Towards use of Patient-informed Relevant Outcomes (PRO) in trials of bloodstream infections: a mixed-methods study protocol (BALANCE+PRO).

INTRODUCTION: Bloodstream infections (BSIs) are a major cause of morbidity and mortality worldwide. Although recent clinical trials have increasingly adopted patient-centred outcome measures, patient-informed selection of outcome measures remains limited in infectious disease research. Greater deliberate engagement and partnership with patients and families are required to ensure that outcomes reflect priorities that are meaningful to those with lived experience. Our study aims to incorporate patient-informed perspectives into outcome selection for BSI clinical trials. METHODS AND ANALYSIS: W

Surgical safety of preoperative or perioperative immune checkpoint inhibitors in colorectal cancer surgery: protocol for a systematic review and meta-analysis focusing on anastomotic leakage.

INTRODUCTION: Immune checkpoint inhibitors (ICIs) are increasingly used before colorectal cancer surgery, particularly for mismatch repair-deficient or microsatellite instability-high tumours. Their surgery-specific safety, especially the risk of anastomotic leakage (AL), remains uncertain. We aim to synthesise AL and other postoperative outcomes after preoperative or perioperative ICI therapy. METHODS AND ANALYSIS: We will search PubMed/MEDLINE, Embase, Web of Science, Scopus, CENTRAL, ClinicalTrials.gov, CNKI and the WHO International Clinical Trials Registry Platform from inception to July

Predictors and prognosis of progression of aortic valve stenosis in patients hospitalized with heart failure with mildly reduced ejection fraction.

BACKGROUND: Although aortic valve stenosis (AS) is a recognized predictor of outcomes in heart failure (HF), limited data is available concerning predictors for and the prognostic value of progressive AS in patients with HF with mildly reduced ejection fraction (HFmrEF). METHODS: From 2016 to 2022, consecutive patients hospitalized with HFmrEF at one institution were retrospectively included. AS progression was assessed during follow-up echocardiography and patients with and without progression were compared regarding the primary endpoint long-term all-cause mortality at 30 months (median fol

Understanding peer support among healthcare assistants delivering hospice care at home: a protocol for a realist review.

INTRODUCTION: Globally, the demand for community palliative care, delivered within the home setting, is rising. Hospice support workers, also referred to as healthcare assistants, play a crucial role in providing this care, but evidence indicates they face challenges relating to inadequate training, isolation and emotional labour. This realist review aims to understand how peer support interventions can support healthcare assistants in delivering hospice care at home. METHODS AND ANALYSIS: The realist review will follow a five-step process to explore the research question: (1) locating existi

From sample to clinical insight: a review of exome sequencing in disease diagnostics.

Exome sequencing (ES) has transformed genomic research and clinical diagnostics by enabling precise identification of disease-associated variants within protein-coding regions, which, while representing a minority of the genome, include many well-characterized pathogenic mutations. This review provides a comprehensive overview of ES methodology, data analysis pipelines, clinical relevance, and ethical considerations. We describe the ES workflow from DNA extraction and library preparation to target enrichment, sequencing to ES data analysis. We have also evaluated major capture technologies and

Analysis of the determinants of sleep quality in a Spanish population: a study protocol for a cross-sectional study.

INTRODUCTION: Sleep problems are a growing public health concern being related, among others, to an increased risk of cardiovascular diseases or worse cognitive functioning. In addition, they can affect aspects related to personal motivation and quality of life. However, very few studies have analysed the possible determinants of sleep quality in the adult population as a whole, establishing patterns based on these determinants. The objectives are to evaluate the determinants of sleep quality in a representative sample of the general adult population between 25 and 65 years old, and to establis

Protocol for a prospective multicenter longitudinal randomized controlled trial (CALIN) of sensory-tonic stimulation to foster parent child interactions and social cognition in very premature infants.

