Gastrointestinal complications of cocaine use: a narrative literature review.

BACKGROUND: Cocaine is the second most widely used illicit substance worldwide and exerts potent sympathomimetic effects by inhibiting the reuptake of norepinephrine, dopamine, and serotonin. While cardiovascular and neurological complications are well recognized, gastrointestinal (GI) manifestations are increasingly reported. These arise from multifactorial mechanisms including vasospasm, endothelial dysfunction, thrombosis, ischemia, and direct mucosal toxicity, often with severe clinical consequences. OBJECTIVE: This narrative review summarizes the mechanisms, pharmacokinetics, pharmacodyn

Successful Colectomy for Hemorrhagic Colitis with Hemolytic Uremic Syndrome and Acute Encephalopathy due to Escherichia coli O157 Infection.

An 81-year-old man was admitted to a primary care hospital due to bloody diarrhea. The findings of abdominal computed tomography indicated ischemic colitis, so conservative therapy was started. On the 4th hospital day, the patient was transferred to our hospital because of renal dysfunction. Physical examination showed clouding of consciousness and abdominal distention. Abdominal computed tomography revealed massive ascites and thickening of the whole colonic wall. With a diagnosis of acute abdomen, an emergent laparotomy was performed. Extended right hemicolectomy was performed because of sev

Acute colitis associated with prolonged administration of neuroleptics.

We describe a 29-year-old patient who developed acute colitis limited to the sigmoid and left colon with features mimicking ischemic injury after a prolonged administration of trifluoroperazine and levomepromazine, two phenothiazines in association with haloperidol, another neuroleptic, and biperidene, an anticholinergic compound. The discontinuation of these drugs was followed by a prompt and complete recovery, and no other cause of acute colitis was found. The subsequent administration of sultopride, a neuroleptic from the benzamide family and then the readministration of haloperidol were we

[Problems posed by massive hemorrhages of colorectal origin. Apropos of 12 case reports].

Urgent surgical operation to treat massive hemmorrhage of colorectal origin was necessary in 12 patients: --colon diverticulosis: 7 cases, --cecal angiodysplasia: 1 case, --pancreatocolic fistula: 1 case, --ischemic colitis: 2 cases, --post-radiation therapy rectitis: 1 case. Lesions exclused from discussion in this report and provoking hemorrhage were colorectal tumors, chronic inflammatsry colitis, rectosigmoid angiomatosis, and post-traumatic or iatrogenic lesions. After a definition of massive hemmorrhage based on pre-operative transfusional requirements, the exploratory procedures necessa

Delayed presentation of an inflammatory polyp following colonic ischemia.

Inflammatory polyps are usually associated with inflammatory bowel disease. The majority of these lesions are asymptomatic, however, they may be a source of obstruction or hemorrhage. These polyps represent re-epithelialization of the damaged colonic mucosa. Although inflammatory polyps have been associated with the acute phase of ischemic colitis, there has been only one report of delayed presentation of a symptomatic inflammatory polyp following ischemic injury to the colon. We present the second such report and a review of the literature.

Cocaine colitis. Is this a new syndrome?

An unusual case of colitis in a 37-year-old cocaine addict is described. The patient presented with right-sided abdominal pain and diarrhea exacerbated by his use of cocaine. Significant antibiotic ingestion was denied. At laparotomy, an edematous cecum and ascending colon were found, the cut surface of which revealed diffuse superficial ulcerations and yellowish fibrinous material. Microscopic examination demonstrated findings consistent with pseudomembranous colitis with an ischemic component. A mechanism involving catecholamine-induced mucosal ischemia is postulated to explain the findings

Diarrhea-predominant irritable bowel syndrome treatment options eluxadoline, rifaximin, and alosetron: analysis of the FDA adverse event reporting system (FAERS) database.

BACKGROUND: Adverse events (AEs) suspected to be associated with the three FDA approved medications (eluxadoline, rifaximin, and alosetron) for diarrhea-predominant irritable bowel syndrome (IBS-D) were examined. RESEARCH DESIGN AND METHODS: We analyzed all reports in the FDA Adverse Event Reporting System (FAERS) database from each medication's date of FDA approval through 30 June 2024. Reports were excluded if they contained other suspected medications or had a reason for use outside of IBS and/or diarrhea. RESULTS: Eluxadoline was associated with 1,002 AEs, most commonly abdominal pain (n

Preoperative Diagnosis of Idiopathic Myointimal Hyperplasia of Mesenteric Veins in a Post-Liver Transplant Patient.

Idiopathic myointimal hyperplasia of the mesenteric veins is a rare cause of ischemic colitis resulting from luminal obliteration of medium- to large-sized mesenteric veins, which may mimic inflammatory bowel disease and infectious colitis, typically diagnosed following surgical resection. Delays in diagnosis are common and often preceded by extended hospitalizations and failed medical therapies including antibiotics and anti-inflammatories. Here, we present a rare instance of the preoperative diagnosis of idiopathic myointimal hyperplasia of the mesenteric veins in a woman who had previously

CD8+ Lymphocyte Infiltration Is a Specific Feature of Colitis Induced by Immune Checkpoint Inhibitors.

BACKGROUND: Immune checkpoint inhibitors (ICPIs) have revolutionized cancer therapy, although immune-related adverse events (irAEs) remain a serious issue. The clinical characteristics of colitis induced by ICPIs are very similar to inflammatory bowel disease. Recently, cluster of differentiation 8 positive (CD8+) lymphocyte infiltration into organs has been associated with the onset of irAEs. The present study compared the histological infiltration of CD8+ lymphocytes in irAE colitis with that in other colitis. METHODS: Newly diagnosed and untreated patients were retrospectively enrolled. Surgical Management of Severe Colitis in the Intensive Care Unit.

