Cochlear implantation without mastoidectomy: the pericanal electrode insertion technique.

OBJECTIVE: A new minimally invasive cochlear implantation method with direct electrode insertion through the external auditory canal (EAC) is presented. MATERIAL AND METHODS: Surgery begins with a retroauricular skin incision. The bony surface of the mastoid plane behind the ear is dissected free and the skin of the EAC is elevated together with the posterior part of the tympanic membrane. Cochleostomy is performed through the EAC with a microdrill anterior to the round window. A rim is drilled into the postero-superior region of the bony EAC immediately above the incus towards the outer bord…

Intracochlear factors contributing to psychophysical percepts following cochlear implantation.

The performance of cochlear implant patients may be related to intracochlear, histopathological factors. We have performed detailed post-mortem examinations of five human, implanted cochleas and for each electrode correlated the psychophysical threshold, comfortable level and dynamic range with spiral ganglion cell survival, presence of fibrous tissue and/or new bone, and distance between the centers of the electrode bands and Rosenthal's canal. The psychophysical parameters were strongly interrelated. Threshold and comfort levels correlated with the distance between the electrodes and Rosenth

Cochlear implantation in a patient with grand mal epilepsy.

A case is reported in which a Nucleus 22 channel intracochlear implant was used to treat a deaf Hungarian woman (aged 37 years) with a 34-ye … ntral region) or indeed, in other regions of the brain at use of the implant. We conclude that intracochlear implantation per se is not a hazardous intervention in patients with fronto-precentral epileptic foci.

Histopathology of temporal bone fractures: implications for cochlear implantation.

Temporal bone fractures often cause loss of audiovestibular function. Those patients with bilateral profound sensorineural hearing losses secondary to temporal bone fractures become candidates for cochlear implantation. The authors present the histopathology of five temporal bone fractures in three patients, evaluating specifically the traumatic effects on the neural elements of the inner ear. Transverse fractures of the temporal bone result in severe loss of hair cells, ganglion cells, and other supporting cells in the inner ear. Occasionally labyrinthitis ossificans may occur as a consequenc…

Histopathology of the temporal bone following multichannel cochlear implantation.

The temporal bone histopathologic findings of a patient having received a Nucleus 22-channel cochlear implant (Cochlear Corp, Englewood, Colo) following bilateral transverse otic capsule fractures is presented. The case was complicated by perilymph fistulas, labyrinthitis, meningitis, and death due to drowning. The left cochlea revealed complete labyrinthitis ossificans; the implanted right cochlea demonstrated new bone formation in the basal turn of the scala tympani, degeneration of the organ of Corti, but normal ganglion cell counts. Results of psychophysical and speech perception 4 months

Long Term Voice Effects of Partial Deafness Cochlear Implantation.

Partial deafness (PD) causes a deprivation in hearing high frequencies. This leads to voice functional voice disorders. Study shows that PD is associated with changes in frequency, amplitude, noise, tremor, and voice irregularities. Partial deafness cochlear implantation (PDCI) has been proven to improve voice quality. The aim of the study was to evaluate the long-term voice effects of PDCI on voice. Voice samples of 16 adult bilaterally PD patients were analyzed 72 months after one-sided PDCI and compared to the results obtained 9 months after the procedure. Results showed that the improvemen…

Determining ideal electrode insertion depth in paediatric cochlear implantation.

BACKGROUND: Speech and language outcomes following early cochlear implantation vary widely among patients. Identifying modifiable, device-specific surgical parameters contributing to this variability is essential to optimise therapeutic outcomes. OBJECTIVE: This study investigates the association between electrode angular depth of insertion (aDOI) and speech perception outcomes in a homogeneous cohort of children implanted with a single lateral wall electrode design. METHODS: A cohort of 25 prelingually deaf children, aged 4 to 5 years at implantation, received unilateral Oticon Neuro ZTI co…

Cochlear Implantation Following Neonatal Meningitis: A Rare Case Report.

Bacterial meningitis in neonates may lead to profound sensorineural hearing loss and rapid postinflammatory cochlear changes that complicate auditory rehabilitation. We present two infants with bilateral profound hearing loss following neonatal bacterial meningitis who underwent early bilateral cochlear implantation. Radiological findings were limited or subtle, but intraoperative findings confirmed cochlear obstruction/ossification requiring adapted surgical management. Full electrode insertion was achieved in both children. Postoperative outcomes showed auditory benefit in both cases, althou…

Electrocochleography Latency: Correlation With Electrode Position During Cochlear Implantation.

OBJECTIVES: Cochlear implant (CI) candidates increasingly exhibit some degree of residual hearing, which should be preserved despite the imp … ophonic latency (CML) as an objective method for continuously tracking CI electrode position during cochlear implantation. In addition, we examined whether CML can be associated with residual hearing. DESIGN: We recorded intraoperative pure-tone ECochG at maximum stimulation levels from 30 CI patients to derive CML. During CI electrode insertion, ECochG was continuous …

Early cochlear implantation for children with single sided deafness.

OBJECTIVES: Single-sided deafness (SSD) can have consequences for a child's language, educational, and social development. A cochlear implant (CI) is the only device which can restore true binaural hearing, yet they are only approved for children with (SSD) over the age of five in the United States. Reports on speech perception outcomes for children implanted at a younger age are limited. The present study aims to examine the effects of age at implantation, duration of deafness, hearing loss etiology, and presence of additional disabilities on device usage and speech perception outcomes. Cochlear Implantation with a Combined Approach: A Case Report.

INTRODUCTION: Ear symptoms of granulomatosis with polyangiitis can range from ear fullness and otalgia to conductive or sensory neural hearing loss and sudden deafness. Cochlear implantation in these patients faces two challenges: access to the round window and control of mastoid and middle ear inflammation. The combined approach in cochlear implantation is a classic trans-facial recess approach facilitated by a trans-canal view. CASE REPORT: In this case report, we present the "combined approach" in a 20-year-old lady with granulomatosis with polyangiitis who underwent cochlear implantation …