INTRODUCTION: Premature birth is associated with long-term somatic and neurological disorders, including cognitive, social and behavioral impairments. Moreover, the mothers of infants born preterm exhibit a higher prevalence of anxiety and depressive symptoms after birth. Early rehabilitation, developmental care, and parenting support have already been shown to have a positive impact on neurological outcome. However, no randomized controlled study has so far assessed the effects on parenting and long-term neurological outcomes of proprioceptive stimulation to trigger positive brain plasticity

Feasibility of a pharmacist-led physical health monitoring for patients on antipsychotic medications: protocol for a longitudinal study.

INTRODUCTION: Physical health conditions are the leading causes of death in people living with severe mental illness. In particular, the risk of metabolic syndrome; the constellation of abnormalities in weight, blood pressure, blood glucose and lipid levels, is high in this cohort. It has been recognised that commonly prescribed pharmacological agents for mental illness can further amplify the risk of developing metabolic syndrome; therefore, monitoring guidelines are in place for consumers prescribed antipsychotics. However, there is a disconnect between recommended guidelines and current pra

Risk factors for bleeding in haemato-oncology patients-a nested case-control study: The BITE study protocol (Bleeding In Thrombocytopenia Explained).

INTRODUCTION: Haemato-oncological patients often receive platelet count driven prophylactic platelet transfusions to prevent bleeding. However, many prophylactically transfused patients still bleed. More knowledge on risk factors for bleeding is therefore needed. This will enable identification of bleeding risk profiles on which future transfusion policy can be optimised. The present BITE study (Bleeding In Thrombocytopenia Explained) aims to identify clinical conditions and biomarkers that are associated with clinically relevant bleeding events. METHODS AND ANALYSIS: A matched case-control s

The implementation of pharmacogenomics into UK general practice: a qualitative study exploring barriers, challenges and opportunities.

Pharmacogenomics describes interpatient genetic variability in drug responses. Information based on whole genome sequencing will soon open up the field of pharmacogenomics and facilitate the use of genomic information relating to drug metabolism and drug responses. We undertook a qualitative study, aiming to explore the potential barriers, opportunities and challenges facing the implementation of pharmacogenomics into primary care. Semi-structured interviews were undertaken with 18 clinical participants (16 GPs and 2 other clinicians). All interviews were recorded and transcribed verbatim. Usi

Barriers and facilitators to buprenorphine use for opioid agonist treatment: protocol for a scoping review.

INTRODUCTION: In the context of the opioid crisis in North America, the benefits of evidence-based opioid agonist treatments such as buprenorphine/naloxone have not been optimised due to low uptake. Numerous factors contribute to the underuse of buprenorphine, and theory-informed approaches to identify and address implementation barriers and facilitators are needed. This scoping review aims to characterise the barriers and facilitators at the patient, healthcare professional, organisation and system level according to the Theoretical Domains Framework (TDF), and identify gaps to inform practic

Conducting reproductive research during a new childhood cancer diagnosis: ethical considerations and impact on participants.

PURPOSE: Research among adults shows benefits and low perceived burden of engaging in behavioral research. However, questions remain regarding the ethics of conducting behavioral research in pediatric populations during sensitive situations, including during a new life-threatening diagnosis or at end-of-life. We examined reactions to participating in a behavioral reproductive research study among male adolescents newly diagnosed with cancer and their parents, as a step towards optimizing fertility preservation utilization in a population where future infertility is common. METHODS: Pediatric

"Do you want to know or not?" How prenatal providers manage clinical uncertainty related to chromosomal risk and noninvasive prenatal testing.

In 2011, noninvasive prenatal testing (NIPT) disrupted clinical screening and testing paradigms. While concerns have been raised over public and patient-oriented NIPT usage, providers' views of NIPT remain understudied. This is significant because providers offer pre-test counseling to pregnant patients. This study sought to better understand how prenatal care providers view NIPT in the context of risk and uncertainty. After obtaining institutional ethical approval, semi-structured interviews were conducted with certified nurse-midwives and obstetrician-gynecologists (n = 20). Interviews were
