes are inconsistent. A systematic review and meta-analysis was performed following PRISMA guidelines. A comprehensive search of PubMed, Embase, Scopus, and Web of Science from inception to September 15, 2025, identified eligible studies. Treatment techniques varied across studies, including radiosurgical targeting of the gasserian gangli…

Inverse association of obesity with bout periodicity in episodic cluster headache: a multicenter cross-sectional study.

BACKGROUND: Cluster headache (CH) is the most painful headache disorder. Despite a large body of evidence on obesity's negative influence on migraine, its impact on cluster headache disease activity remains unexplored. We aimed to determine whether body mass index (BMI) and obesity are associated with lifetime bout occurrence and annual bout frequency in patients with episodic cluster headache (ECH). METHODS: The Korean Cluster Headache Registry (KCHR) is a prospective, multicenter registry of consecutive patients with CH over 4 years. This cross-sectional study included 316 eligible patients…

SYMPATHETIC NERVE ENTRAPMENT POINT INJECTION AS AN ADJUVANT TREATMENT FOR INTRACTABLE CLUSTER HEADACHE: A CASE REPORT.

BACKGROUND: Cluster headache (CH) is characterized by severe unilateral pain ranging from the orbital to the temporal regions with ipsilateral autonomic manifestations. Although most patients respond to drugs or oxygen inhalation, some do not. In this case report, we introduce sympathetic nerve entrapment point injection (SNEPI), a new adjuvant treatment for CH. CASE REPORT: We introduce two CH patients who did not respond well to pharmacological treatment or 100% oxygen inhalation, but who improved after SNEPI. Patient 1, a 42-year-old man, visited the Emergency Department (ED) with severe p…

Cluster headache: forehead sweating pattern during heating and pilocarpine tests. Variation as a function of time.

Thirty-one patients with cluster headache were examined with regard to their forehead sweating pattern, by means of the Evaporimeter. Sweating was stimulated in two different ways: by body heating and by parenterally administered pilocarpine. The resulting increase in evaporation was frequently measured at different positions on both sides of the forehead, and the possibility of variations in the pattern related to the passage of time was specifically scrutinized. Some typical patterns emerged. The previously reported, marked asymmetries of response (deficient heat-induced sweating and pilocar…

Diagnostic delay, lateralisation and changing demographics in a belgian monocentric cluster headache cohort: a retrospective study.

Cluster headache is a primary headache disorder characterised by severe headache attacks, which necessitates a prompt diagnosis and treatment. The objective of the present retrospective single centre cohort study was threefold: firstly, to assess the diagnostic delay after symptom onset and its evolution over time (1979–2024). Secondly, to investigate the male to female ratio over time. Thirdly, to study the lateralisation in patients with side-locked symptoms. The median diagnostic delay decreased progressively over time to less than a year in patients with a symptom onset after 2009. Secondl…

White matter microstructural alterations in episodic cluster headache outside attacks: a tract-based spatial statistics study.

BACKGROUND: Cluster headache (CH) is a highly disabling primary headache disorder increasingly conceptualized as a disorder of distributed brain networks rather than an isolated trigeminovascular dysfunction. While structural and functional MRI studies have demonstrated persistent brain alterations outside the ictal phase, the role of white matter microstructural changes supporting network-level dysfunction remains incompletely understood. METHODS: We investigated cerebral white matter microstructure in 27 patients with episodic CH examined during the bout outside active attacks using diffusi…

Potential association of cluster headache with certain COVID-19 vaccines: An assessment from the pharmacovigilance databases.

OBJECTIVES/BACKGROUND: The main objective of our study was to assess the potential association between coronavirus disease 2019 (COVID-19) vaccination and cluster headache (CH) using a pharmacovigilance approach. A secondary objective was to determine whether COVID-19 vaccines predominantly triggered onset or worsening of the disease. Recent pharmacovigilance studies and case reports have suggested an association between COVID-19 vaccines and cluster headache. In order to evaluate whether COVID-19 vaccines could possibly elicit the onset of cluster headache or be responsible for worsening of t…
