Coccidioidal meningitis (CM) is a devastating complication of coccidioidomycosis. Since the late 1950s, intrathecal (IT) amphotericin B deoxycholate (AmBd) has been successfully used to treat and often cure this disease, reducing mortality rates from 100% to approximately 30%. The introduction of azoles further revolutionized the treatment of coccidioidal infections. However, IT AmBd remains the only known curative option in the management of CM. While the use of IT AmBd is well described in many articles, few discuss the actual methods behind preparation, titration, and dosing strategies util

Coccidioidomycosis (CM) is an infection caused by the coccidioides fungus. The spectrum of the disease ranges from a mild upper respiratory illness to severe disseminated infection that could affects the bones, joints, skin, central nervous system, and the gastrointestinal tract. Previous reports of a positive stool culture in the setting of disseminated CM are extremely rare. Herein, we describe a case of a 43-year-old immunocompetent patient with disseminated CM and evidence of positive coccidioides stool culture.

Use of amphotericin B-impregnated bone cement in combination with systemic antifungals for the treatment of coccidioidal osteomyelitis offers the potential for sustained local concentrations of drug at the site of the infection. Amphotericin B levels in bone of up to 5.1 μg/g have been demonstrated 4 months after placement of bone cement.

Trying to surpass host defenses, fungal infections alter the immune response. Components from nonpathogenic fungi present therapeutic anti-inflammatory and immunomodulating activities. This study reveals that proteins present in a Coccidioides posadasii extract provide anti-inflammatory benefit in experimental arthritis. Zymosan was given intra-articularly to rats and mice, and groups were pretreated with C. posadasii extract either per os or intraperitoneally. Controls received the vehicle. Acute hypernociception was evaluated using articular incapacitation and von Frey methods. Cell influx a…

A 60-year-old male kidney transplant recipient presented with pneumonia 4 months after transplantation. Coccidioidomycosis was suspected and empirically treated with fluconazole. He was maintained on suppressive fluconazole without problems. Three weeks after discontinuing secondary prophylaxis, the patient experienced coccidioidal arthritis and an infection of the soft tissue of the hand that required debridement. Transplant recipients may have quiescent disseminated coccidioidomycosis that is reactivated by immunosuppression after withdrawal of suppressive antifungal therapy.

Coccidioidomycosis is an acquired fungal infection that afflicts primarily the respiratory tract. Cystic fibrosis patients who are being treated with glucocorticoids and immunosuppressed organ recipients may be at risk for infection with Coccidiodes immitis or reactivation of latent infection. The diagnosis is best made by demonstration of the organism in pathologic specimens or by culture. Serologic screening is another method that is reliable in most patients. We studied 98 patients who had serologic screening for Coccidiodes immitis performed as part of their evaluation for lung transplanta

The activity of the novel triazole SCH 51048 was tested against Coccidioides immitis. SCH 51048 inhibited C. immitis in vitro; MICs for 13 isolates ranged from < or = 0.39 to 0.78 micrograms/ml, and minimum fungicidal concentrations ranged from < or = 0.39 to 1.6 micrograms/ml. In vivo, no mice treated with SCH 51048 at 2 to 50 mg/kg of body weight or 100 mg of fluconazole or itraconazole per kg died of systemic coccidioidomycosis, whereas 60 to 100% of the control mice died. SCH 51048 given at 25 or 50 mg/kg was curative, whereas fluconazole or itraconazole given at 100 mg/kg was not curative…

Four antigens prepared from Coccidioides immitis were found to evoke no dermal reactions in 1,245 volunteers on first testing, but 2 months later, upon retesting, 18.6% of the subjects reacted to 1 or more of the antigens. In no subject did other findings indicate an intercurrent coccidioidal infection. Conversion of a skin test to reactive significantly (p less than 0.018) increased with subject age. No association was found with either years of residence within areas endemic for C. immitis or occupational exposure, associations that would be expected if conversion was due to latent hypersens…

The value of 67Ga and methylene diphosphonate labeled with 99mTc in detecting extrapulmonary lesions was evaluated in 12 patients win whom disseminated nonmeningeal coccidioidomycosis developed after a single epidemic exposure in December 1977. Studies were performed with each radionuclide from 4 to 9 months after infection. Comparison of scintigraphy with radiography showed that eight of 14 osteolytic lesions identified by radiography were present on the bone and gallium scans; four were present only on the gallium scan, and two were undetected. Of 15 additional lesions present in both scanni

Primary coccidioidal synovitis of the knee is a rare disorder and controversy exists in the literature over the efficacy of treatment with synovectomy and Amphotericin B therapy. Four cases are presented illustrating features of the natural history, diagnosis, and treatment of the disease.

Amphotericin B, the principal drug used for treating systemic mycoses, possesses undesirable toxic properties. The ability of this antibiotic to potentiate antifungal activity of other compounds suggests that lower doses of amphotericin B could be used in combination with a second drug without loss of therapeutic efficacy. In vitro tests demonstrated that amphotericin B potentiated rifampin against the mycelial growth phase of Coccidioides immitis but not against the spherule-endospore phase. Therapy for murine coccidioidomycosis with a combined amphotericin B-rifampin regimen was not better t…

BACKGROUND: Coccidioides are fungi endemic to western USA and are estimated to cause more than 200 000 symptomatic cases of coccidioidomycosis, also known as Valley fever, annually. Endemicity is influenced by environmental conditions, including precipitation, temperature, and soil composition. As environmental conditions change in the future, the geographical distribution of the fungi can be altered.
METHODS: In this modelling study, we used generalised additive models to construct two Coccidioides suitability models at the county and ZIP code tabulation area (ZCTA) scales. The county model …

Coccidioidomycosis is a fungal disease in arid regions of the United States that is predicted to expand with climate change. Cases in military personnel and military working dogs (MWDs) impact personnel readiness and result in healthcare costs. To examine Coccidioides exposure among MWDs, 276 banked serum samples were retrieved from dogs housed in California, Texas, Arizona, New Mexico, Nevada, and Utah. Using gel immunodiffusion, six (2.1%) specimens were IgG-positive and three (1.1%) were equivocally IgM-positive. The IgG-positive samples were from Arizona (2 [prevalence 8.0%]) and Californi…

Coccidioidomycosis is a common fungal disease that is endemic to arid and semi-arid regions of both American continents. Coccidioides immitis and Coccidioides posadasii are the etiological agents of the disease, also known as Valley Fever. For several decades, the C. posadasii strain Silveira has been used widely in vaccine studies, is the source strain for production of diagnostic antigens, and is a widely used experimental strain for functional studies. In 2009, the genome was sequenced using Sanger sequencing technology, and a draft assembly and annotation were made available. In this study…

Rodents are a natural host for the dimorphic pathogenic fungi Coccidioides immitis and Coccidioides posadasii, and mice are a good model for human infection. Humans and rodents both express Dectin-1 and Toll-like receptor 2 (TLR2) on myeloid cells, and those receptors collaborate to maximize the cytokine/chemokine responses to spherules (the tissue form of the fungi) and to formalin-killed spherules (FKS). We showed that Dectin-1 is necessary for resistance to pulmonary coccidioidomycosis, but the importance of TLR2 in vivo is uncertain. Myeloid differentiation factor 88 (MyD88) is the adapter…