Severe colitis, an umbrella encompassing several entities, is one of the most common acute gastrointestinal disorders resulting in critical illness. Clostridium difficile infection is responsible for the majority of nosocomial diarrhea with fulminant C difficile colitis (CDC) carrying a high mortality. Optimal outcomes can be achieved by early identification and treatment of fulminant CDC, with appropriate surgical intervention when indicated. Ischemic colitis, on the other hand, is uncommon with a range of etiological factors including abdominal aortic surgery, inotropic drugs, rheumatoid dis

[Hemorrhagic colitis due to Escherichia coli O157 infection in a patient with Takayasu's arteritis].

Escherichia coli (E coli) O157 may cause abdominal pain and diarrhea followed by hematochezia. Most of cases resolve spontaneously after several days. Takayasu's arteritis affects medium- and large-sized arteries, aortic arch and its branch, and rarely affects inferior mesenteric artery. In case of Takayasu's arteritis with hematochezia, we must distinguish among ulcerative colitis, ischemic colitis, and infectious colitis with Takayasu's arteritis. We report a case of 17-year-old woman who suffered from hemorrhagic colitis by E. coli O157, and combined with leg claudication and abdominal pain…

Pseudomembranous colitis.

Pseudomembranous colitis is an inflammatory condition of the colon characterized by elevated yellow-white plaques that coalesce to form pseudomembranes on the mucosa. Patients with the condition commonly present with abdominal pain, diarrhea, fever, and leukocytosis. Because pseudomembranous colitis is often associated with C. difficile infection, stool testing and empiric antibiotic treatment should be initiated when suspected. When results of C. difficile testing are negative and symptoms persist despite escalating empiric treatment, early gastroenterology consultation and lower endoscopy wo

Colo-rectal biopsy in inflammatory bowel disease.

The term inflammatory bowel disease (IBD) is viewed as all-inclusive, covering the full panoply of intestinal disorders in which inflammatory changes are a prominent feature, including those of infectious, toxic, and intrinsic origin as well as the idiopathic entities ulcerative colitis and Crohn's disease. This chapter describes and discusses those aspects of colo-rectal biopsy in IBD which can help pathologists make optimal interpretations. The areas covered are: 1) methods used to prepare biopsy specimens for study, 2) normal histologic findings and common artefacts, 3) basic pathologic cha

Fulminant Toxic Megacolon Without Identifiable Underlying Cause Requiring Emergent Total Colectomy: A Case Report.

Toxic megacolon is a life-threatening condition characterized by acute colonic dilation and systemic toxicity, most commonly associated with inflammatory or infectious colitis. However, ischemic colitis represents an important and underrecognized etiology in elderly patients with significant vascular comorbidities. We report the case of a 71-year-old male with a history of long-standing hypertension, type 2 diabetes mellitus, and chronic kidney disease, receiving losartan, nifedipine, and insulin therapy, who presented with five days of progressive diffuse abdominal pain, severe distension, ob

Laninamivir-induced ischemic enterocolitis: A case report.

BACKGROUND: Neuraminidase inhibitor-associated acute hemorrhagic colitis is rare. We report a case of ischemic enterocolitis that was likely caused by laninamivir. CASE SUMMARY: A 54-year-old female patient with influenza type A was administered 40 mg of laninamivir via inhalation once. On the same day, the patient experienced bloody stools and lower abdominal pain. A contrast-enhanced abdominal computed tomography showed edema-like changes from the descending colon to the sigmoid colon, which suggested ischemic enterocolitis. CONCLUSION: We treated a patient with ischemic enterocolitis caus

[Prerequisites for therapy of inflammatory bowel diseases. Pathophysiology--symptomatology--diagnosis].

The etiology of the idiopathic chronic inflammatory bowel diseases Crohn's disease and ulcerative colitis remains unclear. Differences in prognosis, medical and surgical treatment, and the nature of expected complications require not only a differential diagnosis vis-a-vis other disorders, but also the correct diagnosis of these two diseases. Symptomatology and clinical presentation can be mimicked both by infectious, ischemic and other chronic bowel diseases such as collagenous colitis, eosinophilic colitis, and Behçet's disease. This means that, in the absence of pathognomonic changes, the c

The clinical significance of focal active colitis.

Focal crypt injury by neutrophils (cryptitis/crypt abscesses), or focal active colitis (FAC), is a common isolated finding in endoscopic colorectal biopsies. Focal active colitis is often thought of as a feature of Crohn's disease, but may also be seen in ischemia, infections, partially treated ulcerative colitis, and as an isolated finding in patients undergoing endoscopy to exclude neoplasia. Clinical, endoscopic, and pathological data were retrospectively reviewed from 49 patients with focal active colitis, who had no other diagnostic findings on colorectal biopsy and no history of chronic

Idiopathic mesenteric phlebosclerosis combined with melanosis coli in a 51-year-old female.

Idiopathic mesenteric phlebosclerosis (IMP) is a rare ischemic colitis characterized by calcification of mesenteric veins and submucosal veins of the colon. Melanosis coli (MC) is a pigmented mucosal lesion comprising macrophages in the lamina propria of the colorectal mucosa that contain lipofuscin. This study reports a case of IMP combined with MC.Clinicians should consider medication history, bowel preparation, and thorough observation to prevent missed IMP diagnosis when coexisting with MC.
